Objective: The guiding criteria are considered the backbone of Chinese medicine. They have previously been described as functional features (symptoms) leading to the overall assessment of human functions on the basis of a regulatory (cybernetic) model referring to the I Ging. Methods: The Heidelberg model can explain symptoms such as created by "heat" on a rational physiological level. Results & Conclusion: The overall of physiological symptoms are shown as a schematic draft. The basis of "heat" is considered to be a general increase of microcirculation in the periphery. This leads to a couple of local pathophysiological consequences and sensations like 1) red tongue (the tongue is considered an embryological somatotopic system). 2) Sensation of warmth (by increase of capillary flow). 3) pre-inflammatory state, leading to pain modalities like "worse if pressed", as inflammations tend to be increasingly painful under pressure; 4) reddish skin, the mechanisms by which this is induced may include the release of substance P, therefore accompanied by burning sensation. Systemic pathophysiological consequences may include. Relative lack of fluid in the larger vessels, as fluid supplies peripheral capillary flow. This may lead to water saving mechanisms like thirst, dry mucosa with do, mouth, dry nose, dry lips, dry skin, and also dry stool, yellow and sparse urine. 相似文献

Carcinoma of the breast in women less than 40 years old is a significant problem, representing 7.4 per cent of the total number of carcinomas of the breast at our institution. It is apparent that mammography in this age group is of value in confirming the clinical suspicion of a malignant condition of the breast and in evaluating the remainder of the parenchyma of the breast for clinically occult lesions. However, one cannot over emphasize that a negative result from the mammogram should not delay biopsy in the presence of clinically suspicious lesions (1, 4). OBJECTIVE: While hearing preservation has become an accepted treatment goal, there are still major doubts whether and to what extent this aim is feasible in large tumors that cover the auditory nerve and involve the brainstem. METHODS: One thousand eight hundred (1,800) vestibular schwannomas (VS) were operated on at Nordstadt Neurosurgical Department from 1978 to 1999 by the senior surgeon (MS). There were 1,800 tumors in 1,750 patients, 1,605 patients had unilateral tumors, 145 patients had neurofibromatosis type 2 with bilateral tumors, 195 of which were treated surgically in our unit. Preservation of the cochlear nerve was attempted when ever possible. The audiometric data are analyzed by Hannover Classification System graded in steps of 20 dB by audiometry and in steps of 10% to 30% by speech discrimination. Those data were set into relation with the Hannover tumor extension grading T1 to T4. RESULTS: Overall preservation rate was 40% with considerable differences, though, depending on the preoperative hearing quality and the tumor extension. Best results were obtained for intrameatal tumors (T1) with 56% and small intra-extrameatal tumors (T2) with 57% preservation rate. While preservation rate was at 44% in medium-sized tumors (T3), it was at 20% for T4 tumors with brainstem compression or even dislocation. However, satisfactory chances were encountered in cases with normal or good preoperative hearing, on average 54%, specifically 71% in T1, 69% in T2, 58% in T3 and 29% in T4. Regarding the quality of the preserved hearing, up to one third of cases in T1 and T2 may retain their preoperative hearing level, while this ranges around 20% for T3 and 10% to 14% in T4 tumors. DISCUSSION: For long, hearing preservation in medium sized or large tumors has not received any attention because of the presumed lack of realistic chances and the expected bad quality. Obviously, hearing preservation may be achieved in relevant numbers and at satisfactory quality levels. This aspects appears important to be addressed further in view of the fact that over 70% of tumors are presented for surgery at T3 or T4 extensions. The purpose of this study was to determine the actual standardized uptake value (SUV) by using the lesion size from computer tomography (CT) scan to correct for resolution and partial volume effects in positron emission tomography (PET) imaging. This retrospective study included 47 patients with lung lesions seen on CT scan whose diagnoses were confirmed by biopsy or by follow up CT scan when the PET result was considered negative for malignancy. Each lesion's FDG uptake was quantified by the SUV using two methods: by measuring the maximum voxel SUV (maxSUV) and by using the lesion's size on CT to calculate the actual SUV (corSUV). Among small lesions (2.0 cm or smaller on CT scan), ten were benign and 17 were malignant. The average maxSUV was 1.43+/-0.77 and 3.02+/-1.74 for benign and malignant lesions respectively. When using an SUV of 2.0 as the cutoff to differentiate benignity and malignancy, the sensitivity, specificity, and accuracy were 65%, 70%, and 67% respectively. When an SUV of 2.5 was used for cutoff, the sensitivity, specificity, and accuracy were 47%, 80%, and 59% respectively. The average corSUV was 1.65+/-1.09 and 5.28+/-2.71 for benign and malignant lesions respectively. Whether an SUV of either 2.0 or 2.5 was used for cutoff, the sensitivity, specificity, and accuracy remained 94%, 70%, and 85% respectively. The only malignant lesion that was falsely considered benign with both methods was a bronchioalveolar carcinoma which did not reveal any elevated uptake of fluorine-18 fluorodeoxyglucose (FDG). Of the large lesions (more than 2.0 cm and less than 6.0 cm), one was benign and 19 were malignant and the corSUV technique did not significantly change the accuracy. It is concluded that measuring the SUV by using the CT size to correct for resolution and partial volume effects offers potential value in differentiating malignant from benign lesions in this population. The intensive care unit at Queen Elizabeth Central Hospital (QECH) has 4 beds and offers level 2 care. A retrospective audit of all admissions to the unit during 2002 was carried out. There were a total of 339 admissions giving a bed occupancy rate of 82 %. Surgical patients made up 81 % of admissions. 45% of all admissions were ventilated. Overall mortality was 38%. Ventilated patients had a mortality of 71% compared with 10% for non-ventilated. Data are also presented for mortality within the surgical and paediatric surgical admissions. 相似文献

