Antianginal and antiarrhythmic long term therapy with amiodarone may be associated with side effects, therefore it should be used mainly in short term treatment of severe arrhythmias and acute coronary insufficiency. It is important to assess if any inotropic effect may be produced after intravenous administration of this drug in commonly accepted therapeutic doses (5 mg/kg body weight). To investigate this possibility we studied the effects of amiodarone on blood pressure (BP), on heart rate (HR) and on the maximal velocity of circumferential fiber shortening (Vcf Max). Simultaneous echocardiogram (UCG), electrocardiogram (lead DII) (ECG) and BP cuff measurement were performed on 12 subjects without cardiomegaly and clinical evidence of heart failure, immediately before a 30 second intravenous injection of amiodarone and every 30 seconds over a period of 6 minutes after drug administration. Amiodarone administration markedly raised HR within the first 30 seconds from the beginning of the injection and concomitantly decreased diastolic BP. No significant lowering of systolic BP was observed. Vcf Max (circ/sec) raised during the test concomitantly with HR increase, showing a significant relationship between left ventricular performance and HR. The same was also true during atrial pacing performed on one subject. No significant changes in any of the parameters studied were demonstrated after placebo (saline solution) administration to two presumable healthy subjects. Amiodarone does not seem to have any positive or negative intrinsic inotropic effect when administered intravenously at a dose of 5 mg/kg body weight. 相似文献

Clinical toxicity is related to bleomycin pharmacology and renal function in patients with advanced squamous carcinoma of the cervix. Adverse effects were more frequent in patients with severe renal impairment than in patients with adequate renal function. Pharmacokinetic data were obtained in four patients. This information suggests treatment guidelines for the use of bleomycin in patients with renal failure that may reduce the risk of excessive adverse effects. Bleomycin dose reduction of 75% and administration not more often than every 3 days may permit the use of this drug in patients with creatinine clearances of less than 25 ml/min in whom alternative chemotherapy with cis-platinum is not possible. 相似文献

BACKGROUND: The purpose of this prospective Phase II study was to analyze morbidity and mortality of cytoreductive surgery (CRS) and intraperitoneal hyperthermic perfusion (IPHP) in the treatment of peritoneal surface malignancies. METHODS: A total of 205 patients (50 with peritoneal mesothelioma, 49 with pseudomyxoma peritonei, 41 with ovarian cancer, 32 with abdominal sarcomatosis, 13 with colon cancer, 12 with gastric cancer, and 8 with carcinomatosis from other origins) underwent 209 consecutive procedures. Four patients underwent the intervention twice because of disease relapse. There were 70 men and 135 women. Mean age was 52 years (range, 22-76 yrs). CRS was performed by using peritonectomy procedures. IPHP through the closed abdomen technique was conducted with a preheated (42.5 degrees C) perfusate containing cisplatin + mitomycin C or cisplatin + doxorubicin. RESULTS: Major morbidity rate was 12%. The most significant complications were 23 anastomotic leaks or bowel perforations, 4 abdominal bleeds, and 4 sepses. Operative mortality rate was 0.9%. On logistic regression model multivariate analysis, extent of cytoreduction (odds ratio [OR], 2.88; 95% confidence interval [CI], 1.29-6.40) and dose of cisplatin for IPHP > or = 240 mg (OR, 3.13; 95% CI, 1.24-7.90) were independent risk factors for major morbidity. Ten patients presented with Grade 3 to 4 toxicity. CONCLUSIONS: CRS + IPHP presented acceptable morbidity, toxicity, and mortality rates, all of which support prospective Phase III clinical trials. 相似文献

Titanium is the most widely used material for dental implants. The natural formation, in presence of oxygen, of different oxide films (passivation films) is correlated to titanium implant biocompatibility, resistance to corrosion and is responsible for implant bacteriostatic action. Surface roughness is another surface property of Ti-implants that, affecting implant-to-bone contact, improves integration. In the present study data concerning composition, surface roughness and biocompatibility of Ghimas implants and mini-implants undergoing sandblasting with Calcium Magnesium Carbonate (CaMg(CO3)2) are reported. AFM, SEM/EDX, XRD analyses and morpho-functional tests (MTT and ALP) were performed. Cell actin cytoskeletal modification (fluorescence phalloidin staining) was also observed with confocal laser microscopy (CLSM). Data related to surface geometry and chemical properties, associated with evidence of high purity of all the tested materials (XRD and EDX), highlighted the elevated biocompatibility of tested implants and mini-implants. CLSM investigation confirmed osteoblast features of an active cell behavior able to fit cell to chemico-mechanical stimuli present at the bone/implant interface and suggests an effective implant/alveolar bone integration in vivo. 相似文献

Humoral and cellular immune responses have been shown to play a fundamental role in controlling simian and/or simian-human immunodeficiency virus (SIV-SHIV) replication in infected macaques. Therefore, the appropriate induction of both compartments of the immune system should be elicited after immunization. In this context, viral vectors have been proven effective in inducing both humoral and cellular immune responses during immunization protocols after direct injection in vivo. Among them, recombinant self-inactivating lentiviral vectors represent a useful strategy for vaccine development because they efficiently transduce and express foreign genes into a wide variety of mammalian cells. Here we report on the development and evaluation of a self-inactivating HIV-based lentiviral vector expressing a codon-optimized SIV Gag sequence (TY2-SIVGagDX), which when used to transduce dendritic cells mediated in vitro expansion of Gag-specific T cells derived from an SHIV-infected cynomolgus monkey, as measured by interferon (IFN)-gamma enzyme-linked immunospot (ELISPOT) and (51)Cr release standard assays. To evaluate the ability to elicit specific immune responses in vivo, TY2-SIVGagDX was also employed in a vaccination protocol after a single intramuscular injection in BALB/c mice. Results indicated that the vector was able to efficiently induce both cellular and humoral responses, as measured by IFN-gamma ELISPOT assay and antibody production. These data further confirm that lentiviral vectors encoding viral genes represent an advantageous delivery system for vaccine development. 相似文献
