BACKGROUND: In the pathogenesis of urethral stricture, fibrosis is associated with an excessive collagen increase. After the recognition that topical application of Mitomycin-C (MMC) inhibits fibroblast proliferation and is effective in preventing scar formation, many studies have been carried out on this subject outside the scope of urology. The aim of the present study is to observe the intraurethral impact of the employment of low doses of MMC on scar formation and fibrosis in experimental rat model. METHODS: Urethral injuries were made by internal urethrotomy knife. The study was carried out with 35 adult male Wistar albino rats. Five rats were allocated to the control group (group 1), 10 to a group that was administered 2 mg/L MMC (group 2) and 20 to a group that was administered 10 mg/L MMC (group 3). Mitomycin-C was administered to the injured urethra in the form of irrigation for 5 min. The rats were sacrificed 14 days later in order to evaluate chronic inflammation and fibrosis and their penises were histopathologically examined under light microscopy with hematoxilen eosin and trichrom stains. RESULTS: When group 2 was compared with control group, the differences in hemosiderin-laden macrophages (HLM), mononuclear cell infiltration (MCI) and fibrosis were found to be statistically significant (P < 0.01, P < 0.05, P < 0.005, respectively). When group 3 was compared with control group, the differences in HLM, MCI and fibrosis were also found to be statistically significant (P < 0.05, P < 0.05, P < 0.005, respectively). In the comparison of group 2 with group 3, no statistically significant differences were found in terms of the these parameters. CONCLUSIONS: Although MMC is toxic at high doses, the antifibrotic effect of the intraurethral low dose MMC may be useful in combination therapy for internal urethrotomy.  相似文献

ObjectivesTo evaluate the effectiveness, quality and reliability of YouTube videos that address retinal detachment surgery as a source of information and education for patients.MethodsPerforming a YouTube search using the keyword ‘retinal detachment surgery for patients’, the first 100 videos in the search results were included in the study. The number of views, age, number of likes, number of dislikes, number of comments, length, upload source, country of origin, type (patient information and education, patient experience), and surgical technique described (pars plana vitrectomy, scleral buckling, pneumatic retinopexy) were recorded for all videos. The quality and accuracy of the information were evaluated by two independent ophthalmologists using the DISCERN, global quality and usefulness scoring systems.ResultsOf the top 100 videos, 77 met the inclusion criteria. When the videos were evaluated according to the source, 21 (27.3%) had been uploaded by physicians, 30 (39%) by private health institutions, 10 (13%) by health channels, eight (10.4%) by patients, seven (9.1%) by academia (university hospitals), and one (1.3%) by a pharmaceutical company. The mean DISCERN score was 53.36, the mean Global Quality score was 3.49, and the mean usefulness score was 3.86. There was a significant and strong correlation between the usefulness score and the global Quality and DISCERN scores. A significant and strong correlation was also found between the global quality score and the DISCERN score.ConclusionYouTube appears to be a platform with sufficiently informative videos in the area of retinal detachment surgery for patients. Healthcare professionals and professional and academic institutions should use this platform effectively to spread high quality information to patients and their families.  相似文献

In this study, Artemia salina (crustacean filter feeders) larvae were used as a test model to investigate the toxicity of aluminum oxide nanoparticles (Al2O3 NPs) on marine microorganisms. The uptake, toxicity, and elimination of α‐Al2O3 (50 nm and 3.5 μm) and γ‐Al2O3 (5 nm and 0.4 μm) NPs were studied. Twenty‐four and ninety‐six hour exposures of different concentrations of Al2O3 NPs to Artemia larvae were conducted in a seawater medium. When suspended in water, Al2O3 NPs aggregated substantially with the sizes ranging from 6.3 nm to >0.3 µm for spherical NPs and from 250 to 756 nm for rod‐shaped NPs. The phase contrast microscope images showed that NPs deposited inside the guts as aggregates. Inductively coupled plasma mass spectrometry analysis showed that large particles (3.5 μm α‐Al2O3) were not taken up by Artemia, whereas fine NPs (0.4 μm γ‐Al2O3) and ultra‐fine NPs (5 nm γ‐Al2O3 and 50 nm α‐Al2O3) accumulated substantially. Differences in toxicity were detected as changing with NP size and morphology. The malondialdehyde levels indicated that smaller γ‐Al2O3 (5 nm) NPs were more toxic than larger γ‐Al2O3 (0.4 µm) particulates in 96 h. The highest mortality was measured as 34% in 96 h for γ‐Al2O3 NPs (5 nm) at 100 mg/L (LC50 > 100 mg/L). γ‐Al2O3 NPs were more toxic than α‐Al2O3 NPs at all conditions. © 2013 Wiley Periodicals, Inc. The clinical application of detecting COVID-19 factors is a challenging task. The existing named entity recognition models are usually trained on a limited set of named entities. Besides clinical, the non-clinical factors, such as social determinant of health (SDoH), are also important to study the infectious disease. In this paper, we propose a generalizable machine learning approach that improves on previous efforts by recognizing a large number of clinical risk factors and SDoH. The novelty of the proposed method lies in the subtle combination of a number of deep neural networks, including the BiLSTM-CNN-CRF method and a transformer-based embedding layer. Experimental results on a cohort of COVID-19 data prepared from PubMed articles show the superiority of the proposed approach. When compared to other methods, the proposed approach achieves a performance gain of about 1–5% in terms of macro- and micro-average F1 scores. Clinical practitioners and researchers can use this approach to obtain accurate information regarding clinical risks and SDoH factors, and use this pipeline as a tool to end the pandemic or to prepare for future pandemics.  相似文献

