国人心壁内血管系统的研究——左心室壁心外膜区微血管形态计量学研究

为探察人左心室壁心外膜区微血管的三维空间构型和形态计量学指标。本研究应用微血管腐蚀铸型/扫描电子显微镜技术和酶组化(碱性磷酸酶反应)染色方法,观察10例心脏标本,得出心外膜区的毛细血管直径为7.2±1.9μm(±S.D),微血管密度是3484.44/mm~2。获得左心室壁心外膜区从微动脉-动脉端毛细血管-毛细血管网-静脉端毛细血管-“萝卜根样”微静脉这个微循环单位的三维空间构型资料。     

Rat liver regeneration after partial hepatectomy: effects of insulin, glucagon and epidermal growth factor.

This study evaluated the role of insulin, glucagon and the epidermal growth factor (EGF) on liver regeneration after partial hepatectomy. Male Wistar rats, weighing approximately 200 g, were used. A partial hepatectomy, with resection of the medial and left lateral lobes (67.31%), was performed on the control group and seven hormone-treated groups: insulin, glucagon, EGF, insulin plus glucagon, insulin plus EGF, glucagon plus EGF, and a combination of the three hormones. The hormones were administered subcutaneously two days prior to the partial hepatectomy. The groups administered insulin were allowed to drink 20% glucose in water. Another group of rats received simulated operations, i.e., only a laparotomy was performed. The rats were killed at six, 24, 48 and 72 hours after the operation. Remnant liver weight, deoxyribonucleic acid (DNA) content, rate of DNA synthesis, mitotic index, blood glucose and serum insulin levels were measured. The results showed that: 1) the effects of single hormone administration on posthepatectomy liver regeneration were not obvious; 2) combined administration of insulin and glucagon increased the weight of the remnant liver, the DNA content, and the rate of DNA synthesis; 3) the combined administration of insulin, glucagon, and EGF increased the regeneration based on the remnant liver weight and mitotic index; and 4) there was no concordance between the change in blood glucose levels and the effect of hormones during liver regeneration.     

APPLICATION OF ACUPOINT-INJECTION THERAPY TO PAIN SYNDROME: CLINICAL ANALYSIS OF 250 CASES

In this paper,250 cases of headache,shoulder pain,lumbago and leg pain weretreated with 20％Angelicae Sinensis mixed with Vitamine B_(12) point-injection therapy.The total effective rate was 95.2％and in which the cure rate was 53.6％.Authors have found a close correlation between the effectiveness and the duration of disease.     

Transitional mucosa at anastomosis. A cause of local tumor recurrence in patients with rectal cancer after anterior resection.

Transitional mucosa adjacent to colorectal cancers is essentially characterized by an excess of sialomucins at the expense of the normally predominant sulphomucins in epithelial cells lining the intestinal crypts which presents the early stage of oncogenic transformation of colorectal epithelium. The presence or absence of sialomucins at the resection margins was studied histochemically using the high iron diamine-alcian blue(HID-AB) stain in 64 rectal cancer patients in Dukes' B stage who underwent curative anterior resection. The correlation was revealed between the presence of sialomucins at the resection margins and subsequent development of local tumour recurrence. Fourteen of 27 patients (51.9%) with sialomucins predominant pattern at either resection margin developed local recurrence compared with 4 of 37 patients (10.8%) with sulphomucins predominant pattern (P less than 0.001). It is suggested that determination of the transitional mucosa around anastomosis in patients treated for the rectal carcinoma by anterior resection appears to identify those with a higher risk of local recurrence.     

