Komplikationsmanagement nach winkelstabiler Plattenosteosynthese am proximalen Humerus

PURPOSE: In this prospective study, complications observed after angularly stable proximal humerus plate fixation (locking proximal humerus plate) were analysed by deriving specific therapies.PATIENTS: Fifty patients (median age 65 years, range 25-84 years, 39 female, 11 male) with displaced proximal humerus fractures (seven single, 36 double, and seven triple fractures) were evaluated 3, 6, and 12 months after angularly stable plate fixation using a standard protocol.RESULTS: Age- and gender-matched median constant scores 12 months postoperatively showed for the three fracture types 86, 87, and 55 points and complication rates of 14, 19, and 100%, respectively. There were seven primary and seven secondary implant displacements, five humeral head necroses, four osseous malalignments, two nonunions, two deep infections, and one heterotopic ossification. Nine reoperations were required in six patients: implant removal (n=3), reosteosynthesis (n=2), revision because of nonunion (n=2), and deep infection (n=2).CONCLUSIONS: Differentiated analysis of complications and the development of specific prevention and therapeutic strategies considering surgical technique, implant, fracture morphology, and humeral head perfusion minimize the rate of complications observed after angularly stable locking proximal humerus plate fixation.

In recent years, wavefront analysis has ceased to be purely a laboratory application and emerged as a method used in ophthalmological diagnosis. This development has been promoted mainly by the widespread use of wavefront-guided LASIK (laser in situ keratomileusis). However, aberrometry is still not a common diagnostic technique, and for many ophthalmologists interpretation of the results is difficult. The second part of this serial paper reviews findings that are relevant for the ophthalmological community and highlights current scientific applications in this area.     

Gallbladder dyskinesia in chronic acalculous cholecystitis

To test the hypothesis that there is an early stage of cholesterol gallstone formation in man characterized by symptoms of chronic cholecystitis, poor gallbladder emptying, and biliary cholesterol crystals, we studied cholecystokinin-stimulated gallbladder emptying by DISIDA scintigraphy and examined bile for cholesterol crystals in symptomatic patients with normal oral cholecystography and gallbladder sonography. Of 36 patients studied, 16 had biliary cholesterol crystals; their mean 30-min gallbladder ejection fraction was 25.9±14.8%. Among the 20 patients without crystals, the mean ejection fraction was 60.3±23.3%. Fifteen patients, 11 with crystals and four without, had cholecystectomy because of persistent symptoms. All with crystals preoperatively and three without had chronic cholecystitis histologically. One patient without crystals had normal histology. We conclude that poor gallbladder contractility, well-established as an etiologic factor in animal models of cholesterol cholelithiasis, is now linked to acalculous cholecystitis, an early stage of human cholesterol cholelithiasis.     

Auditive Verarbeitungsgeschwindigkeit und Sprachleistungen

ZusammenfassungHintergrund Lernstörungen werden zunehmend auf Defizite bei der Sprachwahrnehmung und diese wiederum auf eine Verlangsamung der auditiven Verarbeitungsgeschwindigkeit zurückgeführt. In der Studie wird untersucht, ob sich bei Kindern mit Schulproblemen sprachliche Leistungen durch ein Zeitverarbeitungstraining verbessern lassen.Patienten und Methoden Bei 21 Kindern mit spezifischen Lernstörungen wurden über 8 Wochen Ordnungsschwelle und Richtungshören trainiert und im Vergleich zu einer Kontrollgruppe (n=21) Sprachleistungen bis 6 Monate danach überprüft.Ergebnisse Leistungssteigerungen wurden hinsichtlich der Ordnungsschwelle, nicht aber der sprachlichen Fähigkeiten beobachtet. Die Eltern schätzten die Wirkung des Trainings eher positiv ein.Schlussfolgerungen Ein Zeitverarbeitungstraining verbessert unmittelbar geübte Leistungen, nicht aber sprachliche Fähigkeiten. Von den Eltern wahrgenommene Verbesserungen sind nicht objektivierbar und als unspezifische Effekte zu bewerten. Die Ergebnisse sprechen insgesamt gegen die Wirksamkeit eines Zeitverarbeitungstrainings bei Kindern mit Schulschwierigkeiten.     

Morbidität nach Herztransplantation

The morbidity in the long-term course following heart transplantation in childhood is mainly determined by the morbidity of the transplanted graft, by side effects caused by immunosuppression and by psychosocial morbidity due to the special situation of life and growing up with a transplanted heart. Transplant vasculopathy as a specific disease of the transplanted organ itself, is a common complication following heart transplantation and is an important factor of morbidity and mortality, considerably limiting the long-term prognosis. Progressive disturbance of renal function and cumulative incidence of malignant tumors is a further factor limiting prognosis caused by the side effects of immunosuppression.

Akzessorisches Skrotum

Zusammenfassung Der Unterschied zwischen ektopem und akzessorischem Skrotum wird dargelegt. Bei Letzterem findet sich definitionsgemäß ektope Skrotalhaut ohne Nachweis von testikulärem Gewebe bei normal angelegten Genitalorganen. Es werden die häufigere perineale von der selteneren lateralen Form differenziert. Neben einer Literaturübersicht werden 2 Kinder mit dieser extrem seltenen Malformation vorgestellt. Der erste Fallbericht schildert ein Kind mit einem ungewöhnlich lokalisierten akzessorischen Skrotum auf dem linken proximalen Oberschenkel bei ansonsten unauffälligem primärem männlichem Genitale. In der Mehrzahl der publizierten Fälle von Knaben mit einem akzessorischen Skrotum befinden sich diese perineal in der Mittellinie. Die zweite Fallbeschreibung zeigt einen solchen Patienten mit einem Skrotalanhängsel in der Medianlinie mit einem chromosomalen Mosaik 46,XY (10%)/47,XYY (90%) und Klumpfüßen beidseits. Die Therapie besteht in der kompletten Exzision des akzessorischen Skrotums mit plastischer Defektdeckung bzw. in der Resektion des perinealen Tumors mitsamt der ektopen Skrotalhaut und einer Perineoplastik. Die histologische Untersuchung ergab Skrotalhaut mit unauffälligem subkutanem Fettgewebe (Fall 1) respektive ein fibröses kindliches Hamartom (Fall 2).
