OBJECTIVE: This study investigated the effects of a soy-based low-calorie diet on weight control, body composition, and blood lipid profiles compared with a traditional low-calorie diet. METHODS: Thirty obese adults (mean body mass index 29-30 kg/m(2)) were randomized to two groups. The soy-based low-calorie group consumed soy protein as the only protein source, and the traditional low-calorie group consumed two-thirds animal protein and the rest plant protein in a 1200 kcal/d diet for 8 wk. A diet record was kept everyday throughout the study. Food intake was analyzed before and after the study. Anthropometric data were acquired every week, and biochemical data from before and after the 8-wk experiment were compared. RESULTS: Body weight, body mass index, body fat percentage, and waist circumference significantly decreased in both groups (P < 0.05). The decrease in body fat percentage in the soy group (2.2%, 95% confidence interval 1.6-2.8) was greater than that in the traditional group (1.4%, 95% confidence interval -0.1 to 2.8). Serum total cholesterol concentrations, low-density lipoprotein cholesterol concentrations, and liver function parameters decreased in the soy-based group and were significantly different from measurements in the traditional group (P < 0.05). No significant change in serum triacylglycerol levels, serum high-density lipoprotein cholesterol levels, and fasting glucose levels was found in the soy or traditional group. CONCLUSION: Soy-based low-calorie diets significantly decreased serum total cholesterol and low-density lipoprotein cholesterol concentrations and had a greater effect on reducing body fat percentage than traditional low-calorie diets. Thus, soy-based diets have health benefits in reducing weight and blood lipids.  相似文献

OBJECTIVES: To define the accuracy (compared with magnetic resonance imaging [MRI]) of sonographic examination in detecting knee effusion and to determine whether the presence of knee effusions in patients with traumatic knee injury can predict knee internal derangement as assessed by MRI. DESIGN: Prospective study. SETTING: Hospital rehabilitation department. PARTICIPANTS: Thirty patients (19 men, 11 women) with traumatic knee injury were recruited. Subjects received sonographic examination and MRI on the same day. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: The presence or absence of knee effusion was assessed by sonographic examination. MRI was used as criterion standard to evaluate whether the presence of knee effusion and internal derangement, which included tear of anterior and posterior cruciate ligaments, as well as meniscus tear. RESULTS: The sensitivity of sonographic examination for detecting knee effusion was 79.1%, and specificity was 50%. The positive-predictive value (PPV) was 86.3% and negative-predictive value (NPV) was 37.5%. The PPV of sonographic effusion to internal derangement was 90.9%, and the NPV was 37.5%. CONCLUSIONS: Sonographic examination can accurately detect effusion of the knee. The detection of knee effusion in patients with traumatic knee injury by sonographic examination is highly indicative of internal knee derangement.  相似文献

Here, we report the neuroimaging findings and neurological changes in 17 unpublished patients with Macrocephaly-Capillary Malformation (M-CM). This syndrome has been traditionally known as Macrocephaly-Cutis Marmorata Telangiectatica Congenita (M-CMTC), but we explain why M-CM is a more accurate term for this overgrowth syndrome. We analyzed the 17 patients with available brain MRI or CT scans and compared their findings with features identified by a comprehensive review of published cases. White matter irregularities with increased signal on T2-weighted images were commonly observed findings. A distinctive feature in more than half the patients was cerebellar tonsillar herniation associated with rapid brain growth and progressive crowding of the posterior fossa during infancy. In four such cases, we confirmed that the tonsillar herniation was an acquired event. Concurrently, with the development of these findings, ventriculomegaly (frequently obstructive) and dilated dural venous sinuses were observed in conjunction with prominent Virchow-Robin spaces in many of those in whom cerebellar tonsil herniation had developed. We postulate that this constellation of unusual features suggests a dynamic process of mechanical compromise in the posterior fossa, perhaps initiated by a rapidly growing cerebellum, which leads to congestion of the venous drainage with subsequently compromised cerebrospinal fluid reabsorption, all of which increases the posterior fossa pressure and leads to acquired tonsillar herniation. We make a distinction between congenital Chiari I malformation and acquired cerebellar tonsil herniation in this syndrome. We also observed numerous examples of abnormal cortical morphogenesis, including focal cortical dysplasia, polymicrogyria which primarily involved the perisylvian and insular regions, and cerebral and/or cerebellar asymmetric overgrowth. Other findings included a high frequency of cavum septum pellucidum or vergae, thickened corpus callosum, prominent optic nerve sheaths and a single case of venous sinus thrombosis. One patient was found to have a frontal perifalcine mass resembling a meningioma at age 5 years. This is the second apparent occurrence of this specific tumor in M-CM.  相似文献

