The objective of this study was to determine the expression and transferability of tetracycline and erythromycin resistance among 188 MRSA strains from a Malaysian tertiary hospital. The minimum inhibitory concentrations (MICs) for oxacillin, erythromycin, tetracycline and ciprofloxacin ranged from 4 to 512 μg/ml, 0.25 to 256 μg/ml, 0.5 to 256 μg/ml and 0.5 to 512 μg/ml, respectively. Tetracycline-resistant strains showed co-resistance towards ciprofloxacin and erythromycin. There was a significant increase (P<0.05) of high-level tetracycline (≥MIC 256 μg/ml) and erythromycin (≥MIC 128 μg/ml) resistant strains in between the years 2003 and 2008. All erythromycin-resistant strains harboured ermA or ermC gene and all tetracycline-resistant strains harboured tetM or tetK gene. The blaZ was detected in all MRSA strains, whereas ermA, tetM, ermC, tetK and msrA genes were detected in 157 (84%), 92 (49%), 40 (21%), 39 (21%) and 4 (2%) MRSA strains, respectively. The blaZ, tetM, ermC and tetK genes were plasmid-encoded, with ermC gene being easily transmissible. Tn5801-like transposon was present in 78 tetM-positive strains. ermA and tetM genes were the most prevalent erythromycin and tetracycline resistance determinants, respectively, in MRSA strains. The association of resistance genes with mobile genetic elements possibly enhances the spread of resistant traits in MRSA.  相似文献

Introduction

This study examines the effects of palm vitamin E (PVE) or α-tocopherol (α-TF) supplementation on adrenocorticotropin hormone (ACTH), corticosterone and gastric lesions in rats exposed to water-immersion restraint stress (WIRS).

Material and methods

Sixty male Sprague-Dawley rats (200-250 g) were divided into three groups. Group I: 20 rats as a control group were given a normal diet. Group II: 20 rats received oral supplementation of PVE at 60 mg/kg body weight. Group III: 20 rats received oral supplementation of α-TF at 60 mg/kg body weight. After the treatment period of 28 days, each group was further subdivided into two groups: 10 rats not exposed to stress, and the other 10 rats subjected to WIRS for 3.5 h. Blood samples were taken to measure the ACTH and corticosterone levels. The rats were then sacrificed and the stomach excised and opened along the greater curvature and examined for lesions.

Results

Rats exposed to WIRS had lesions in their stomach mucosa. Our findings showed that dietary supplementation of PVE or α-TF was able to reduce gastric lesions significantly in comparison to the stressed controls. The WIRS increased plasma ACTH and corticosterone significantly. Palm vitamin E and α-TF treatments reduced these parameters significantly compared to the stressed controls.

Conclusions

Supplementation with either PVE or α-TF reduces the formation of gastric lesions, probably by inhibiting the elevation of ACTH and corticosterone levels induced by stress.  相似文献

Relaxin is a hormone structurally related to insulin and insulin‐like growth factor, which exerts its regulatory effect on the musculoskeletal and other systems through binding to its receptor in various tissues, mediated by different signaling pathways. Relaxin alters the properties of cartilage and tendon by activating collagenase. This hormone is also involved in bone remodeling and healing of injured ligaments and skeletal muscle. In this review, we have summarized the literature on the effect of relaxin in musculoskeletal system to provide a broad perspective for future studies in this field.  相似文献

Patients with issues or health problems usually plan to discuss their concerns with their health care providers. If these concerns were not presented or voiced during the health care provider-patient encounter, the patients are considered to have unvoiced needs. This article examines the extent and possible determinants of patients' unvoiced needs in an outpatient setting. A cross-sectional study was conducted in 5 Ministry of Health Malaysia primary health facilities throughout the country. Of 1829 who participated, 5 did not respond to the question on planned issues. Of the 1824 respondents, 57.9% (95% confidence interval = 47.1-68.7) claimed to have issues/problems they planned to share, of whom 15.1% to 26.7% had unvoiced needs. Extent of unvoiced needs differed by employment status, perceived category of health care provider, and study center. Perceived category of health care provider, method of questionnaire administration, and study center were the only significant determinants of unvoiced needs. Unvoiced needs do exist in Malaysia and there is a need for health care providers to be aware and take steps to counter this.  相似文献

