A prognostic model for Hodgkin's disease was worked out using the data on disease-free survival among patients receiving 4-8 courses of COOP(MOPP)/ABVD plus (sub)total irradiation. Patients with stage I-II Hodgkin's disease (less then 4 lesions) without large involved mediastinal masses, intoxication symptoms and focal splenic involvement were referred to the favorable prognosis group. The poor prognosis group featured stage III(2)-IV tumor as well as large masses of involved mediastinal tissue, focal splenic involvement at any stage plus 7 or more lesions. An assessment of tumor advancement across lesions is more significant for radiotherapy planning rather than that of organ involvement. It is reasonable to distinguish two substages--III(1) and III(2). Our model was compared with GHSG and it was suggested that ways be found to use both of them in prognosing of disease outcome.  相似文献

The number of known mutations in human nuclear genes, underlying or associated with human inherited disease, has now exceeded 100,000 in more than 3700 different genes (Human Gene Mutation Database). However, for a variety of reasons, this figure is likely to represent only a small proportion of the clinically relevant genetic variants that remain to be identified in the human genome (the ‘mutome’). With the advent of next‐generation sequencing, we are currently witnessing a revolution in medical genetics. In particular, whole‐genome sequencing (WGS) has the potential to identify all disease‐causing or disease‐associated DNA variants in a given individual. Here, we use examples of recent advances in our understanding of mutational/pathogenic mechanisms to guide our thinking about possible locations outwith gene‐coding sequences for those disease‐causing or disease‐associated variants that are likely so often to have been overlooked because of the inadequacy of current mutation screening protocols. Such considerations are important not only for improving mutation‐screening strategies but also for enhancing the interpretation of findings derived from genome‐wide association studies, whole‐exome sequencing and WGS. An improved understanding of the human mutome will not only lead to the development of improved diagnostic testing procedures but should also improve our understanding of human genome biology.  相似文献

Published data and our own results relating to exteroceptor and a variety of interceptor neurons in the brain and spinal cord, such as intraspinal Hesse ocelli and light-sensitive epiphyseal and ependymal neurons, are presented. Light-sensitive ganglion neurons in invertebrates are also described, along with intrinsic spinal cord bipolar sensory neurons within the spinal cord, primary chemo-and thermosensitive neurons, and sensory unipolar neurons associated with the three fine “central nerves” of Motavkin, which perforate the sheath of the spinal cord and ending with bush-like receptors close to vessels or near the ependyma of the central canal. Data on all known intracortical interoceptors in vertebrates are generalized into a single scheme. Computerized time-lapse video recording was used to detect the process of formation of syncytial couplings between the processes of different neurons in dissociated neuron cultures. These studies showed that once the processes of one neuron had formed connections with another neuron, death of the cell body (its trophic center) was not followed by Wallerian degeneration. Translocation of cytoplasmic varicosities along the branches of one neuron to another was observed over periods of several hours. Electron microscopic studies of the nerve processes of cells in the intramural intestinal plexus in the early postnatal period demonstrated all the transitional states from fusion and perforation of the membranes of contacting dendrites to complete fusion of the neuroplasm of processes with formation of residual membranous structures at the location of the former intercellular contact. __________ Translated from Morfologiya, Vol. 131, No. 2, pp. 7–15, March–April, 2007.  相似文献
