Associated with a host of different diseases, pruritus is a cardinal symptom that poses an interdisciplinary diagnostic and therapeutic challenge. Over time, that symptom may progress independently of the initial cause, thus losing its function as a warning sign and turning into a clinically relevant disease of its own. In Germany, approximately 13.5 % of the general population are affected by chronic pruritus, with an incidence of 7 %. All forms of chronic pruritus require targeted treatment consisting of (a) diagnosis and management of the underlying disease, (b) dermatological treatment of primary or secondary (for example, dry skin, scratch lesions) symptoms, (c) symptomatic antipruritic treatment, and (d) psychological/psychotherapeutic treatment in case of an underlying or associated psychological or psychosomatic condition. Medical care of patients with chronic pruritus should therefore include an interdisciplinary approach, in particular with respect to diagnosis and therapy of the underlying disease as well as in terms of the management of treatment and adverse events. The objective of the present interdisciplinary guidelines is to define and standardize diagnostic and therapeutic procedures in patients with chronic pruritus. Excessive dorsiflexion (dorsal tilting) of the lunate on a lateral wrist radiograph can be an important sign of carpal injury. Lunate dorsiflexion is a well-recognized sign of an intercarpal ligamentous injury pattern known as dorsal intercalated segment instability (DISI). It is less well recognized that excessive dorsal tilting of the lunate (DISI configuration) can also be produced by displacement of a scaphoid waist fracture. Since the management and prognosis of displaced scaphoid fractures may be quite different from those for nondisplaced fractures, radiologists can make an important contribution by recognizing dorsal tilting of the lunate and appreciating that it may be an important, indirect sign of scaphoid fracture displacement, which may not be directly visualized with standard wrist radiography. In this setting, computed tomography or complex motion tomography may be helpful for further evaluation of the scaphoid fracture. 相似文献

Mutations in the connexin 26 (Cx26) gene (GJB2) are associated with autosomal nonsyndromic sensorineural hearing loss. This study describes mutations in the Cx26 gene in cases of familial and sporadic hearing loss (HL) by gene sequencing and identifies the allelic frequency of the most common mutation leading to HL (35delG) in the population of eastern Austria. For this purpose we have developed and applied a molecular beacon based real-time mutation detection assay. Mutation frequencies in the Cx26 gene of individuals from affected families (14 out of 46) and sporadic cases (11 out of 40) were 30.4% and 27.5%, respectively. In addition to known disease related alterations, a novel mutation 262 G-->T (A88S) was also identified. 35delG accounted for almost 77% of all Cx26 mutations detected and displayed an allelic frequency in the normal hearing population of 1.7% (2 out of 120). The high prevalence of the 35delG mutation in eastern Austria would therefore allow screening of individuals and family members with Cx26 dependent deafness by a highly specific and semi-automated method. 相似文献

PURPOSE OF REVIEW: Recent studies have gained widespread information about the complex regulation of genetic, environmental, immunologic, and pharmacologic factors that contribute to the development of allergic inflammatory skin diseases such as atopic dermatitis. Neuroimmune mechanisms, however, still remain to be elucidated. This review will focus on the interaction between the cutaneous immune and peripheral nervous system in allergic inflammatory skin such as atopic dermatitis. RECENT FINDINGS: Neuropeptides and neuropeptide-positive nerve fibres are prominently increased in lesions of atopic dermatitis. The density of nerve fibres is increased while peripheral nerve endings are in an active state of excitation. In this regard, neurotrophins particularly described for their functional role on nerve cells are also expressed in atopic dermatitis skin. In addition, neurotrophins modulate the functional role of eosinophils as main target effector cells in atopic dermatitis, as described recently. Interestingly, eosinophils are capable of neurotrophin as well as neuropeptide production itself, pointing to a bidirectional communication between neuronal cell populations and main target effector cells. SUMMARY: Neurotrophins and neuropeptides modulate both the functional activity of sensory neurons and immune cells. We have therefore developed the concept of a neuroimmune network between target effector cells and sensory nerves that links pathogenic events to dysfunctions of the cutaneous immune and peripheral nervous system in allergic inflammatory skin diseases. 相似文献

Chagas' disease patients (CDP) develop both humoral and cellular immune
responses against the major cysteine proteinase (cruzipain) from
Trypanosoma cruzi. Here we demonstrate that complexes formed by cruzipain
and alpha 2-macroglobulin (alpha 2M) are efficiently internalized by human
monocytes, and that this process results in enhanced presentation of
cruzipain peptides to CD4+ T cells from CDP. Purified or serum alpha 2M
binds to polymorphic cruzipains, but only a fraction of the proteinases
become covalently linked. Once bound to alpha 2M, fluorescein-labeled
cruzipain (FITC-cruzipain) or [125I]cruzipain were more efficiently
internalized by normal peripheral blood mononuclear cells (PBMC) or
monocytes; this effect was abolished by (I) pre-treating the cells with
receptor-associated protein (rRAP), a known antagonist the of alpha 2M
receptor (alpha 2MR/LRP), and (II) inactivating [125I]cruzipain's active
site prior to the reaction with alpha 2M, indicating that the exposure of
receptor binding sites on alpha 2M complexes required bait region cleavage.
We then sought to determine if the alpha 2MR/LRP-dependent uptake of alpha
2M:cruzipain by monocytes resulted in increased CD4+ T cell responses of
PBMC-CDP (n = 13). These effects were only revealed after depletion of
CD19+ B lymphocytes from PBMC-CDP; the threshold of T cell stimulation was
far lower in cultures stimulated with alpha 2M:cruzipain, as compared to
antigen alone. Myocardial specimens from CDP with chronic myocardiopathy
(three necropsies) were analyzed by immunohistochemistry with mAb
anti-cruzipain or anti-alpha 2MR/LRP (CD81+). Extracellular depots of
cruzipain were localized amidst inflammatory mononuclear infiltrates, part
of which contained CD91+ macrophage-like cells. Ongoing studies should
clarify if T. cruzi cysteinyl proteinases play a role in the pathogenesis
of Chagas' heart disease.

Simultaneous molecular karyotyping and mapping of viral DNA integration sites by 25-color COBRA-FISH

Combined binary ratio labeling (COBRA) fluorescence in situ hybridization (FISH) allows 24-color FISH karyotyping of human metaphase chromosomes utilizing only four fluorochromes, instead of the five required for combinatorial labeling procedures. Here we show that by introduction of a fifth fluorochrome, COBRA-FISH permits molecular cytogenetic mapping of viral integration sites in complex karyotypes in the context of a 24-color hybridization. We were able to detect a single copy of the human papillomavirus 16 in the SiHa cell line and to confirm the site of integration at 13q21-31. We also demonstrate the gene mapping possibility of 25-color hybridization by detecting a MYC cosmid on normal metaphase chromosomes. The possibility of mapping single-copy probes in the background of 24-color hybridization expands the tools for cytogenetic mapping of DNA sequences and will contribute to the understanding of the role of viral integration and chromosome rearrangement in virus-mediated carcinogenesis. 相似文献
