Introduction

This study was conducted in vivo to investigate the accuracy of Root ZX II in locating the apical foramen in teeth with apical periodontitis (AP).

Methods

Twenty-seven single-root teeth scheduled for extractions were selected; in AP group, there were 12 teeth with pulp necrosis and radiographic evidence of apical lesions, and VT group consisted of 15 vital teeth. After endodontic access, the coronal portion of the canal was flared, and electronic measurement was performed by using a 15 K-file until the device level reading “APEX” was reached. The file was fixed in place, and the tooth was extracted. The apical third of the root was shaved until exposure of the file. The distance from the file tip to the most coronal border of the apical foramen was obtained.

Results

The mean distance in AP group was +0.117 (standard deviation, 0.373) and in VT group was -0.105 (standard deviation, 0.218). The unpaired t test showed no difference between the groups when comparing the mean distances (P > .05). The apical foramen was accurately located within ±0.5 mm in 83% of the teeth in AP group and in 100% of VT group. Statistical analysis showed no difference between the groups (P = .1092).

Conclusions

The Root ZX II device was accurate in locating the apical foramen regardless of the presence of AP.  相似文献

Several materials and techniques have been proposed to improve alveolar wound healing and decrease loss of bone height and thickness that normally follow dental extraction. The objective of this research was the histologic analysis of bone morphogenetic proteins implanted into dental alveoli of rats after extraction. A total of 45 adult male Wistar rats were divided into three groups of 15 animals each: control (no treatment), implanted with pure hydroxyapatite (HA, 3 mg) and implanted with hydroxyapatite plus bone morphogenetic proteins (HA/BMPs, 3 mg). Five animals from each group were sacrificed at 7, 21 and 42 days after extraction for the histometric analyses of the osteoconductive potential of hydroxyapatite associated or not with BMPs. After dissection, fixation, decalcification and serial microtomy of 6-micron thick sections, the samples were stained with hematoxylin-eosin for histologic and histometric analyses. Both HA and HA/BMPs caused a delay in wound healing compared to control animals, evaluated by the percentage of bone tissue in the alveoli. The treatment with HA/BMPs had the greatest delay at 21 days, even though it produced values similar to the control group at 42 days. The materials did not improve alveolar repair in the normal period of wound healing and the association of HA/BMPs did not have osteoconductive properties with granulated hydroxyapatite as the vehicle.  相似文献

BackgroundChildren with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) are typically asymptomatic but contagious. The authors investigated the positivity rate of asymptomatic SARS-CoV-2 infection in pediatric dental patients.MethodsThe authors reviewed consecutive charts of children younger than 18 years scheduled for elective dental procedures from April 1, 2020, through August 1, 2020. All patients were screened for signs and symptoms of SARS-CoV-2 infection. Asymptomatic patients scheduled for dental procedures underwent polymerase chain reaction (PCR) testing for SARS-CoV-2. Sociodemographic characteristics were abstracted, and positivity rates were calculated. Variables for patients who were SARS-CoV-2 positive and SARS-CoV-2 negative were compared using Fisher exact and Mann-Whitney U tests.ResultsThe sample size was 921. The median age was 6 years, and 50.9% were boys. The overall SARS-CoV-2 positivity rate was 2.3%. Age, insurance status, medical history, and dental diagnosis were comparable in patients who were SARS-CoV-2 positive and SARS-CoV-2 negative. Positivity rates were statistically higher for Hispanic or Latinx patients than other groups (P = .038).ConclusionsAlthough the yield of testing was low, the systematic evaluation of asymptomatic pediatric dental cases via PCR resulted in the identification of SARS-CoV-2 carriers who could have been infectious. In this study, Hispanics or Latinx had a higher positivity rate than other demographic groups.Practical ImplicationsPCR testing for SARS-CoV-2 of asymptomatic patients in pediatric dentistry adds value to the use of screening questionnaires for the identification of infected people who could be contagious.  相似文献

OBJECTIVES: The present in vitro study aimed at evaluating the fracture resistance of both implant-supported screw- and cement-retained porcelain fused to metal (PFM) single crowns. A scanning electron microscope (SEM) evaluation of the mode of failure of the specimens was also performed. METHODS: Forty PFM premolar-shaped identical single crowns were realized. The restorations were divided into two groups: cement-retained (group 1) and screw-retained (group 2) prostheses. Compressive loading tests and SEM fractographic analyses were performed. The data were statistically analysed by means of the Student's t-test, with a confidence interval of 95%. RESULTS: The mean fracture load value was 1657 (+/-725) N in group 1 and 1281 (+/-747) N in group 2; the statistical analysis pointed out no significant differences between the two groups (p=0.115). The mean work at maximum load value was 0.775 (+/-0.619) J in group 1 and 0.605 (+/-0.526) J in group 2; the statistical analysis pointed out no significant differences between the two groups (p=0.355). All the samples were affected by cohesive fractures of the porcelain. Screw-retained crowns showed microcracks at the level of the occlusal access to the screw and extensive fractures in the whole thickness of the ceramics. On the contrary, cement-retained restorations were affected by less wide paramarginal fractures of the porcelain. SIGNIFICANCE: A stronger implant-prosthetic connection was noticed in cemented restorations group than in screw-retained single crowns. Even though negatively influenced by the presence of the occlusal access to the screw, the metal-ceramics bond can be considered predictable in both the implant-prosthetic connection systems analysed.  相似文献

