Carcinoma arising from an epiphrenic diverticulum: a frequently misdiagnosed disease.

Epiphrenic diverticulum is rare, comprising of about 10% of all esophageal diverticula. Carcinoma arising within such a diverticulum is even less common. We report on two patients with squamous cell carcinoma arising from an epiphrenic diverticulum. Some features of malignant change related to the epiphrenic diverticula were misdiagnosed or missed in our cases and other reports due to lack of clinical experience. It is important to exclude the possibility of malignancy as this is vital for surgical planning. Although the prognosis for patients with malignancy arising from an epiphrenic diverticulum is generally poor, a high index of suspicion may increase the chance of cure.     

咬肌与颅面形态的关系

目的探讨咬肌与颅面形态之间的关系，了解咬肌体积与邻近骨骼结构的大小及形态的相关性。方法对40例要求改变脸型者进行磁共振成像（MRI）测量，人体测量，头部正位、侧位和下颌骨曲面断层X线检查，测量并计算咬肌体积（MsV），测量头长（HL）、头宽（HB）、面长（FL）、面宽（FB）、下颌角间宽（IB）、下颌骨体长（CL）、下颌角切线长（MAL）、下颌角角度（JA），计算颜面形态指数FI（FL／FB）、头颅指数C（IHB／HL）。用SPSS11．5软件统计分析MsV与HI。HB、FL、FB、IB、CL、MAL、JA、FI、CI之间的相关性。结果MsV与JA呈明显负相关，与CL、MAL、IB、FB、HL呈明显正相关，与CL、FI、HB无明显相关性。结论咬肌肥大可能是方脸面型的成因之一，是影响面型的重要因素。     

绝经后骨质疏松症中药治疗研究文献的质量评价

目的 了解目前国内中药治疗绝经后骨质疏松症的临床治疗研究的现状。方法 对检索到的50篇有关中药治疗绝经后骨质疏松症的临床研究论文，按照牛津循证医学中心证据水平标准和临床试验的没计原则，从诊断标准、纳入和排除标准、组间基线可比性、随机、对照、双盲、统计学方法、疗效判断、疗程及药物的不良反应、随访等多个方面进行分析评价。结果 50篇相关文献中，按照牛津循证医学中心证据水平标准评价，推荐级别为B级的占44.0％，C级的占56.0％；证据水平为2b级的占40.0％，3b级的占4.0％，4级的占56.0％；随机对照试验文献占44.0％；临床对照试验文献占4.0％；采用随机双盲者占9.1％，随机单盲者占18.2％，说明组间基线可比的占31.8％，有诊断标准的占90.9％，有纳入和排除标准的占86.4％，说明统计方法的占52.4％，有客观疗效评价指标的占90.9％，说明药物不良反应的占9.1％。结论 有关中医药治疗绝经后骨质疏松症的临床研究论文日益增多，但随机对照试验比例偏低，研究设计及论文撰写水平有待进一步提高。     

Interleukin 17-Producing γδT Cells Increased in Patients with Active Pulmonary Tuberculosis

Although it has been known that y8 T cells may play an important role in the immune response to infection of Mycobacterium tuberculosis (M. tb), the mechanisms by which the T8 T cells participate in the innate and/or acquired immunity to tuberculosis (TB) have not been full elucidated. In the present study, 27 patients with active pulmonary TB and 16 healthy donors (HD) were performed. We found that proportion of IL-17-producing cells among lymphocyte was similar between TB patients and HD, whereas the proportions of γδ T cells in IL-17- producing cells (59.2%) and IL-17-producing cells in γδ T cells (19.4%) in peripheral blood were markedly increased in TB patients when compared to those in HD (43.9% and 7.7%, respectively). In addition, the proportions of IFN-γ-producing γδ T cells in TB patients were obviously lower than that in HD. Upon re-stimulated with M. tb heat-treated antigen (M. tb-HAg) in vitro, fewer IL-17-producing γδ T cells were generated from HD and TB patients, whereas IFN-γ-producing γδ T cells were increased in TB patients compared to that in HD. Our findings in TB patients and healthy human were consistent with other murine investigation that the IL-17- producing γδ T cells were main source of IL-17 in mouse model of BCG infection, suggesting that γδ T cells might be involved in the formation of tubercular granuloma in pulmonary TB patients, but need further identification. Sonographic diagnosis of early recurrence after inguinal herniorrhaphy: Potential pitfalls

Three patients who presented with scrotal swelling within a few days of inguinal herniorrhaphy are reported. Ultrasonography scans performed in these patients all demonstrated features suspicious of recurrence of hernia. One patient underwent surgical exploration, which revealed only a scrotal haematoma without evidence of recurrent hernia. The other two patients were managed conservatively because clinically the swellings were regarded to be more compatible with haematoma. Both patients had subsequent resolution of the scrotal swelling with no clinical evidence of recurrence of hernia on follow‐up. It is concluded that sonographic diagnosis of recurrence of hernia shortly after inguinal herniorrhaphy can occasionally be misleading.     

持续不卧床腹膜透析患者的自我管理教育

目的探讨门诊腹膜透析患者自我管理教育的方法。方法对82例门诊腹膜透析患者进行自我管理教育,包括集体教育活动、讨论小组和个别指导等多种形式,责任护士随访评估。结果患者尿量、总液体清除量、肌酐清除率干预前后比较,差异有显著性意义（P〈0.05,P〈0.01）;干预前后患者睡眠质量及主观、客观生活质量显著提高,临床症状显著缓解（均P〈0.01）。结论以患者为中心的自我管理教育可提高门诊腹膜透析患者的生活质量。     

Early indices of warm ischemic damage in autotransplanted canine kidneys preserved by simple hypothermic storage

After measurement of normal renal function, dog kidneys (n = 52) were subjected to 3, 15, or 30 min of normothermic warm ischemia (WI). After 24 hr of preservation by simple hypothermic storage (SHS) in a modified Collins solution, autotransplant was done and renal function was again measured beginning at 1 hr. Compared with preharvest values, kidneys with minimal (3 min) WI had significantly decreased clearances of creatinine (Cr) and para-aminohippuric acid (PAH), PAH extraction, absolute and fractional Na reabsorption, Na excretion, and urinary Na concentration; no change in urine flow rate or K excretion; and significantly increased fractional excretion of Na, K, and H2O. Compared with minimal WI, 30-min WI produced further significant decreases in clearances of Cr, PAH, and PAH extraction; and further increases in fractional excretion of Na, K, and H2O. Urine flow was also decreased by about half and urine Na concentration rose significantly. Several parameters were very significantly correlated with the length of WI, but the most reliable index was the fractional reabsorption of Na. When several functional parameters were used together, kidneys with significant (30 min) WI prior to preservation could be identified with a high degree of statistical reliability.