乳癌根治术后并发臂丛神经损伤

乳癌根治术是治疗乳腺癌的主要手段,但并发臂丛神经损伤的报道却很少,总结从1989年10月～1991年2月华山医院诊治的9例,3例门诊治疗,6例住院治疗。其中5例行神经松解后,皮瓣或肌皮瓣转移覆盖神经瘢痕区。随访到8例,疼痛改善3例,感觉和运动改善2例,疗效不理想,认为寻找预防措施是减少乳癌根治术后臂丛损伤的根本办法。     

上皮样肉瘤的病理学研究

Thirteen cases of epithelioid sarcoma were studied by light microscopy (LM), electron microscopy and immunohistochemical technique. LM revealed a nodular pattern of the tumor cells, which consisted of various degrees of a typical epithelioid cells and plump spindle-shaped cells. Irregular necrosis was seen in the centre of some nodules. Ultrastructurally the tumor cells exhibited abundant intermediate filaments, desmosome-like cell junctions and small intercellular spaces surrounded by microvilli. Immunohistochemically, there was positivity for vimentin and keratin. The author discussed diagnosis, differential diagnosis and histogenesis of epithelioid sarcoma.     

Antibodies to glutamic acid decarboxylase discriminate major types of diabetes mellitus.

Insulin-dependent diabetes mellitus (IDDM) is marked by circulating antibodies to a 64,000-M(r) islet cell antigen identified as glutamic acid decarboxylase (GAD). We describe a radioimmunoprecipitation assay with GAD isolated from pig brain. The sera tested were from 80 patients with IDDM including 26 with disease of recent onset and 54 with disease of longer duration (3-42 yr), 20 with non-insulin-dependent diabetes mellitus (NIDDM), and 55 nondiabetic subjects. Conventional assays for islet cell cytoplasmic antibodies were performed concurrently. The level of antibody in serum was expressed in units based on percentage reactivity of a standard reference serum. The frequency of antibody to GAD in IDDM was 69% in short-duration cases and 59% in long-duration cases. The latter was substantially higher than the frequency of islet cell cytoplasmic antibody. Antibodies to GAD were elevated (means +/- 3 SD) in 5% NIDDM cases and in none of the nondiabetic subjects. A simple laboratory test with a defined autoantigen has substantial implications for population screening and early diagnosis of IDDM and for better understanding of its pathogenesis.     

健康教育在青光眼患者中的应用及效果

目的探讨健康教育在青光眼患者中的应用,并评价其实施效果。方法利用个体指导、健康处方、集体授课的形式,采取讲授、示范和操作的方法,对84例青光眼患者进行疾病知识、眼压监测、饮食、生活、运动等方面的健康教育。结果实施健康教育前患者知识掌握的优良率为28.6%,而实施健康教育后为80.9%,(P<0.05);实施健康教育后学会控制不良情绪、自我监测眼压、眼球按摩、正确使用降眼压眼水的人数比以前增多(P<0.05)。结论健康教育能提高青光眼患者的疾病知识水平和自我护理水平及监查依从性。     

Randomized placebo-controlled trial comparing montelukast and cetirizine for treating perennial allergic rhinitis in children aged 2–6 yr

Leukotriene receptor antagonists (LTRAs) were recently added to the method of treating allergic rhinitis (AR). However, in children under 6 yr old, there has been no study about its efficacy in treating AR. We aim to compare the clinical efficacy of montelukast, cetirizine and placebo in the treatment of children from 2 to 6 yr old with perennial allergic rhinitis (PAR), to see if there are any significant differences. Sixty children were selected and treated with montelukast, or cetirizine, or placebo once daily. The efficacy of the three agents was compared with the Pediatric Rhinoconjunctivitis Quality of Life Questionnaire (PRQLQ) and Total Symptom Score (TSS) by diary. In addition, we also examined serum IgE, serum eosinophil cationic protein (ECP), blood eosinophil counts, nasal airway resistance (NAR) and eosinophil percentage in nasal smears. The results revealed that both montelukast and cetirizine were significantly efficacious compared with placebo in NAR, eosinophil percentage in nasal smears, PRQLQ, TSS and all symptom items except nasal itching, throat itching and tearing. For nasal itching, only cetirizine was significantly efficacious. On the other hand, for night sleep quality, montelukast was significantly superior to cetirizine.