Introduction: The importance of serum lipids as cardiovascular risk factors is well recognized. However, most published studies have focused on western countries. The present study aimed to describe and analyze the lipid profile parameters in Malaysian dyslipidemic patients, and to identify concomitant clinical problems and risk factors associated with cardiovascular disease (CVD) among such patients. Methods: A retrospective record review was carried out at Hospital Universiti Sains Malaysia. The records were reviewed for 890 dyslipidemic patients who attended the hospital in 2007. Data were collected for age at time of presentation, sex, ethnicity, smoking status, pre-treatment lipid levels, and presence of associated illnesses. The study sample was classified according to the National Cholesterol Education Program Adult Treatment Panel III risk groups. Results: The mean (SD) values for total cholesterol, low-density lipoprotein cholesterol, high density lipoprotein cholesterol, and triglycerides were 6.4 (1.3), 4.1 (1.3), 1.4 (0.5) and 1.9 (1.2) mmol/l, respectively. Less than half of study sample (43.1%) had coronary heart disease and coronary heart diseases equivalents, 24.3% were at moderate risk, and 32.6% were at low risk. Hypertension was present in 79.9% of the study sample, while 27.5% were diabetics. Cardiovascular disease was reported among 17.9%. Logistic regression revealed that family history of premature cardiovascular disease, higher age risk group; ethnicity and total cholesterol were predictors for the development of cardiovascular disease. Conclusion: The present review showed that dyslipidemic patients had high total cholesterol levels, according to National Cholesterol Education Program Adult Treatment Panel III guidelines. They were clinically diagnosed at middle age. Hypertension and diabetes were the commonest associated clinical problems. A large proportion of the patients were within the coronary heart disease or coronary heart disease risk equivalent group. Family history of premature cardiovascular disease, age, ethnicity, and total cholesterol are important risk factors for the development of cardiovascular disease in Malaysian dyslipidemic patients.  相似文献

This study reports the in vitro inhibitory potential of crude extract of Quercus lusitanica (Q. lusitanica) seeds on the replication of dengue virus type 2 (DEN-2). In vitro antiviral activity of Q. lusitanica extract, assessed in C6/36 cells (cloned cells of Aedes albopictus larvae) employing a virus inhibition assay, showed dose-dependent inhibition. The Q. lusitanica extract at its maximum non-toxic concentration of 0.25 mg/ml completely inhibited 10-1,000 TCID50 of virus, as indicated by the absence of cytopathic effect (CPE). The low dose of Q. lusitanica (0.032 mg/ml) showed 100% inhibition with 10 TCID50 of virus, but only 50% and 25% inhibition with 100 and 1,000 TCID50, respectively. The NS1 is a glycoprotein present in all flaviviruses and appears essential for virus viability. To further evaluate Q. lusitanica extract as an antiviral compound, we investigated the effect of Q. lusitanica extract on the NS1 protein expression of infected C6/36 cells through proteomics technique. The result showed downregulation of NS1 protein expression of infected C6/36 cells after treatment with this extract. In conclusion, we found that Q. lusitanica extract has a good inhibitory effect on the replication of dengue virus type 2, both in conventional cell culture and proteomics technique.  相似文献

目的:评估梅毒性葡萄膜炎患者的临床表现和视力. 方法:梅毒性葡萄膜炎患者3例在USM医院接受治疗. 结果:患者3例被诊断为继发性梅毒性葡萄膜炎,这3例患者患病之前均不知道患有梅毒,但他们有明确的乱交史.并且每个月都伴有逐步的视力下降.其中两人伴有发热、跟痛、眼前悬浮物.视力从6/12到手动.所有患者均出现前葡萄膜炎,玻璃体炎和视神经炎.第一例患者出现了多灶性脉络膜视网膜炎,并伴有渗出性视网膜脱离.第二例患者出现渗出性视网膜脱离,而第三例患者仅出现了脉络膜视网膜炎.所有患者每周注射苄青霉素2.4MU,共4wk,其中2例患者口服多西霉素200mg 2次/d,共3mo.治疗效果良好,其中2例患者有显著的视力上升,分别从6/120到6/21和6/12到6/6.其中较严重的1例患者出现逆转录酶为阳性. 结论:眼梅毒作为非肉芽肿性的炎症与渗出性视网膜脱离有关.治疗后虽然视力恢复比较缓慢,但普遍有良好的效果.  相似文献

The aim of this study was to determine the suitability of tannin-added Rhizophora spp. particleboards as phantom materials in the application of low- and high-energy photons. The tannin-added Rhizophora spp. particleboards and density plug phantoms were created with a target density of 1.0 g/cm3. The elemental composition and effective atomic number of the particleboards were measured using energy dispersive X-ray analysis. The mass attenuation coefficient of the particleboards for low-energy photons were measured using the attenuation of X-ray fluorescence. The mass attenuation coefficients of high-energy photons were measured using the attenuation of 137Cs and 60Co gamma energies. The results were compared to the calculated value of water using XCOM calculations. The results showed that the effective atomic number and mass attenuation coefficients of tannin-added Rhizophora spp. particleboards were similar to those of water, indicating the suitability of tannin-added Rhizophora spp. particleboards as phantom materials for low- and high-energy photons.  相似文献
