Comparison of pronator tenotomy and pronator rerouting in children with spastic cerebral palsy 总被引:1,自引:0,他引:1
Forty-one patients with cerebral palsy and pronation contracture of the forearm were treated with pronator teres rerouting compared with 16 patients who were treated with pronator teres tenotomy. The mean age of patients with pronator tenotomy was 4 years 3 months compared with 7 years 3 months for patients with rerouting. Follow-up averaged 94 months for tenotomy and 21 months for rerouting. Average gain in supination was 78 degrees for rerouting and 54 degrees for tenotomy. No patient lost active range of motion during follow-up. Although pronator teres tenotomy increased active supination of the forearm, greater active supination of the forearm was afforded patients treated with pronator teres rerouting. 相似文献

Objective: It has been shown that adiponectin serves as an insulin-sensitizing adipokine. Serum concentrations of adiponectin are low in children with obesity, and increase with fat mass loss, indicating that adiponectin can serve as a biomarker. Since the prevalence of overweight and obesity is increased in children with congenital adrenal hyperplasia (CAH), our study aimed to evaluate serum levels of adiponectin in a cohort of CAH children and adolescents, and their associations with clinical parameters such as chronological age (CA), body mass index (BMI), Tanner stage (TS), medication and metabolic control.
Patients and methods: We studied 51 patients, aged between 5.6 and 19.6 years (median 11.8; 30 females, 21 males), cross-sectionally. All patients had genetically confirmed CAH and received standard steroid substitution therapy. Adiponectin was measured by an enzyme linked immunoassay. Since BMI SDS of the CAH cohort were significantly higher compared to the reference population, we built matched pairs with healthy Caucasian subjects from a normal representative cohort for sex, Tanner stage, chronologic age and BMI.
Results: Adiponectin concentrations were significantly higher in CAH patients (median 11 μg/L) compared to the matched controls (6.7 μg/L, p < 0.0001). Correlation analyses in CAH patients revealed a significant inverse relationship between adiponectin and CA, TS, BMI, serum DHEAS and serum testosterone, but no correlation with hydrocortisone and fludrocortisone dosage.
Conclusion: Currently, the importance of the elevated adiponectin concentrations in CAH children for risk assessment is not clear. However, our data imply that besides adequate metabolic control of glucocorticoid substitution, a long-term follow-up of other metabolic markers of insulin resistance should be conducted in CAH patients. 相似文献

At the Department of Radiation Oncology, Westmead Hospital, between 1980 and 2000, 60 patients with squamous cell carcinoma of anal canal or margin (including 15 with Stage IIIA or IIIB) were treated radically; 55 received chemoradiation (89% were prescribed mitomycin C and 5‐fluorouracil). Five‐year overall survival was 64% (95% confidence interval (CI): 48–79%), with a median survival of 9.75 years (median follow up 5.6 years, range 5 months to 22.5 years). Ten patients have died of disease. At 2 years the local control rate was 86%, and colostomy‐free survival was 83%. Relapse after 2 years was uncommon. Tumour size was the main factor driving outcomes, especially survival. Patients with larger tumours (T > 4 cm) had a hazard ratio for survival of 5.7 (95% CI: 1.8–17). Fourteen (24%) patients experienced treatment interruptions as a result of acute toxicity, including one death from neutropoenic sepsis. Seven (12%) patients, in total, experienced one or more late toxicities, grade 3 or above, including four women (all postmenopausal) who developed a radiation‐induced bone injury. Most patients with anal cancer can expect to retain a functional sphincter after chemoradiation/radiation. Further studies are in progress to determine the optimal chemoradiation protocol. 相似文献

We describe an unusual infant with the diagnosis of alveolar capillary dysplasia who had a relatively prolonged life without extracorporeal membrane oxygenation (ECMO). We have used this case as a springboard for a thorough review of the literature. This was a full-term female infant who presented with a picture of persistent pulmonary hypertension of the newborn. She was treated as such, with various ventilatory modes, alkalinizing agents, surfactant therapy, tolazoline, prostacyclin and nitric oxide. Because of the prolonged clinical course the possibility of alveolar capillary dysplasia was raised. The parents refused ECMO. Despite all efforts she progressively deteriorated and died at 22 days of age. Macro- and microscopic examination of the lung at autopsy were diagnostic of alveolar capillary dysplasia. A detailed review of 39 cases published in the literature with comments regarding incidence, etiology, pathophysiology, clinical picture, diagnosis and treatment is presented. 相似文献

