穿琥宁粉针雾化吸入治疗小儿呼吸道病毒感染疗效观察

小儿呼吸道感染是小儿科最常见的疾病，以咽炎、扁桃体炎、气管炎和肺炎多见。病原体以病毒感染为主。未经治疗有诱发心肌炎、脑炎的可能，应积极进行有效治疗。我们自2004年6月至12月应用注射用穿琥宁冻干粉剂雾化吸入治疗小儿呼吸道病毒感染58例，取得满意效果，现总结报告如下：     

持续弹性外牵引矫正乳头内陷

目的 探索一种行之有效的、微创的乳头内陷矫正方法。方法 用钢丝、弹簧和塑料注射器制成弹性可调式的乳头内陷矫治器，利用弹性牵引作用，持续牵拉内陷乳头3～6个月。结果 14例患者均得以矫正，并获得良好的近期效果。1例患者因左侧乳头内陷严重，治疗2周时钢丝接近脱出乳头而更换矫治器并延长佩戴时间。随访患者6～12个月。效果稳定。结论 弹性可调式乳头内陷矫治器用于先天性乳头内陷的治疗，近期临床效果较好，为目前较佳的非手术治疗乳头内陷的方法，但其机理和远期效果还需进一步研究和观察。     

Lymphoid Neogenesis in Murine Cardiac Allografts Undergoing Chronic Rejection

Lymphoid neogenesis is the process by which ectopic lymphoid accumulations that resemble lymph nodes arise in nonlymphoid tissues. Such lymphoid accumulations, known as tertiary lymphoid organs (TLO), are observed in chronic autoimmunity and they propagate immune pathology by setting up local antigen presenting sites. Whether lymphoid neogenesis occurs in transplanted organs and contributes to rejection is not well understood. To begin to address this question, we retrospectively analyzed 319 murine cardiac allografts for microscopic evidence of lymph-node-like structures. We found 78 allografts that had either classical TLO, characterized by discrete T- and B-cell zones and high endothelial venules (HEV) expressing peripheral node addressin (PNAd) (n = 34), or PNAd(+) HEV without organized lymphoid accumulations (n = 44). These changes were present in both short- and long-lived allografts and were invariably associated with rejection. Importantly, they occurred in 78% of allografts undergoing chronic rejection (n = 85) but in only 7% of allografts undergoing primarily acute rejection (n = 184). These findings indicate that, like autoimmunity, alloimmunity is associated with lymphoid neogenesis in the target organ and suggest a role for local T-cell activation in chronic allograft rejection.     

A simple and inexpensive particle agglutination test to distinguish recent from established HIV-1 infection.

OBJECTIVES: We sought to modify the Serodia HIV-1/HIV-2 particle agglutination assay (PA), a simple and cost-effective HIV assay that is used globally for the detection of HIV antibodies, as a sensitive/less sensitive test (S/LS) to identify recently infected individuals and to estimate HIV incidence. METHODS: The Serodia PA test was modified as an S/LS test (PA-LS) by using HIV antigen-coated gelatin particles at a dilution of 1:68 and a specific diluent, and calibrated using 37 HIV clade B seroconversion panels (309 samples) from Trinidad and from a commercial source that were tested at dilution intervals from 1:10 to 1:80,000. The greatest sensitivity for correctly classifying samples from recent and established infections was determined by receiver operator curve (ROC) analysis. RESULTS: At a 1:40,000 sample dilution and a days post-seroconversion cutoff of 190 days, the PA-LS test yielded a 97% sensitivity for classifying recent and established infection samples. Furthermore, at a 1:20,000 dilution, the positive predictive value for correctly identifying recently infected individuals was 99%. The PA-LS test offers a 30-44-fold cost saving over currently available S/LS tests. CONCLUSION: A modified, low cost and simple-to-perform PA test is appropriate for use in resource-limited countries, and has exhibited excellence in distinguishing recent from established HIV infection.     

Discussing options between patients and health care professionals in genetic diagnosis: ethical and legal criteria

The specific characteristics of genetic data lead to ethical-legal conflicts in the framework of genetic diagnosis. Several international organisations, including UNESCO and the Council of Europe, have enacted rules referring to the use of genetic information. This paper discusses possible legal and ethical criteria that could be used in genetic testing.

Sunnybrook（多伦多）面神经评定系统是近年来发展起来的一种新的面神经功能主观评定系统[1,2],得到了很多学的认可,在2005年9月在荷兰召开的国际面神经大会上,有近1/4的论用到了这一评分标准;而国内学们很少使用此标准,所以有必要向国内的同道作一介绍.

笔者毕业后十余年中,临床上遇到心脑血管病人大部分正在应用西药治疗,中医讲究辨证论治,西药是否影响中医的辨证,临床上脉象是否随着西药运用而改变呢?中医脉象学是中医诊病、治病重要手段,望闻问切中“切诊”是主要组成部分。脉象变化是辨证论治重要依据,求证离不开脉象,组成脉象的主要因素及原因有:①脉位:脉动显现部位的浅深,脉位表浅为浮脉,深沉为沉弦;②至数:脉搏的频率,以一个呼吸周期为单位,一息脉来四~五至为平脉,一息六至为数脉,一息三至为迟脉;③脉长:脉动应指的轴向范围长短,越超三部脉象称为长脉,应指不及三部,但见关部或寸部者…
