A case of mandibular fracture surgically consolidated in a wrong position resulting in craniomandibular disorders is reported. The inadequate surgical alignment of the healed bony segments caused a malocclusion. This changed the original neuromuscular system such that compensatory mechanisms began to change the whole balance of the organism. The patient presented a mandibular crossbite, an asymmetry of the face, and extensive alteration of muscular, articular, and postural function. The bony malunion and malocclusion were treated using an interdisciplinary surgical-orthodontic treatment for correcting functional disorders and aesthetic deformity. Electromyography and computerized mandibular scanning were used to evaluate improvement of the muscular activity, during rest and function, and of the mandibular kinesiology. Timing of surgical treatment and adequate fixation and immobilization of fracture segments are very important to avoid complications such as infection, delayed union, nonunion, malunion, skeletal discrepancies, nerve injury, and (rarely) ankylosis. The surgical approach should be based on the general criteria of traumatologic therapy, restoring the original bone shape and the right occlusal relations as soon as possible.  相似文献

The present investigation was designed to study the protective effect given by thyroid hormone (TH) on root resorption: (1) whether intra-peritoneal versus oral TH administration had the same efficiency; and (2) whether this effect involved local or systemic mechanisms. For this purpose, circulating T3 levels, systemic alkaline phosphatase (APase) activity, and 5'deiodinase (5'D) activity were evaluated in the periodontal area of 80 Sprague-Dawley rats, 8 weeks of age, in which orthodontic appliances had been inserted. The results showed that TH-treated animals (intra-peritoneal or oral) had significantly less force-induced root resorptive lesions compared with a control group, without apparent changes in T3 or APase levels, and that periodontal remodelling was accompanied by a significant increase in local T3 generation as a result of T4 deiodination. This 5'D activity was higher in those animals that received exogenous TH. These results suggest that this protective TH mechanism may be achieved at a local level and that administration of low doses of TH may play a protective role on the root surface either during orthodontic treatment or in those patients that present spontaneous root resorptive lesions.  相似文献

The aim of this study was to measure the demineralization capacity of 37% phosphoric acid on bovine enamel at different time-points after bleaching with 30% carbamide peroxide. Five, 4 × 4-mm sections were obtained from the enamel of 10 bovine incisors. After applying 30% carbamide peroxide (Vivastyle) for 90 min, specimens were stored in artificial saliva for 0, 24, 72 h, or 7 d and then immersed in 37% phosphoric solution. At 15, 30, 60, 90, and 120 s, 5-ml aliquots were extracted. A control group of specimens was not bleached. Ca2+ concentrations were measured by atomic absorption spectrophotometry. A larger amount of Ca2+ was extracted from enamel by phosphoric acid after the application of 30% carbamide peroxide. Twenty-four hours after bleaching, significantly more Ca2+ was extracted from bleached than from control specimens at all time-points, and this greater susceptibility to the action of the acid persisted for at least 1 wk after bleaching.  相似文献

The adsorption of hypoxanthine on a mercury electrode from sulfate solutions 0.2 and 0.5 M at pH 2.0 and 5.0 is studied. Differential capacity, zero charge potential and maximum surface tension measurements are used to establish the characteristics of the dilute layer. Condensed film formation is reported for first time, detected from differential capacity data at high hypoxanthine concentrations in solutions at pH 5.0. The data for the dilute layer conform to a Frumkin isotherm, contrary to previous findings about a Langmuir isotherm. The data are also analysed following the Nikitas approach and a value for the size ratio parameter close to one is obtained. The discussion in terms of Esin-Markov effect and electrosorption valency in comparison with other aromatic compounds allows some conclusions to be drawn about the orientation of the molecule, the role played by electrostatic and π-electron interactions and the effect of intermolecular interactions.  相似文献

Because the risk of dental caries increases with the use of orthodontic appliances and its control cannot depend only on the patient's self-care, this study evaluated the effect of a glass ionomer cement on reducing enamel demineralization around orthodontic brackets. Fourteen orthodontic patients were randomly divided into 2 groups of 7; they received 23 brackets fitted to their premolars, bonded with either Concise (3M Dental Products, St Paul, Minn), a composite resin (control group), or Fuji Ortho LC (GC America, Chicago, Ill), a resin-modified glass ionomer cement (experimental group). The volunteers lived in a city that has fluoridated water, but they did not use fluoridated dentifrices during the study. After 30 days, the teeth were extracted and longitudinally sectioned; in the enamel around the brackets, demineralization was assessed by cross-sectional microhardness. The determinations were made at the bracket edge cementing limits, and at occlusal and cervical points 100 and 200 microm away from them. In all of these positions, indentations were made at depths from 10 to 90 microm from enamel surface. Analysis of variance showed statistically significant effects for position, material, depth, and their interactions (P<.05). The Tukey test showed that the glass ionomer cement was statistically more efficient than the control, reducing enamel demineralization in all analyses (P<.05). The use of glass ionomer cement for bonding can be encouraged because it decreases the development of caries around orthodontic brackets.  相似文献

BACKGROUND: The aim of this study was to compare the response to conventional periodontal treatment between patients with or without type 2 diabetes mellitus from a clinical and metabolic standpoint. METHODS: A prospective, parallel, comparative longitudinal clinical study was performed between type 2 diabetics and non-diabetics with moderate generalized chronic periodontitis. The study period was 6 months. Conventional periodontal scaling and root planing were performed, and the response to this treatment was compared between the groups at 3 and 6 months, measuring the plaque index, bleeding on probing, probing depth, level of clinical attachment, and gingival recession. In the diabetic patients, the clinical response was related to measurements of HbA1c and glucose in blood at 3 and 6 months. RESULTS: An improvement in all clinical variables was observed, with no statistically significant differences between the groups, with the exception of probing depth (P <0.0207). The improvement observed in blood HbA1c levels confirmed a positive metabolic response to periodontal treatment, with a lower value for this variable at each measurement time. CONCLUSIONS: Both groups of patients showed a clinical improvement after basic non-surgical periodontal treatment. The diabetic patients showed improved metabolic control (lower HbA1c) at 3 and 6 months after periodontal treatment.  相似文献

The purpose of this study was to assess the clinical performance of bonded composite (Excite/Tetric Ceram - Vivadent) versus a resin-modified glass ionomer cement (Vitremer - 3M) for restoring non-carious cervical lesions. A total of 70 restorations (thirty-five per material) were placed in 30 patients, 18-50 aged, by one operator. Rubber dam was employed in all cases, lesions were pumiced, enamel margins were not beveled, and no mechanical retention was placed. The restorations were directly assessed by two independent evaluators using modified-USPHS criteria for six clinical categories. The ratings for clinical acceptability restorations (alfa plus bravo) were as follows (Tetric Ceram/Vitremer): retention (86%/100%), marginal integrity (100%/100%), marginal discoloration (100%/100%), wear (97%/100%), postoperative sensitivity (100%/100%) and recurrent caries (100%/100%). Statistical analysis was completed with Fisher's exact or Pearson Chi-square tests at a significance level of 5% (P<0.05). Results showed that almost all restorations were clinically satisfactory with no significant differences between materials groups. Five restorations of Excite/Tetric Ceram failed. No restorations of Vitremer have yet failed.  相似文献