Synergistic action of genistein and cisplatin on growth inhibition and cytotoxicity of human medulloblastoma cells 总被引:3,自引:0,他引:3
OBJECTIVE: Recent experimental data have shown that dietary soy isoflavones such as genistein can significantly suppress invasiveness and growth of a number of human malignancies. In this study we examined whether genistein, at a concentration typical of plasma levels following soy formula intake, in combination with cisplatin or vincristine exhibited an additive or synergistic inhibitory effect on the growth of medulloblastoma cells. METHODS: Three human medulloblastomas cell lines (HTB-186, CRL-8805 and MED-1) were treated with genistein at 6 microM, the maximum reported dietary plasma level in children, combined with cisplatin (0-10 microM) or vincristine (0-1 microM). Monolayer cell growth and cytotoxicity, as measured by colonigenic survival in soft agarose, were then compared in control and drug-treated cultures. Presence of apoptosis, using the DNA ladder assay and laser scanning cytometry, was investigated in all cell lines at those concentrations at which an enhancement of antiproliferative effect of cisplatin and vincristine in presence of genistein was observed. RESULTS: Genistein at 6 microM led to a 2.8-fold increase in the monolayer growth inhibitory effect of cisplatin (0.05 microM) in HTB-186 cells (p = 4.5 x 10(-4) by one-tailed t test). Genistein increased colonigenic survival inhibition of HTB-186 2.6-fold at the same cisplatin concentration (p = 1.5 x 10(-4)). Genistein caused a 1. 3-fold increase in antiproliferative effect of cisplatin (0.5 microM) in CRL-8805 cells (p = 3.1 x 10(-4)). Similarly the inhibition of colonigenic survival was enhanced 2.0-fold in CRL-8805 (p = 1.22 x 10(-5)). The addition of genistein to 0.5 microM cisplatin led to a 1.7-fold increase in monolayer growth inhibition and 2.4-fold increase in colonigenic survival inhibition of MED-1 cells (p = 8.3 x 10(-4) and p = 1.1 x 10(-4) respectively). These effects were primarily synergistic but also additive in nature. The combination of genistein and vincristine, as compared to vincristine alone, caused a minimal-to-modest increase in antiproliferative effect on medulloblastoma cells studied here. We were unable to detect apoptosis by two methodologies in any of the medulloblastoma lines when genistein was combined with cisplatin or vincristine. CONCLUSION: These results indicate that genistein at typical dietary plasma levels can significantly enhance the antiproliferative and cytotoxic action of cisplatin and, to a lesser extent, vincristine. The implication for treatment of medulloblastomas of early childhood may be a reduction in the chemotherapeutic dose recommendations of these agents and subsequently a decrease in the risk of treatment sequelae for these patients. 相似文献

This study objectively investigated the functional results of 28 index finger pollicizations for correction of congenital deficiency of the thumb. The results indicate that the average total active range of motion at the pollicized digit was 98 degrees, or approximately 50% that of a normal thumb. The average grip strength was 21% of standard values; lateral, tripod, and tip pinch strength values ranged from 22% to 26% of standard values. The pollicized digit was used in the manner of a normal thumb or in modified fashion in 84% of 14 defined activities, with increased use for handling large objects (92%) and less use for small objects (77%). The time required to perform activities averaged 22% longer than the standard for a normal hand. Patients with a radial club hand, a five-finger hand, or a mirror hand on the affected extremity had significantly poorer results; however, patients without one of these associated conditions had near normal use of the pollicized digit, except for reduced strength. These results were not influenced by the age of the patient at the time of operation. This information should be helpful in counseling parents with children who are candidates for pollicization as to what functional results they might anticipate. 相似文献
