IntroductionMultiparametric magnetic resonance imaging (mpMRI) has resulted in accurate prostate cancer localization and image-guided targeted sampling for biopsy. Despite its more recent uptake, knowledge gaps in interpretation and reporting exist. Our objective was to determine the need for an educational intervention among urology residents working with mpMRIs.MethodsWe administered an anonymous, cross-sectional, self-report questionnaire to a convenience sample of urology residents in U.S. and Canadian training programs. The survey included both open- and closed-ended questions employing a five-point Likert scale. It was designed to assess familiarity, exposure, experience, and comfort with interpretation of mpMRI.ResultsFifty-three surveys were completed by residents in postgraduate years (PGY) 1–5 and of these, only 12 (23%) reported any formal training in mpMRI interpretation. Most residents’ responses demonstrated significant experience with prostate biopsies, as well as familiarity with reviewing mpMRI for these patients. However, mean (± standard deviation [SD]) Likert responses suggested a relatively poor understanding of the components of Prostate Imaging-Reporting and Data System (PI-RADS) v2 scoring for T2-weighted films (2.45±1.01), diffusion-weighted imaging (DWI) films (2.26±0.90), and dynamic contrast-enhanced (DCE) films (2.21±0.99). Similar disagreement scores were observed for questions around interpretation of the different functional techniques of MRI images. Residents reported strong interest (4.21±0.91) in learning opportunities to enhance their ability to interpret mpMRI.ConclusionsWhile mpMRI of the prostate is a tool frequently used by care teams in teaching centers to identify suspicious prostate cancer lesions, there remain knowledge gaps in the ability of trainees to interpret images and understand PI-RADS v2 scoring. Online modules were suggested to balance the needs of trainee education with the residency workflow. 相似文献
Coumarin and benzothiazole scaffolds can be found in a number of natural or synthetic antioxidants. In an effort to develop a novel radical scavenger and potential antioxidant, a series of coumarin derivatives containing 2‐methylbenzothiazoline motif and related compounds was synthesized and evaluated for their DPPH (1,1‐diphenyl‐2‐picrylhydrazyl) and ABTS?+ (2,2′‐azinobis(3‐ethylbenzothiazoline‐6‐sulfonic acid) radicals scavenging activities. Among them, 7‐hydroxy‐3‐(2‐methyl‐2,3‐dihydrobenzo[d]thiazol‐2‐yl)‐2H‐chromen‐2‐one ( 3e ) has shown a significant free radical scavenging activity. From the structure–activity point of view, it was found that phenolic coumarin ring and benzothiazoline moiety in target compounds may contribute to the scavenging activity against free radicals. 相似文献

Although this disease is an extremely rare malignant cancer, its pleural subtype is even less common. BACKGROUND: Carcinosarcomas are rare neoplasms of the female genital tract. They tend to be highly aggressive and are generally associated with a poor prognosis. Carcinosarcomas of the uterine cervix are extremely rare, with only approximately 35 cases previously reported in English. CASE: A 68-year-old woman presented with cervical carcinosarcoma. She remained without evidence of recurrent disease for 18 months after surgical resection and pelvic radiation treatment. CONCLUSION: In a review of all cases reported in the literature, it appears that cervical carcinosarcomas tend to present at an earlier stage than carcinosarcomas of the uterine corpus, therefore allowing early diagnosis and treatment. They may therefore be associated with a better overall prognosis than their counterparts in the corpus. Some studies have shown improved survival of patients of carcinosarcoma of the uterine corpus whose treatment included postoperative radiation and chemotherapy. Due to the better prognosis of cervical carcinosarcomas, we suggest studies to evaluate the role of aggressive, multimodal therapy, with the intent of obtaining a cure of cervical carcinosarcomas. 相似文献
The aim of this experimental study was to investigate hepatotoxicity effects of noise and toluene, and in particular, to study hepatotoxicity effects of simultaneous exposure to noise and toluene by histopathological and biochemical experiments. To experiment hepatotoxicity effects of noise and toluene, 100 dB white noise and 1000 ppm toluene vapors were generated during two consecutive weeks in healthy male New Zealand White rabbits. Non-simultaneous exposure to noise and toluene increased liver enzymes and the serum levels of superoxide dismutase, malondialdehyde, and total antioxidant capacity, and also decreased serum level of glutathione peroxidase. Alanine transaminase, aspartate transaminase, gamma-glutamyl transferase, malondialdehyde, total antioxidant capacity, and superoxide dismutase levels increased by simultaneous exposure to noise and toluene. Furthermore, catalase and alkaline phosphatase level decreased by simultaneous exposure to noise and toluene. The hematoxylin and eosin stain (H&E) experiments indicated significant swelling, lipidosis, eosinophilic cytoplasm, pyknosis, karyorrhexis, and disruption of the cytoplasmic membrane in the liver tissue due to exposure to noise, toluene and simultaneous exposure to them.
