Slow pathway modification has become the mainstay for the treatment of atrio-ventricular nodal re-entrant tachycardia (AVNRT) ablation because of high success rate and low incidence of complications. Our patient had a rare complication of slow pathway modification by radiofrequency ablation (RFA) in form of delayed complete heart block, occurring 10 days after the procedure and resolving in 6 weeks to normal conduction. Complete AV block is a rare immediate complication of RFA but can present weeks later. Transient atrio-ventricular (AV) block during the procedure is seen in all patients who develop delayed AV block and these patients should be monitored closely.  相似文献

Cardiovascular disease is one of the leading causes of death in the United States and also a major disease nationwide. Over 700,000 coronary artery bypass graft (CABG) procedures are performed annually all around the world, of which 350,000 are performed in the United States. The use of mechanical stabilizers to isolate and immobilize the surface region of the heart is not without its limitations such as hemodynamic deterioration, and arrythmia induction requiring inotropic support. Consequently, the use of mechanical stabilizers leads to a poor immobilization of the surgical field in spite of significant forces of traction and retraction used with these devices. The primary goal of this research is to develop effective haptic (sense of touch) and visual servoing methods with the long-term goal of eliminating the need for mechanical stabilizers and extracorporeal support for CABG procedures. We present in this paper the results from our initial work in the area of tracking a deformable membrane using vision and providing haptic feedback to the user, based on the visual information through the vision hardware and the material properties of the membrane. In our first experiment, we track the deformation of a rubber membrane in real-time through stereovision while providing haptic feedback to the user interacting with the reconstructed membrane through the PHANToM haptic device. In the second experiment, we verify the ability of our vision system to track a point on a surface undergoing a complex 3D motion.  相似文献

OBJECTIVE: To review the history, development, and current applications of robotics in surgery. BACKGROUND: Surgical robotics is a new technology that holds significant promise. Robotic surgery is often heralded as the new revolution, and it is one of the most talked about subjects in surgery today. Up to this point in time, however, the drive to develop and obtain robotic devices has been largely driven by the market. There is no doubt that they will become an important tool in the surgical armamentarium, but the extent of their use is still evolving. METHODS: A review of the literature was undertaken using Medline. Articles describing the history and development of surgical robots were identified as were articles reporting data on applications. RESULTS: Several centers are currently using surgical robots and publishing data. Most of these early studies report that robotic surgery is feasible. There is, however, a paucity of data regarding costs and benefits of robotics versus conventional techniques. CONCLUSIONS: Robotic surgery is still in its infancy and its niche has not yet been well defined. Its current practical uses are mostly confined to smaller surgical procedures.  相似文献

In the last decade, advances in wireless and sensor technologies, and the implementation of telemedicine, have led to innovative digital health care for cardiac patients. Continuous monitoring of patients’ biomedical signals, and acute changes in these signals, may result in timely, accurate diagnoses and implementation of early interventions. In this review, we discuss commonly used wireless and leadless cardiac devices including pulmonary artery pressure sensors, implantable loop recorders, leadless pacemakers and subcutaneous implantable cardioverter-defibrillators. We discuss the concept and function of each device, indications, methods of delivery, potential complications, consideration for implantation, and cost-effectiveness.  相似文献

Vancomycin-resistant enterococci (VRE) are a growing health problem, and uncertainties exist regarding the optimal therapy for bloodstream infection due to VRE. We conducted systematic comparative evaluations of the impact of different antimicrobial therapies on the outcomes of patients with bloodstream infections due to VRE. A retrospective study from January 2008 to October 2010 was conducted at Detroit Medical Center. Unique patients with blood cultures due to VRE were included and reviewed. Three major therapeutic classes were analyzed: daptomycin, linezolid, and β-lactams. Three multivariate models were conducted for each outcome, matching for a propensity score predicting the likelihood of receipt of one of the therapeutic classes. A total of 225 cases of bacteremia due to VRE were included, including 86 (38.2%) cases of VR Enterococcus faecalis and 139 (61.8%) of VR Enterococcus faecium. Bacteremia due to VR E. faecalis was more frequent among subjects treated with β-lactams than among those treated with daptomycin or linezolid. The median dose of daptomycin was 6 mg/kg of body weight (range, 6 to 12 mg/kg). After controlling for propensity score and bacteremia due to VR E. faecalis, differences in mortality were nonsignificant among the treatment groups. Therapy with daptomycin was associated with higher median variable direct cost per day than that for linezolid. This large study revealed the three therapeutic classes (daptomycin, linezolid, and β-lactams) are similarly efficacious in the treatment of bacteremia due to susceptible strains of VRE.  相似文献
