目的探讨老年腹股沟疝患者行疝修补术后并发症的危险因素,为预防并发症的发生提供依据。
方法选取2017年1至12月安徽医科大学附属六安医院收治的行疝修补术的老年腹股沟疝患者122例作为研究对象,搜集患者的基本临床资料,并随访术后1年的并发症发生情况,采用单因素分析与多因素Logistic回归分析术后并发症发生的危险因素。
结果随访1年,失访患者9例,随访成功的113例患者中有21例存在相关并发症,总发生率为18.58%:其中手术部位感染6例(切口浅表感染4例,皮下脓胀1例,补片感染1例),术后疼痛4例,尿潴留3例,血清肿3例,股静脉血管损伤2例,术后复发2例,膀胱损伤1例。单因素分析结果显示,并发症发生与年龄、粘连程度、内环口大小、术前合并慢性阻塞性肺疾病、麻醉ASA分级与疝内容物是否完全回纳相关(P<0.05);多因素Logistic回归分析结果显示:年龄、粘连程度、内环口大小与疝内容物是否完全回纳是术后并发症发生的独立危险因素(P<0.05)。
结论老年患者腹股沟疝进行手术治疗后一年内的并发症发生率不低,其中高龄、疝环重度粘连、内环口较大与疝内容物无法完全回纳的患者术后并发症的风险更高,需要重点关注这类术后并发症高危患者以采取相应预防措施。 相似文献

目的了解医护双人观察在结肠镜检查退镜过程是否可以提高息肉检出率。方法连续性收集内镜室诊断性结肠镜检查患者,前瞻性地随机分医护双人观察组和医生单人观察组,记录比较两组的息肉检出率。结果在4个月研究期间共完成985例诊断性结肠镜检查,其中271例检出息肉,总704枚。498例医护双人观察组和487例医生单人观察组的息肉检出率分别为30.5%和24.4%,差异有统计学意义(P=0.032)。在医生单人观察组中,护士证实医生在21例患者中共漏检息肉27枚。结论医护双人观察在结肠镜检查退镜过程中协助医生共同观察可以提高息肉检出率。 相似文献

Although the mechanisms of action (MoA) of nonstructural protein 3 inhibitors (NS3i) and NS5B inhibitors (NS5Bi) are well understood, the MoA of cyclophilin inhibitors (CypI) and NS5A inhibitors (NS5Ai) are not fully defined. In this study, we examined whether CypI and NS5Ai interfere with hepatitis C virus (HCV) RNA synthesis of replication complexes (RCs) or with an earlier step of HCV RNA replication, the creation of double-membrane vesicles (DMVs) essential for HCV RNA replication. In contrast to NS5Bi, both CypI and NS5Ai do not block HCV RNA synthesis by way of RCs, suggesting that they exert their antiviral activity prior to the establishment of enzymatically active RCs. We found that viral replication is not a precondition for DMV formation, since the NS3-NS5B polyprotein or NS5A suffices to create DMVs. Importantly, only CypI and NS5Ai, but not NS5Bi, mir-122, or phosphatidylinositol-4 kinase IIIα (PI4KIIIα) inhibitors, prevent NS3-NS5B-mediated DMV formation. NS3-NS5B was unable to create DMVs in cyclophilin A (CypA) knockdown (KD) cells. We also found that the isomerase activity of CypA is absolutely required for DMV formation. This not only suggests that NS5A and CypA act in concert to build membranous viral factories but that CypI and NS5Ai mediate their early anti-HCV effects by preventing the formation of organelles, where HCV replication is normally initiated. This is the first investigation to examine the effect of a large panel of anti-HCV agents on DMV formation, and the results reveal that CypI and NS5Ai act at the same membranous web biogenesis step of HCV RNA replication, thus indicating a new therapeutic target of chronic hepatitis C. 相似文献

Syndecan-1 (SDC1, CD138) is a key cell surface adhesion molecule essential for maintaining cell morphology and interaction with the surrounding microenvironment. Deregulation of SDC1 contributes to cancer progression by promoting cell proliferation, metastasis, invasion and angiogenesis, and is associated with relapse through chemoresistance. SDC1 expression level is also associated with responses to chemotherapy and with prognosis in multiple solid and hematological cancers, including multiple myeloma and Hodgkin lymphoma. At the tissue level, the expression levels of SDC1 and the released extracellular domain of SDC1 correlate with tumor malignancy, phenotype, and metastatic potential for both solid and hematological tumors in a tissue-specific manner. The SDC1 expression profile varies among cancer types, but the differential expression signatures between normal and cancer cells in epithelial and stromal compartments are directly associated with aggressiveness of tumors and patient''s clinical outcome and survival. Therefore, relevant biomarkers of SDC signaling may be useful for selecting patients that would most likely respond to a particular therapy at the time of diagnosis or perhaps for predicting relapse. In addition, the reciprocal expression signature of SDC between tumor epithelial and stromal compartments may have synergistic value for patient selection and the prediction of clinical outcome. 相似文献

BACKGROUND: There is a need for a critically evaluated reference method for thyroxine to provide an accuracy base to which routine methods can be traceable. We describe a candidate reference method involving isotope-dilution coupled with liquid chromatography/mass spectrometry. METHODS: An isotopically labeled internal standard, thyroxine-d(5), was added to serum, followed by equilibration, protein precipitation, and ethyl acetate and solid-phase extractions to prepare samples for liquid chromatography-mass spectrometry electrospray ionization (LC/MS-ESI) analysis. For separation, a Zorbax Eclipse XDB-C(18) column was used with a mobile phase consisting of 1 mL/L acetic acid in acetonitrile-water (32:68 by volume) for positive ions and a Zorbax Extend-C(18) column with a mobile phase consisting of 2 mL/L ammonium hydroxide in methanol-water (32:68 by volume) for negative ions. [M + H](+) ions at m/z 778 and 783 for thyroxine and its labeled internal standard were monitored for positive ions and [M - H](-) ions at m/z 776 and 781 for negative ions. Samples of frozen serum pools were prepared and measured in three separate sets. RESULTS: Within-set CVs were 0.2-1.0%. The correlation coefficients of all linear regression lines (measured intensity ratios vs mass ratios) were 0.999-1.000. Positive- and negative-ion measurements agreed with a mean difference of 0.45% at three concentrations (50, 110, and 168 microg/L). The detection limits (at a signal-to-noise ratio of approximately 3 to 5) were 30 and 20 pg for positive and negative ions, respectively. The results from the LC/MS-ESI method were within 1 SD of the composite means from many routine clinical methods, although it appears that the clinical method means may be biased high by 4-5 microg/L across the concentrations. Some routine clinical methods may be biased by up to 20% at low concentrations. CONCLUSIONS: This well-characterized LC/MS-ESI method for total serum thyroxine with a theoretically sound approach, demonstrated good accuracy and precision, and low susceptibility to interferences qualifies as a candidate reference method. Use of this reference method as an accuracy base may reduce the apparent biases in routine methods along with the high interlaboratory imprecision. 相似文献
