Preoperative care of infants with nephroblastoma. The International Society of Pediatric Oncology 6 experience.

The International Society of Pediatric Oncology (SIOP) recommends preoperative treatment in the management of eligible patients with Wilms' tumor. Until 1980, children younger than 12 months of age (infants) at diagnosis had been excluded from the SIOP trials. SIOP 6, conducted from 1980 to 1987, was the first SIOP study to include infants older than 6 months of age. This retrospective analysis of 145 infants registered to SIOP 6 demonstrates that in infants older than 6 months and having favorable histology (FH), a two-drug preoperative chemotherapy (CT) regimen of 4 weeks significantly ameliorated stage distribution as determined at delayed surgery but did not affect a good outcome. However, the CT dose utilized in SIOP 6 resulted in an unacceptable toxicity in this age group, and SIOP 9, the new SIOP study of Wilms' tumor, recommends a reduced dose of CT in infants. Preoperative CT is not recommended in infants younger than 6 months of age. Specifically, the high incidence (29%) of mesoblastic nephroma in this age group does not justify such an approach. Histopathologic diagnosis should be obtained in these patients before any treatment.     

Long-term results after 'floppy' Nissen/Rossetti fundoplication for gastroesophageal reflux disease.

Eighty-two patients (median age, 51 years; range, 27-87 years) with refractory gastroesophageal reflux disease (GERD) underwent floppy Nissen/Rossetti fundoplication during the period 1980 to 1990. The median postoperative hospital stay was 5 days (3-16). Median follow-up was 72 months and included endoscopy and a clinical and a questionnaire assessment. Subjective symptoms of reflux were abolished in 98%, and 96% of the patients were satisfied with the results. Endoscopy showed healing of the esophagitis in 98%. There had been no disruption of the fundoplication. Twenty patients complained of gaseous distention and increased flatulence; only two of them had severe problems. Three patients were unable to belch. In conclusion, the floppy fundoplication has been an effective operation with no deaths, a low incidence of morbidity and adverse side effects, and without a tendency for late failure.     

The concept of compensation in cognitive aging: the case of prose processing in adulthood.

Although the rule of cognitive aging appears to be one of generalized decline with advancing age, much recent theoretical and empirical work has addressed a variety of potential exceptions to this rule. One mechanism through which decline might not be experienced is known as compensation. We describe a conceptualization of compensation that applies to lifespan cognitive development, in general, and the aging of prose processing skills, in particular. Our model of the forms and processes of compensation directs attention to the developing dynamics of cognitive skills and compensatory mechanisms. We outline three main approaches to research on compensation in cognitive aging: a) the logical experimental, b) on-line observation and manipulation, and c) several verbal report procedures. In summarizing our own research program, we emphasize that a) each of these approaches can render valuable information, b) each have some limitations, and c) there are potential theoretical benefits from employing more than one of them in a given study.     

纤维支气管镜检查对肺癌诊断价值分析

本文报告我院从１９８３年１２月以来曾行３８００例次纤维支气管镜（简称纤支镜）检查，其中经病理证实和临床随访，最后证实肺癌者９８０例。鳞癌４５０例（４５．９１％），小细胞未分化癌２００例（２０．４％），腺癌１８０例（１８．４％），大细胞未分化癌１０例（１．０２％），肺泡细胞癌８例（０．８２％），粘液表皮样癌５例（０．５１％），病理报告癌但未分类５７例（５．８％），病理未证实，但纤支镜窥见病变，后经化疗或放疗有效，最后死于非其它疾病者７０例（７．１％）。纤支镜检阳性率为７９．５４％，同时讨论纤支镜检对本病的诊断价值。     

Relative parathyroid insufficiency in hypomagnesemia]

In patients with normocalcaemic tetany and hyperventilation syndrome (functional breathing syndrome), respectively, the parathyroid function was studied by means of the EDTA test according to Kaiser and Ponsold. In agreement with experimental results on inhibition of the parathyroid hormone release in magnesium deficiency patients with low serum magnesium concentrations show a pathological EDTA test signalling a relative parathyroid insufficiency.     

Clavicular fractures after radical neck dissection

Fracture of the medial end of the clavicle as a late complication following radical neck dissection is rare, with an incidence of approximately 0.4%-0.5%. Radical neck dissection alters the mechanics of the shoulder girdle, and distorts normal muscle forces acting on the clavicle. We present two patients to illustrate the problems of differential diagnosis and mechanism of fracture. Once the diagnosis has been made the fracture should be managed conservatively, although clinical and radiological differentiation from avascular necrosis or bony metastasis may be difficult. Biopsy is essential in doubtful cases.     

Exchange transfusion in children with blastic hyperleukocytosis in acute lymphoblastic leukemia

In 10 children with acute lymphoblastic leukaemia (mean leucocyte count 452 x 10(9)/l) 17 exchange transfusions were done for rapid reduction of this count. The control group comprised 11 children with similar values of white blood cell counts (mean 324 x 10(9)/l) treated initially with small, increasing doses of corticosteroids. In contrast to acute myeloid leukaemia the results do not support the usefulness of exchange transfusions in children with hyperleukocytosis during ALL.
