FATAL CEREBRAL ANOXIA FOLLOWING THE USE OF LIGNOCAINE SPRAY FOR BRONCHOSCOPY

Fatal cerebral damage of the chemical type is reported in asix-year-old girl following the use of lignocaine for bronchoscopyin dosage currently advised by some authorities. Evidence ispresented for considering that the cerebral damage resultedfrom anoxia aggravated by convulsions and the central actionof the drug. The maximal safe dosage of lignocaine for bronchoscopyis considered to be less than advocated by some authoritiesand should not exceed 80 mg for a fit 70 kg adult, reduced proportionatelyfor children. The extremely rapid absorption of drugs from thebronchial mucous mem brane is emphasized.     

Renal Extracorporeal Shock Wave Lithotripsy Performed in Patient with Implantable Cardioverter Defibrillator

The effect of extracorporeal shock wave lithotripsy on the automatic implantable cardioverter defibrillator is unknown. To evaluate what effect might occur, a non-implanted automatic implantable cardioverter defibrillator was subjected to a full course of extracorporeal shock wave lithotripsy while inactive. Bench testing by the manufacturer after lithotripsy demonstrated normal function of the device. A patient with an automatic implanted cardioverter defibrillator who required contralateral extracorporeal shock wave lithotripsy then underwent this procedure. The right renal calculus was destroyed successfully with no apparent damage to the automatic implantable cardioverter defibrillator. A test of the automatic implantable cardioverter defibrillator after lithotripsy demonstrated normal sensing and conversion of induced ventricular tachycardia.     

Multiple Inflamed Nevi in a Child with Oculocutaneous Albinism: What Lessons can be Learned?

Abstract:   We report a 12-year-old girl with oculocutaneous albinism who developed nonitchy and nontender erythematous spots. A skin biopsy specimen of one of the spots confirmed that the lesion was an inflamed melanocytic nevus. We emphasize the atypical presentation of nevi in individuals with albinism.     

Intra‐Atrial Conduction Block Mimicking Atrioventricular Nodal Block After Multiple Catheter Ablation Procedures for Atrial Tachycardia in a Patient with Cardiomyopathy

Intra‐Atrial Conduction Block . A 42‐year‐old woman with a history of cardiomyopathy and multiple ablation procedures for atrial tachycardia developed intra‐atrial conduction block that mimicked atrioventricular (AV) nodal block during radiofrequency ablation at the cavotricuspid isthmus. She was treated with atrial pacing (from the coronary sinus), which overcame intra‐atrial conduction block and resulted in AV nodal conduction. Mitochondrial Electron Transport Enzymes in Normal and Leukemic Human Leukocytes

Assays of TD transhydrogenase and succinate cytochrome C reductase carried out on the homogenate of normal and leukemic human leukocytes andon the 1.5 M. fraction (fraction 3) of a continuous sucrose density gradientanalysis of a crude mitochondrial fraction, provided strong evidence for themitochondrial location of TD transhydrogenase. A slightly higher activity ofthese enzymes was found in the homogenate of cells from chronic lymphocyticand acute leukemia compared to those of normal polymorphonuclear leukocytes, normal lymphocytes, and cells from chronic myeloid leukemia. Thesedifferences were very much more marked when purified mitochondrial fractions from these cells were examined. The significance of these findings is discussed.
