Antidepressant and antipsychotic drugs are regularly encountered in different aspects of forensic toxicology, and some cases require the examination of hair samples. In this study, common antidepressant and antipsychotic drugs regarding hair concentrations over the past decades were reviewed. Although numerous publications around method validations, case reports, or controlled dose studies were found, apparently there is a lack of comprehensive data for many substances. Information on the hair length and dosage across the publications varied largely, and case numbers were generally low except for several retrospective controlled dose studies. Many substances were described only in method validations or case reports, and data were obtained from small case numbers. On the contrary, clozapine, haloperidol, amitriptyline, nortriptyline, risperidone and its metabolite, methylphenidate, citalopram, chlorpromazine, chlorprothixene, and quetiapine had a well‐founded database as these substances were investigated in controlled dose studies with higher case numbers. Given the advancements made in analytical techniques over the past years, gas chromatography–mass spectrometry and liquid chromatography with tandem mass spectrometry techniques were the methods of choice and allowed the detection of chemical compounds at low concentrations. The controversy around a potential use of hair analysis to estimate the dosage remains as dose‐concentration studies provided divergent results. A harmonization on the investigated hair length as well as on the extraction protocol would be of favor to achieve better comparability. Although hair analysis research focused mainly on drug abuse, availability of more data on antidepressants and antipsychotics would help to gain better knowledge and assist other forensic investigators. 相似文献

Chronic inflammatory diseases of the lung are leading causes of morbidity and mortality worldwide. Many of these disorders can be attributed to abnormal immune responses to environmental stimuli and infections. As such, understanding the innate host defense pathways and their regulatory systems will be critical to developing new approaches to treatment. In this regard, there is increasing interest in the role of microRNAs (miRNAs) in the regulation of pulmonary innate host defense responses and the inflammatory sequelae in respiratory disease. In this review, we discuss recent findings that indicate an important role for miRNAs in the regulation in mouse models of various respiratory diseases and in host defense against bacterial and viral infection. We also discuss the potential utility and limitations of targeting these molecules as anti-inflammatory strategies and also as a means to improve pathogen clearance from the lung. 相似文献

Background: The assessment of physical activity intensity and duration is essential for understanding group activity patterns.Methods: The present study evaluated the validity of measurement of total energy expenditure (TEE) and physical activity level (PAL) using a categorized physical activity diary. In 29 young healthy men, aged 18–27 years, with body mass index range 21–43 kg m?2, TEE using doubly-labelled water (DLW), resting metabolic rate (RMR) by indirect calorimetry, physical activity level (PAL defined as TEE/RMR) and activity pattern, timing and level from 7-day physical activity diaries were determined.Results: TEE by DLW and estimated by activity diary were correlated (r?=?0.61, p?=?0.005). The mean underestimation of TEE by the activity diary compared with the DLW method was 2.50?±?0.72 MJ day?1. Sedentary (lying, sitting and standing) time averaged 18 h day?1 and was negatively correlated with PAL (r?=??0.44, p?=?0.018) whilst time spent in light intensity activity (5 h day?1) was positively correlated (r?=?0.51, p?=?0.005).Conclusions: It is concluded that the categorized physical activity diary measured TEE with limited accuracy but presents an inexpensive, convenient method of discriminating individual and group physical activity patterns. 相似文献

ObjectivesEmotional distress can be a potential barrier to shared decision making (SDM), yet affect is typically not systematically assessed in medical consultation. We examined whether urological patients report anxiety or depression prior to a consultation and if emotional distress predicts decisional conflict thereafter.MethodsWe recruited a large sample of urological outpatients (N = 397) with a range of different diagnoses (42 % oncological). Prior to a medical consultation, patients filled in questionnaires, including the Hospital Anxiety and Depression Scale. After the consultation, patients completed the Decisional Conflict Scale. We scored the rate of anxiety and depression in our sample and conducted multiple regression analysis to examine if emotional distress before the consultation predicts decisional conflict thereafter.ResultsAbout a quarter of patients reported values at or above cut-off for clinically relevant emotional distress. Emotional distress significantly predicted a higher degree of decisional conflict. There were no differences in emotional distress between patients with and without uro-oncological diagnosis.ConclusionsEmotional distress is common in urology patients – oncological as well as non-oncological. It predicts decisional conflict after physician consultation.Practice ImplicationsEmotional distress should be systematically assessed in clinical consultations. ObjectiveTo predict the clinically relevant transmitted irradiance that is available for luting when a CAD/CAM restoration is inserted. The influence of irradiance, exposure distance, light curing unit (LCU) angulation and direction of polymerization is analyzed when curing through crowns of different thicknesses.MethodsThree modern CAD/CAM resin-based composites (RBCs) were used to produce 45 crown-shaped specimens. The distance between fissure and crown base was set at 1.0, 1.5 and 2.0 mm (n = 5). Transmitted irradiance, while using a violet-blue LCU, was measured with a photo-spectrometer. 180 exposure conditions per specimen were investigated by variation in LCU curing mode, angulation, exposure distance and direction. Data was analyzed using univariate ANOVA followed by Tukey HSD (α = 0.05) and comparison of 95% confidence intervals.ResultsThe CAD/CAM-RBC’s decadic absorption coefficient ranges from 0.317 mm?1 to 0.387 mm?1 and the reflection correcting factor for crowns ranges from 0.305 to 0.337. Transmitted irradiance decreases significantly with increasing exposure distance and decreasing incident irradiance. For tilt angles greater than 10°, transmitted irradiances are significantly reduced (?11% for 20°, ?23% for 30°). Significantly lowest transmitted irradiances were measured for vestibular curing direction (up to ?15%).SignificanceA calculation model can predict the transmitted irradiance through a CAD/CAM restoration in dependence of restoration thickness and radiant emittance. The practitioner can be supported by this model to adapt material choice of dental restoration and adhesive system to the individual situation.
