Transesophageal echocardiographic identification of thrombus producing obstruction of left pulmonary artery descending lobar branches and bronchial artery dilatation

We report an elderly patient in whom a thrombus in the distal left pulmonary artery was shown by transesophageal echocardiography to extend and produce obstruction of the descending lobar branches as well as dilatation of the left bronchial artery.     

Two-dimensional echocardiographic identification of pulmonary artery branch stenosis

Pulmonary artery branch stenosis is a not uncommon congenital lesion, noted in approximately 4% of children at the time of cardiac catheterization. Real-time 2-dimensional echocardiography was used to examine 10 patients with angiographically documented pulmonary branch stenosis. Five cases of pulmonary artery branch narrowing were identified by echocardiography without prior knowledge of angiographic findings. In 5 other patients whose angiographic results were known, the narrowings were identified in 4 of 5 cases. These results demonstrate the feasibility of evaluating pulmonary branch stenosis with the nonionizing, noninvasive method of 2-dimensional echocardiography.

Lagenaria siceraria fruit extract ameliorate fat amassment and serum TNF-α in high-fat diet-induced obese rats

ObjectiveTo investigate the effects of ethanolic extract of Lagenaria siceraria fruit (ELSF) on fat amassment and serum TNF– α in high-fat diet-induced obese rats.MethodsThe high fat diet induced obese rats were orally treated with orlistat (50 mg/kg) and ELSF (100, 200, 300 mg/kg/day) to the respective treatment groups. The body weight, fasting blood glucose level, lipid profile, serum levels of tumor necrosis factor– α (TNF–α) in rats were measured after 30 days of treatment and compared to the obese control animals.ResultsELSF significantly (P < 0.001) reduced the body weight gain, fasting blood glucose, total cholesterol, triglyceride, total protein and TNF–α.ConclusionsThese encouraging findings suggest that Lagenaria siceraria has excellent pharmacological potential to prevent fat amassment.     

Transesophageal Echocardiographic Findings in Sclerosing Mediastinitis

We report the usefulness of transesophageal echocardiography in evaluating a patient with sclerosing mediastinitis. The technique enabled us to identify a mass obstructing the superior vena cava and right upper and lower pulmonary veins, and infiltrating and invaginating into the left atrial cavity. Transesophageal echocardiography was superior to computed tomography and transthoracic echocardiography in delineating these findings.     

Relation between coronary artery calcium and incident chest pain in a community-dwelling screening population

Incident chest pain occurred in 30.3% of 1,743 asymptomatic healthy men and women who were followed for up to 4 years. Proportions of patients who had coronary artery calcium were similar among those who had no chest pain, noncardiac pain, atypical pain, or cardiac chest pain. Incident chest pain is common and should be examined according to a patient's pretest probability of developing coronary artery disease, without excessive influence of the presence of coronary artery calcium.     

Custom-made endoprosthetic total humerus reconstruction for musculoskeletal tumours

We analysed 11 patients with malignant musculoskeletal tumours of the humerus who underwent limb salvage surgery with total humeral custom endoprosthesis from 1990 to 2009.