In this study, a novel magnetic degradable material was developed by adding Fe ions into DP-Bioglass (Na(2)O-CaO-P(2)O(5)-SiO(2)) as thermoseed for hyperthermia cancer therapy under an alternating magnetic field. We have investigated the properties of developed magnetic DP-Bioglass including morphology, chemical composition, and magnetism. The degradability was conducted by measuring the released concentrations of Na, Ca, Si, P, and Fe ions. The biocompatibility was analyzed by biological assays, and the functional hyperthermia effect to cancer cells was evaluated by in vitro cell culture test. In the results, the morphology of synthesized magnetic DP-Bioglass was revealed in sphere and rod shape with particle size around 50-100 nm. From the hysteresis loop analysis, it showed that the group of Fe/Bioglass = 0.2 possessed the maximum magnetization property. When cultured with fibroblasts, the magnetic DP-Bioglass had no significant influence on cell viability and mediated low cytotoxicity. The thermal-induced property demonstrated that after exposure to an alternating magnetic field, the cell number of human Caucasian lung carcinoma cells (A549) was significantly decreased when temperature was increasing to 45 degrees C. In brief, successfully incorporated with Fe ions by sol-gel method, this magnetic degradable DP-Bioglass possessed the potential and properties of hyperthermia effect to lung carcinoma cells.  相似文献

A 51-year-old male presented with sudden onset lower abdominal pain followed by weakness of both legs. Examination revealed blood pressure of 220/130 mmHg, with a grade 2/6 systolic murmur audible at the apex of the heart, and absence of both femoral arterial pulses. Two-dimensional and transesophageal echocardiography showed no evidence of intracardiac tumor or dissection of the ascending and thoracic aorta. Moreover, an aortogram demonstrated total occlusion of the abdominal aorta just below the renal arteries. A myxomatous-like material occupying the abdominal aorta just above the bifurcation of the common iliac arteries was discovered during surgery. Histologic examination of the embolic material confirmed the diagnosis of myxomatous embolus. One year after the embolic episode, the patient was well and two-dimensional and transesophageal echocardiography revealed no evidence of residual intracardiac tumor.  相似文献

ObjectiveTo identify autonomic dysregulation in frontal lobe epilepsy (FLE).MethodsWe studied 14 male and 11 female subjects with FLE and an equal number of matched healthy control subjects. Lead I electrocardiograms were obtained for 5 min in the interictal state during daytime. Frequency-domain analysis of heart rate variability was performed and the data subsequently converted to heart rate interval and high frequency (HF; 0.15–0.45 Hz) power which representing vagal or parasympathetic regulation, as well as low frequency (LF; 0.04–0.15 Hz) power and LF/(HF + LF) expressed in normalized units (LF%) (considered to mirror sympathetic regulation). Differences in data between groups were compared using t-test.ResultsThe epilepsy group had a lower mean heart rate interval and a lower high frequency power.ConclusionsPatients with FLE have interictally faster heart rates, attributed to lower parasympathetic drive, which may contribute to the higher incidence of sudden death that is seen in this group of patients. This suggests that the mechanism of decreased HRV in patients with FLE is probably different from that in patients with temporal lobe epilepsy.  相似文献

This in vitro study aimed to investigate the influence of flowable composite lining with different thicknesses on the marginal quality and internal porosity of Class II composite restorations. Thirty-two intact molars, each prepared with two box-only Class II cavities, were randomly divided into four groups: Group 1, P60 filling alone; Group 2, ultrathin flowable composite lining/co-cured with overlaying composite; Group 3, thin lining/pre-cured and Group 4, thick lining/pre-cured. The teeth were then thermocycled for 1500 cycles (between 5 degrees C and 60 degrees C) and dye immersed for 24 hours. Exterior surface replicas of these restorations were fabricated before and after thermocycling and examined by SEM to evaluate percentages of the five marginal patterns. Data was statistically evaluated using one-way ANOVA test. The teeth were subsequently sectioned longitudinally. The interface microleakage of cervical margin was measured as to the extent of dye penetration. Internal voids were separately recorded in the cervical interface and the cervical and occlusal halves of the restorations. Mann-Whitney test was applied to analyze the interface microleakage and internal voids. Results revealed that replicas of Group 4 presented the highest percentage of marginal openings both before and after thermocycling in SEM examination. Group 2 exhibited superior marginal quality in interface microleakage evaluation compared to the other groups, while Group 4 exhibited the worst. The pre-cured groups (Group 3 and 4) showed significant reduction in interface and cervical voids. Despite the reduction in interface voids, a thick lining may impair the marginal sealing, especially after thermocycling. It was concluded that a minimally thin flowable composite lining improved cavity adaptation and marginal sealing.  相似文献