BACKGROUND Reconstruction of facial skin defects requires good-quality skin cover to satisfy aesthetic expectations of patient, especially when the skin defect is on the uncovered area of the face. Limitations in the available local tissue and donor-site morbidity restrict the options.
OBJECTIVE In an effort to solve these problems, we have begun to use a subcutaneous pedicled retroauricular reverse-flow flap.
METHODS Between January 1997 and December 2005, reverse-flow subcutaneous pedicled retroauricular island flap was used to cover facial defects in 12 patients who underwent surgical excision of skin tumor. The patients ranged in age from 44 to 81 years with a mean age of 58 years.
RESULTS Only one case experienced a superficial necrosis in the distal one-quarter part of the flap. The functional and aesthetic results were satisfactory for both patients and surgeons, and no tumor recurrence was observed during the 12 to 28 months (mean, 18.8 months) follow-up period.
CONCLUSIONS This flap can be used reliably for the reconstruction of facial skin defects of small and medium size. The preference of frontal branch pedicled flap enables more distal facial area defects to be covered, such as dorsal nasal, nasolabial, and upper lip, than flaps based on parietal branch.  相似文献

Background: Supraventricular tachyarrhythmia is a common problem in chronic obstructive pulmonary disease (COPD) patients. The purpose of this study is to analyze the factors associated with paroxysmal atrial fibrillation (AF) in COPD patients. Methods: Forty COPD patients (38 male, 2 female, mean age 60 ± 9 years) and 33 healthy controls (29 male, 4 female, mean age: 58 ± 10 years) were included in this study. Echocardiography, 24‐hour ambulatory and 12‐lead ECG, pulmonary function tests, arterial blood gases, and serum electrolytes were measured. On ECG, maximum (Pmax) and minimum (Pmin) duration of P wave and its difference, P‐wave dispersion (PWd), were measured. Results: On echocardiography, diastolic dysfunction was found in 14 of the 40 (35%) COPD patients. Heart rate variability analysis revealed that COPD patients had decreased SDANN, SDNN, SDNNIDX in time‐domain, and decreased LF in frequency domain parameters. Fourteen of the 40 COPD patients (35%) had AF. Patients with AF were older (57 ± 10 vs 64 ± 5 years, P = 0.03) and had lower SDANN, SDNN, and LF/HF ratio as compared to patients without AF in univariate analysis. All P‐wave intervals (Pmax Pmin and PWd) were increased in COPD patients compared to controls. P‐wave dispersion was significantly increased in COPD patients with AF, as compared to patients without AF (57 ± 11 vs 44 ± 7 ms, P = 0.001). In logistic regression analysis PWd was found to be the only factor associated with the development of AF (P = 0.04). Conclusions: The presence of AF was significantly related to the prolongation of PWd, but not with pulmonary function, arterial blood gasses, and left and right atrial function. AIM: Our aim was to study the correlation of serum prolidase and insulin like growth factor-1 to liver collagen and assess their utility as markers of fibrosis during four different periods of hepatic injury and fibrosis after bile-duct ligation in rats. METHODS: Forty-eight Wistar albino rats were included in the study and divided into six groups. Seven rats served as the control group (Control), while seven rats had a sham operation (Sham group). Thirty-four rats underwent bile-duct ligation. Bile-duct ligated (BDL) animals were sacrificed at the end of the first week (Group 1; n = 8), second week (Group 2; n = 8), third week (Group 3; n = 9), or fourth week (Group 4; n = 9) after BDL. Liver collagen, liver prolidase, and serum prolidase and IGF-I, were determined. RESULTS: There was a positive correlation between liver collagen and serum prolidase (r(s): 0.843, P < 0.001) levels and a negative correlation among liver collagen and serum IGF-1 levels (r(s): -0.667, P < 0.001). The peak levels of liver collagen and serum prolidase were reached in the third week while the lowest levels of IGF-1 were found at the end of the third week. CONCLUSION: Serum prolidase and IGF-1 either independently or in combination correlate with liver collagen content in hepatic fibrosis.  相似文献
