Purpose

According to the “fundamental cause theory”, emerging knowledge on health-enhancing behaviours and technologies results in persisting and even widening health disparities, favouring the higher socioeconomic groups. This study aims to assess (trends in) socioeconomic inequalities in site-specific cancer mortality in Belgian women.

Methods

Data were derived from record linkage between the Belgian census and register data on mortality for 1991–1997 and 2001–2008 for all Belgian female inhabitants aged 50–79 years. Both absolute and relative inequalities by education and housing conditions were calculated.

Results

The results revealed persisting socioeconomic inequalities in total and site-specific cancer mortality. As expected, these inequalities were larger for the more preventable cancer sites. Generally, socioeconomic inequalities remained quite stable between the 1990s and the 2000s, although for some preventable cancer sites (e.g., uterus and oesophagus) a significant increase in inequality was observed.

Conclusions

These persisting socioeconomic inequalities are likely due to differences in exposure to risk factors and unhealthy behaviours, and access and utilization of healthcare across the social strata. Since equality in health should be a priority for a fair public health policy, efforts to reduce inequalities in risk behaviours and access and use of health care should remain high on the agenda.

OBJECTIVES: This study investigated whether work dedication and job resources are longitudinally related to work-related musculoskeletal disorders and whether job resources buffer the impact of job demands on these disorders? METHODS: Data were used from a longitudinal three-phase study (2004, 2005, 2006) on health at work among a sample of Dutch workers. The first survey was sent in 2004 by e-mail to 3100 members of an existing panel. For the analyses, 1522 participants were included with full longitudinal data. The analyses were performed using an autoregressive model with generalized estimating equations. RESULTS: The job-resource quality of communication was found to predict the risk of work-related musculoskeletal disorders over time. This effect was not mediated by work dedication. A high quality of communication was also found to buffer the negative effects of a high physical workload on the risk of work-related musculoskeletal disorders. Furthermore, a low level of social support by colleagues was found to buffer the negative effect of a medium physical workload on work-related musculoskeletal disorders. CONCLUSIONS: This study shows that job resources are not only important for promoting work dedication, but may also moderate the negative impact of high job demands on the risk of work-related musculoskeletal disorders. With respect to social support, the question is raised of whether this can also work negatively. The results of this study imply that, besides avoiding or reducing risks to health in the workplace and lowering job demands, strengthening job resources may additionally buffer harmful effects of job demands on musculoskeletal health.  相似文献

Click-evoked otoacoustic emissions (CEOAEs) from preterm infants were analyzed to characterize developmental changes of cochlear active mechanisms. Due to their strong time-varying properties, CEOAEs were studied with a time-frequency approach--the wavelet transform (WT). By means of the WT, CEOAEs were decomposed into 12 frequency bands, spanning the 0.25-6.25 kHz range. For each band, the root-mean-square (RMS) level and latency were studied as functions of both frequency and age. Because CEOAEs were averaged using the non-linear mode of acquisition, the developmental changes in observed in this study are related to the non-linear component (which is actually the most predominant component of the active cochlear response) of CEOAEs, the linear one being mostly canceled out by non-linear averaging. In our study, there was evidence that properties of CEOAE non-linear components are related to the post-conception age (PCA) in that the levels and latency of CEOAE frequency components changed until the age of about 38 weeks post-conception, whereas after 38 weeks, CEOAE features were very similar to those of term newborns. In particular, the CEOAE levels increased and latency decreased with age. The observed changes in CEOAE properties seem to reveal a development of cochlear active mechanisms, although contributions from outer and middle ear development cannot be excluded. Also, in agreement with previous physiological and behavioral findings, our results revealed that the development of CEOAE properties was not the same for all the frequencies, being greater for frequencies 4 kHz, and resembled the development of the cochlear partition, which proceeds from base to apex.  相似文献

Rett syndrome in females with CTS hot spot deletions: a disorder profile   总被引:2,自引:0,他引:2  
From a series of 107 females with Rett syndrome (RTT), we describe the long-term history of ten females with a deletion in the C-terminus of the MECP2 gene. We observed that their disorder profile is clinically recognizable with time and different from other atypical and milder RTT phenotypes. In females with hot spot deletions in the C-terminus, dystonia is present from childhood and results in a serious spine deformation in spite of preventive measures. Their adaptive behavior is surprisingly better preserved and in contrast with the typical decline in motor functioning. The delineation of disorder profiles by long-term clinical observation can teach us about genotype/phenotype relationships and eventually about the effect of epigenetic phenomena on the final phenotype.  相似文献
