Experiencing vaginismus–sufferers beliefs about causes and effects

The present study examined the experience of vaginismus in 89 sufferers and ex-sufferers using quantitative and qualitative methodologies. The results provide insights into the effects of vaginismus on the sufferers' self perception and on their relationships with important others. It is suggested that vaginismus should not be regarded as a psychosexual problem but as a psychosocial experience in which defences have been created to protect the self, and that the vaginal spasm is only one of the many effects of this defence.     

Distal clavicular physeal injury

Fourteen patients with separation of the distal clavicular metaphysis from the epiphysis were reviewed. These injuries, especially in the older child and adolescent, mimic acromioclavicular separation. However, they are really growth-plate injuries in which the epiphysis and physis maintain their normal anatomic relation to the shoulder joint, while the distal metaphysis is displaced superiorly, away from the underlying structures. The periosteal sleeve is generally intact inferiorly, and the ligamentous structures connecting the clavicle to the coracoid usually remain attached to the periosteal sleeve. Depending on the degree of displacement, these patients may be treated by either closed methods or pin fixation. The periosteal sleeve is usually extremely osteogenic and will readily fill in any gap between the periosteum and metaphysis. In fact, in one patient this led to clavicular "duplication" and necessitated resection of the original distal clavicle. In the skeletally immature patient it is extremely important to recognize that these physeal injuries are discrete from acromioclavicular separation.     

Existence of a pool of T-lymphocyte colony-forming cells (T-CFC) in human bone marrow and their place in the differentiation of the T- lymphocyte lineage

The existence and characteristics of bone marrow T-cell progenitors have not yet been established in man. Several pieces of evidence such as the reconstitution of certain immunodeficiencies by bone marrow graft suggest that T-cell precursors are present in the bone marrow. We report the growth of T-cell colonies from bone marrow populations using PHA-stimulated lymphocyte-conditioned medium containing T-cell growth factor (TCGF). Rosetting experiments and complement-dependent cytotoxicity assays with monoclonal antibodies indicate that the bone marrow T colony-forming cells (T-CFC) are E- OKT 3- and la+, i.e., immature progenitors. The colonies derived from these cells have the phenotype of mature T cells: E + OKT 3 + la- with either helper (OKT 4+) and suppressor (OKT 8 +) antigens. These results suggest that a thymic microenvironment may not be necessary for the in vitro proliferation and differentiation of the T-cell lineage in adult humans. These methodologies may permit direct investigation of early phenomena concerning the T-cell lineage, such as the acquisition of self-tolerance, the formation of a repertoire of specificities, and the HLA restriction phenomena that we believe takes place before the thymic maturation.

Ethyl (3S,4aR,6S,8aR)-6-(4-ethoxycar- bonylimidazol-1-ylmethyl)decahydroiso-quinoline-3-carboxylic ester: a prodrug of a GluR5 kainate receptor antagonist active in two animal models of acute migraine

Amino diacid 3, a highly selective competitive GluR5 kainate receptor antagonist, exhibited high GluR5 receptor affinity and selectivity over other glutamate receptors. Its diethyl ester prodrug 4 was orally active in two models of migraine: the neurogenic dural plasma protein extravasation model and the nucleus caudalis c-fos expression model. These data suggest that a GluR5 kainate receptor antagonist might be an efficacious antimigraine therapy with a novel mechanism of action.