A radiative vapor condenser sheds heat in the form of infrared radiation and cools itself to below the ambient air temperature to produce liquid water from vapor. This effect has been known for centuries, and is exploited by some insects to survive in dry deserts. Humans have also been using radiative condensation for dew collection. However, all existing radiative vapor condensers must operate during the nighttime. Here, we develop daytime radiative condensers that continue to operate 24 h a day. These daytime radiative condensers can produce water from vapor under direct sunlight, without active consumption of energy. Combined with traditional passive cooling via convection and conduction, radiative cooling can substantially increase the performance of passive vapor condensation, which can be used for passive water extraction and purification technologies.Energy and clean water are global challenges that are intertwined in an unfavorable way: even in areas where water is abundant, energy may not be available to purify it for human use (1, 2). There has been strong interest in developing passive technologies to purify or harvest water without using fuel or electricity. In this context, passive vapor condensation becomes particularly important because many passive water technologies go through the vapor phase of water in their harvesting or purification processes.Traditional vapor condensation technique is based on convective and conductive heat exchange with ambient environments. This technique is widely used in systems with hot vapors (3–6). However, with ever-increasing emphasis on passive systems, there are many situations in which warm- or even room-temperature vapor needs to be effectively condensed, such as extracting water from atmosphere (7–9) and warm vapor generated from high-efficiency solar evaporation (10). For vapor at such temperatures, most traditional condensers fail. For this reason, there is a clear need for a condensation technique to complement traditional condensers.A different technique is based on radiative vapor condensation. Darkling beetles in the Namib desert (11) use this technique to collect water. Their bodies function as a cooling surface by shedding thermal energy through midinfrared (mid-IR) radiation toward a clear nighttime sky, generating dew from humid air. This mechanism is also used by commercial radiative dew condensers (7–9). However, neither Namib beetle nor existing dew condensers can operate in the daytime (7). Those nighttime radiative condensers are incompatible with many emerging water technologies that require 24 h operation or direct access to sunlight.Recently, Fan et al. showed that passive radiative cooling to subambient temperatures can be realized even during the daytime, by integrating a high-efficiency solar reflector with a high-emissivity thermal emitter in the mid-IR atmospheric transparency window (12). Using this work as a basis, here we demonstrate a daytime radiative condenser. Compared to existing radiative vapor condensers (7–9), our condenser can function even in the presence of sunlight, which is essential for integration into passive water-harvesting systems that mainly operate during daytime. 相似文献
BACKGROUND: Substantial redistribution of lymphocytes occurs upon the initiation of highly active antiretroviral therapy (HAART) and immune-based HIV therapies. OBJECTIVE: To evaluate the relative contribution of apoptosis and proliferation to changes in lymphocyte populations in peripheral blood and lymph node resulting from interleukin-2 (IL-2) therapy in patients receiving stable HAART. METHODS: Lymphocyte apoptosis was analyzed on various subtypes using fluorescence activated cell sorting with an annexin-V antibody in peripheral blood and by the TUNEL (terminal uridine nucleotide end labelling) method in corresponding lymph node sections. Lymphocyte proliferation was evaluated using an antibody against the cell cycle-associated marker Ki-67 (MIB-1) in peripheral blood and lymph nodes. RESULTS: A transient increase in apoptosis was seen in peripheral blood and lymph nodes during a cycle of subcutaneous IL-2. A pronounced proliferative effect of IL-2 (from 6.4% of total lymphocytes in patients only treated with HAART to 23.4% in those treated with HAART + IL-2) was detected in peripheral blood, affecting the CD4, CD8 and CD16/56 subsets to a similar extent. Remarkably, the proliferative effect also occurred in lymphoid tissues. While the lymph node structure gradually disintegrated over 24 months in some individuals, the amount of proliferating lymphocytes, including CD4 cells, B cells and follicular dendritic cells, greatly increased upon IL-2, while HIV RNA load in lymph nodes remained unaffected. CONCLUSION: These results show that IL-2 leads to lymphocyte proliferation in peripheral blood and lymph nodes without an impact on viral load in lymphoid tissue. These results have important implications for attempts to reconstitute the immune system in HIV disease. 相似文献
