Polypeptide growth factors are a class of potent natural biologic mediators which regulate many of the activities of wound healing including cell proliferation, migration, and metabolism. Platelet-derived growth factor (PDGF) and insulin-like growth factor-I (IGF-I) have been shown to regulate DNA and protein synthesis in bone cells in vitro and to interact synergistically to enhance soft tissue wound healing in vivo. We have hypothesized that the combination of PDGF and IGF-I may, therefore, enhance regeneration of both the soft and hard tissue components of the periodontium. To test this hypothesis we performed conventional periodontal surgery on all 4 quadrants of the mouth of 13 beagle dogs with naturally occurring periodontal disease. Following flap reflection, degranulation, and root planing, all premolar teeth in 2 quadrants of each dog received a combination of 3 micrograms of recombinant PDGF-B and IGF-I in a methylcellulose gel, while the premolar teeth in the contralateral quadrants received the gel alone. Teeth in 4 additional animals also received 125I-PDGF or 125I-IGF-I in the treated sites. The clearance rate of the 125I-labeled protein, changes in local bone metabolism, and amount of new bone and cementum with inserting collagen fibers were measured. The clearance studies revealed that the half-life of the factors at the site of application was 3.0 hours for IGF-I and to 4.2 hours for PDGF-B. Greater than 96% of the radio-labeled proteins was cleared by 96 hours and no radioactivity was detected 2 weeks after application. There was a significant (P less than 0.01) 2-fold increase in uptake of the bone-seeking radiopharmaceutical Technetium 99-MDP at 2 and 4 weeks in growth factor treated sites compared to controls, indicating that there was increased metabolic activity within the bone at these sites. Computer-aided histologic analyses of biopsies obtained at 2 and 5 weeks post-operatively revealed a significant (P less than 0.01), 5 to 10 fold increase in new bone and cementum in PDGF-B/IGF-I treated sites at both time points compared to controls receiving the placebo gel. The height and total area of new bone continued to increase from 2 to 5 weeks. The new bone underwent a normal maturation process as judged by histologic appearance. As an alternative to a conventional fixed partial denture, a modification of the hybrid resin-bonded bridge was used to compensate for a spatial discrepancy accentuated by a missing tooth. The proximal margins of the restoration were extended onto the facial surfaces of the abutment teeth to create a partially veneered surface. The abutment teeth were widened by shifting the embrasure into the edentulous space, thus creating more esthetic proportions.  相似文献

PURPOSE: The objectives of this investigation were to evaluate the effect of disinfection on surface quality and dimensional stability of more recent, reformulated vinylpolysiloxane (VPS) and polyether (PE) materials. METHODS: Using ANSI/American Dental Association (ADA) specification 19 protocols, 50 impressions of stainless steel dies were made with each material. Ten impressions of each material were randomly assigned to a treatment group: (1) no disinfectant; (2) 10-minute dual phenol immersion; (3) 1-hour dual phenol; (4) 10-minute sodium hypochlorite (NaOCl); and (5) 1-hour NaOCl. Impression surface quality immediately after disinfection was categorized as smooth/shiny, matte, or wrinkled/sticky. Dimensional stability was evaluated by measuring dimensional accuracy according to specification 19 after 24-hour, 1-week, and 2-week storage at ambient laboratory conditions. OBJECTIVES: The purpose of this study was to examine the influence of variability in bone properties and loading on peri-implant crestal and cancellous bone strains using a probabilistic approach, in combination with finite element (FE) analysis. MATERIAL AND METHODS: Oblique occlusal loading was applied to a single endosseous implant embedded in a two-dimensional (2-D) FE model of a premolar section of a mandible. Perfect bonding was assumed at all interfaces. Five independent parameters (cortical bone thickness (T), cortical (ECORT) and cancellous (ECANC) bone Young's moduli, and vertical (FVERT) and horizontal (FHOR) occlusal forces) were assigned statistical distributions based on data in the literature. Two cancellous bone distribution models were examined, one with a lower mean and range (LM) and the second with a higher mean and range (HM) of cancellous bone Young's modulus values. Sets of randomly chosen values for the five parameters were selected from the distributions and FE analyses were performed for all randomly selected sets. RESULTS: In the LM model, 50% of the cases experienced hyper-physiologic peri-implant crestal strains in the region of commonly reported saucerization, compared with about 25% of the cases in the HM model. Relative probabilistic sensitivities (%) of bone strains to the independent input parameters (T, ECORT, ECANC, FVERT and FHOR) were as follows: 29, 11, 30, 13, and 17 for the LM model, and 17, 11, 35, 21, and 15 for the HM model, respectively. CONCLUSION: Probabilistic analyses of FE models suggest that up to twice as many cases in the LM distribution may be at risk of saucerization as compared with the HM distribution model. Although based on hypothetical distribution values and the limitations inherent to a 2-D analysis, this probabilistic study demonstrated that FE models are very sensitive to the often arbitrarily assigned values for cancellous bone Young's modulus, and also to values used for cortical bone thickness, when the cancellous bone modulus is low.  相似文献

Bristol's much-publicised cardiac surgery problems and subsequent enquiry(1) have drawn attention to the need for audit of treatment outcomes throughout all hospital specialties. Patient anxiety, government policy and the desire of the professions to re-establish public confidence, have further encouraged changes to the system. For medical and dental specialties, such challenges have already been taken up by the Royal Colleges with the establishment of clinical effectiveness committees. Hospitals have modified their procedures and, for consultants, yearly appraisal is already a reality. The Orthodontic Clinical Effectiveness Working Party of the Royal College of Surgeons of England (now the Clinical Effectiveness Committee of the British Orthodontic Society) set up this audit to measure the outcome of fixed appliance treatment and to establish a benchmark for the standard of treatment to be expected from a consultant orthodontist. This paper describes how the audit was carried out, presents the findings and goes on to discuss some of the wider issues involved in audit, clinical governance and appraisal. The Consultant Orthodontists Group of the British Orthodontic Society funded this audit and the results and data set of dental casts remain their property.  相似文献

Polycaprolactone, a thermoplastic aliphatic polyester, is reportedly susceptible to both alkaline and enzymatic hydrolyzes. This screening study examined the susceptibility of Resilon, a polycaprolactone-based root filling composite, to alkaline hydrolysis. There were 15-mm diameter disks of Resilon and Obtura gutta-percha prepared by compressive molding and immersed in 20% sodium ethoxide for 20 or 60 min. Control disks were immersed in ethanol for 60 min. These disks were examined using field-emission scanning electron microscopy and energy dispersive X-ray analysis. For Resilon, the surface resinous component was hydrolyzed after 20 min of sodium ethoxide immersion, exposing the spherulitic polymer structure and subsurface glass and bismuth oxychloride fillers. More severe erosion occurred after 60 min of sodium ethoxide treatment. Gutta-percha was unaffected after immersion in sodium ethoxide. As Resilon is susceptible to alkaline hydrolysis, it is possible that enzymatic hydrolysis may occur. Biodegradation of Resilon by bacterial/salivary enzymes and endodontically relevant bacteria warrants further investigation.  相似文献