E mu-pim-1 transgenic mice are predisposed to develop lymphomas. Due to
their low spontaneous tumour incidence and their increased sensitivity
towards the lymphomagen ethylnitrosourea these mice may present an
interesting model for short-term carcinogenicity testing. Here, we report
on the further exploration of this transgenic mouse model with two
additional carcinogens known to have, among others, the
lymphohaematopoietic system as target, i.e. benzo[a]pyrene (B[a]P) and
12-O-tetradecanoylphorbol-13-acetate (TPA). B[a]P, given three times a week
(by gavage) for 13 weeks at 4.3, 13 or 39 mg/kg body weight, resulted in a
dose-related increase in lymphomas up to a 90% incidence in E(mu)-pim-1
mice during the observation period of 40 weeks. B[a]P also induced tumours
of the forestomach within this observation period, though at a lower
incidence and apparently equally effective in wildtype and transgenic mice.
TPA, on the other hand, was unable to induce lymphomas (or tumours in any
other organ) in either transgenic or wildtype animals within the
observation period of 44 weeks, when applied dermally at the maximum
tolerated dose of 3 microg/mouse, twice a week for 35 weeks. Molecular
analysis showed that B[a]P-induced lymphomas in transgenic mice were of
T-cell origin, 80% of which had elevated levels of c-myc expression. None
of the lymphomas had increased N-myc expression and mutation analysis of
the ras-gene family revealed a K-ras mutation in only one out of eight
tumours investigated. Also, none of the lymphomas showed aberrant
expression of p53 as determined by immunohistochemistry. It is concluded
that the E mu-pim-1 mouse model will not be very suitable for short-term
carcinogenicity testing in general: only genotoxic chemicals that have the
lymphohaematopoietic system as target for carcinogenesis in wild- type
mice, appear to be efficiently identified.

Mucormycosis is a rare fungal infection of childhood, occurring mainly in
patients with chronic illnesses such as diabetes and malignancies. The
fungus seldom grows in culture and confirmation of the diagnosis depends on
histologic examination of infected tissues. To date, the reported natural
history of the disease has been rapid progression and a fatal outcome.
Therefore, the importance of early diagnosis by tissue biopsy and early
treatment with surgical debridement and systemic antifungal therapy cannot
be overemphasized. The pulmonary system is the most common site for
mucormycosis in patients with leukemia. We report what we believe to be the
first successfully treated case of isolated muscular mucormycosis occurring
in a child with biphenotypic acute leukemia. The diagnosis was made
promptly by tissue examination at the time of surgical debridement. The
patient was also given systemic amphotericin-B therapy.

Data on body composition in conjunction with reference centiles are helpful in identifying the severity of growth and nutritional disorders in infancy and for evaluating the adequacy of treatment given during this important period of rapid growth. Total body fat (TBF) and fat-free mass (FFM) were estimated from total body electrical conductivity (TBEC) measurements in 423 healthy term Caucasian infants, aged 14-379 days. Cross sectional age, weight, and length related centile standards are presented for TBF and FFM. Centiles were calculated using Altman's method, based on polynomial regression and modelling of the residual variation. The TBF percentage steeply increased during the first half year of life, and slowly declined beyond this age. Various simple TBEC derived anthropometric prediction equations for TBF and FFM are available to be used in conjunction with these standards. Regression equations for the P50 and the residual SD, depending on age, weight, or length, are provided for constructing centile charts and calculating standard deviation scores. 相似文献
