Creation of radiofrequency lesions in a porcine model: correlation with sonography, CT, and histopathology

OBJECTIVE: We studied the correlation between sonographic and CT appearances of radiofrequency thermal lesions created in porcine liver and histopathologic findings to evaluate the accuracy of these techniques in revealing the extent of tissue necrosis. MATERIALS AND METHODS: We used sonographic guidance and a 2.0-cm-diameter, eight-prong retractable radiofrequency electrode to view 12 hepatic lesions that were created in five pigs. Biphasic helical CT was performed 12-48 hr after ablation. The animals were sacrificed immediately after CT, and their livers were histopathologically examined. The maximum lesion size in the long and short axes as measured on CT and sonography was then correlated with the histopathologically determined lesion size. RESULTS: On sonography, lesions changed rapidly within 5 min after the termination of ablation. An early echogenic cloud became peripherally hypoechoic with a variable thin echogenic rim. Early (0-2 min after ablation) sonograms led to an underestimation of true lesion sizes on histopathology (r = 0.3-0.49; p < 0.05). Delayed (2-5 min after ablation) sonograms also led to an underestimation of true lesion size (r = 0.5-0.62; p < 0.05); however, lesions were larger and better demarcated. Biphasic contrast-enhanced helical CT revealed avascular lesions surrounded by hyperemic rims that closely correlated with true pathologic lesions size (r = 0.93-0.95; p < 0. 05). Lesions with hyperemic rims that were measured on CT led to overestimations of true lesion size. CONCLUSION: Sonography led to underestimations of the true size of ablated lesions within the first 5 min after creation; however, delayed images provided better results. The avascular lesion measured on contrast-enhanced helical CT closely correlated with the size of ablated tissue; therefore, contrast-enhanced CT is preferred for serially monitoring the effect of radiofrequency ablation.     

Ocular pressure waveform reflects ventricular bigeminy and aortic insufficiency

Ocular pulse amplitude (OPA) is defined as the difference between maximum and minimum intraocular pressure (IOP) during a cardiac cycle. Average values of OPA range from 1 to 4 mmHg. The purpose of this investigation is to determine the source of an irregular IOP waveform with elevated OPA in a 48-year-old male. Ocular pressure waveforms had an unusual shape consistent with early ventricular contraction. With a normal IOP, OPA was 9 mmHg, which is extraordinarily high. The subject was examined by a cardiologist and was determined to be in ventricular bigeminy. In addition, he had bounding carotid pulses and echocardiogram confirmed aortic insufficiency. After replacement of the aortic valve, the bigeminy resolved and the ocular pulse waveform became regular in appearance with an OPA of 1.6–2.0 mmHg. The ocular pressure waveform is a direct reflection of hemodynamics. Evaluating this waveform may provide an additional opportunity for screening subjects for cardiovascular anomalies and arrhythmias.     

Stimulation of adrenergic activity by desipramine enhances hematopoietic stem and progenitor cell mobilization along with G‐CSF in multiple myeloma: A pilot study

Hematopoietic stem cell (HSC) release is positively regulated by the sympathetic nervous system through the β3 adrenergic receptor. Preclinical studies have demonstrated that the combination of desipramine and G‐CSF resulted in improved HSC mobilization. Here, we present the results of an open‐label single‐arm pilot study in patients with multiple myeloma undergoing autologous stem cell transplantation (ASCT) to assess the safety and efficacy of desipramine combined with G‐SCF to induce HSC mobilization. The primary endpoint was safety of the combination including engraftment kinetics. The secondary endpoint was the proportion of patients who collected ≥5 × 10⁶ CD34⁺ cells/kg. Outcomes were compared with historical matched controls during the same time period with multiple myeloma mobilized with G‐CSF. All study patients received desipramine 100 mg daily for 7 days, starting 4 days prior to G‐CSF administration (D‐3) and continued taking it along with G‐CSF for a total of 7 days. Six of ten patients enrolled completed the protocol with minimal side effects. All of them achieved the target collection of 5 × 10⁶ CD34 cells/kg in a median of 1.5 apheresis session with two patients needing additional plerixafor (16%), while 11 out of 13 patients (85%) achieved the target of 5 × 10⁶ CD34 cells/kg in the historical control group in a median of 2 apheresis procedures and seven patients needed plerixafor (54%). The combination of desipramine and G‐CSF is safe and signals improved mobilization over G‐CSF alone, providing a possible alternative means of mobilization that needs further investigation.     

Is blood agar an alternative to sabouraud dextrose agar for the isolation of fungi in patients with mycotic keratitis

To compare the blood agar (BA), sabouraud dextrose agar (SDA) and chocolate agar (CA) for the isolation of fungi in patients with mycotic keratitis. Corneal Scrapings of 229 patients with clinically diagnosed microbial keratitis were inoculated on BA, SDA, CA. The culture media were evaluated for the rate and time taken for the fungal growth. Seventy six of 229 patients had fungal keratitis. Fungus grew on BA in 60/76(78.9 %), on SDA in 76/76 (100 %), on CA in 40/76(52.6 %) patients. The fungi which grew on BA (60/76) also grown on SDA at the same time. The colony morphologies of different fungi were better on SDA than BA/CA. Among the different culture media, SDA is essential for the isolation fungi in patients with mycotic keratitis.