The objective of this study was to compare the occlusal changes of Class II malocclusion treatment provided by two different types of appliances. Group 1 comprised 25 patients treated with the Fr?nkel appliance and group 2 comprised 30 patients treated with the eruption guidance appliance. Evaluations were performed on the initial and final study models of the patients using the peer assessment rating (PAR) index. The final mean PAR index, the mean PAR change, and the percent PAR reduction of each group were compared using the Student's t-test. Results demonstrated that there were no statistically significant differences between the final PAR index, the PAR changes, and the percent PAR reduction of the two groups. Therefore, it was concluded that the occlusal changes provided by the two appliances are similar, as evaluated by the PAR index.  相似文献

The decline in prevalence and incidence of dental caries in developed countries over the last two decades is considered to be due mainly to the widespread use of fluoride in different forms, but simultaneously with decline in caries, an increase in dental fluorosis has been reported. The aim of this study was to compare the Conventional Sealant Technique (CST) and Enameloplasty Sealant Technique (EST) using a flowable resin as sealant in molars affected by dental fluorosis. A total of 40 extracted third molars affected by dental fluorosis were divided at random into two groups of 20 teeth each, and Tetric Flow resin was used as sealant. All teeth were studied for lateral adaptation and resin penetration by direct and indirect techniques; all samples were replicated in epoxy resin and were evaluated with Scanning Electron Microscopy (SEM). The results demonstrated that EST allowed a deeper sealant penetration and a superior sealant adaptation than CST, both in direct and indirect evaluations by SEM. The most important variables being penetration-interface and penetration depth both being statistically significant (p<0.05). The CST did not flow into the bottom of the fissures, leaving spaces that can favor the fracture of the material and initiate the process of dental caries. We conclude that a flowable ceromer is an excellent material alternative to be used as sealant and that EST is quite necessary in molars affected by dental fluorosis, the combination of both being a reliable method to be used as primary prevention approach of dental caries in endemic areas of dental fluorosis.  相似文献

OBJECTIVE: To investigate the fracture strength and pattern of failure of teeth with weakened roots reconstructed by different procedures. METHODS: In an in vitro study root posts were placed in 50 endodontically treated canines, divided into 5 groups (n=10) as follows: cast metallic post; glass fibre post with smaller diameter than the root canal; glass fibre post with smaller diameter than the root canal + glass fibre strips; glass fibre post with smaller diameter than the root canal + accessory glass fibre posts; anatomical post (glass fibre post with smaller diameter than the root canal, relined with low viscosity composite resin). Posts were luted with resin cement and the coronal portion of posts was constructed with composite resin. Metallic crowns were cemented on the posts. Specimens were submitted to compressive load in a universal testing machine. Fracture strength values of each group were compared. RESULTS: Fracture strength values were for Groups 1-5 respectively: 1087.06; 745.69; 775.41; 920.64; 876.12kgf, with significant differences between Groups 1 and 2 and between Groups 1 and 3 (p<0.05). Observed patterns of fracture were: Group 1 - 100% of roots fractured; Groups 2 and 4 - variable fracture modes; Group 3 - 60% of fractures occurred in the cervical root third; Group 5 - 50% of failures occurred in the coronal portion of the post. CONCLUSIONS: The fracture strength of teeth with cast metallic posts, teeth with anatomical posts or teeth with glass fibre posts combined with accessory posts was similar. All teeth restored with cast metallic posts presented fractures and were unfavourable to maintenance of the remaining tooth structure. Teeth with fibre posts (Groups 2 to 5) presented variable fracture modes; however, the maximum percentage of unfavourable fractures was 30%.  相似文献

OBJECTIVE: The present study aimed at evaluating different restoring configurations of a crownless maxillary central incisor, in order to compare the biomechanical behavior of the restored tooth with that of a sound tooth. MATERIALS AND METHODS: A 3D FE model of a maxillary central incisor is presented. An arbitrary static force of 10 N was applied with an angulation of 125 degrees to the tooth longitudinal axis at level of the palatal surface of the crown. Different material configurations were tested: composite, syntered alumina, feldspathic ceramic endocrowns and glass post resorations with syntered alumina and feldspathic ceramic crown. RESULTS: High modulus materials used for the restoration strongly alter the natural biomechanical behavior of the tooth. Critical areas of high stress concentration are the restoration-cement-dentin interface both in the root canal and on the buccal and lingual aspects of the tooth-restoration interface. Materials with mechanical properties underposable to that of dentin or enamel improve the biomechanical behavior of the restored tooth reducing the areas of high stress concentration. SIGNIFICANCE: The use of endocrown restorations present the advantage of reducing the interfaces of the restorative system. The choice of the restorative materials should be carefully evaluated. Materials with mechanical properties similar to those of sound teeth improve the reliability of the restoartive system.  相似文献

Abstract  – This report presents a case of replantation of a traumatically avulsed central incisor in a 32-year-old woman. The tooth was replanted after a 30-min extra-alveolar period. Emergency unconventional immobilization was performed, using 2–0 chromic gut, and a restorative composite made Gunning-type splint. The tooth is still in place 27 years after replantation (1976 to date) without marked resorption.  相似文献

This study evaluated the root-end sealing ability of mineral trioxide aggregate (MTA Angelus), reinforced zinc oxide-eugenol cement (Super EBA), resin-modified glass ionomer (Vitremer) and zinc-free amalgam (GS-80) (control). The root canals of eighty human lower molars were accessed, cleansed, shaped and obturated. Apexes were resected and cavities were prepared. Teeth were divided into 4 groups of 40 cavities, root-end filled with the materials, and immersed in methylene blue for 72 h at 37 degrees C. Roots were then sectioned transversally at each millimeter and evaluated under magnification, observing the dye penetration in each section. Data were evaluated using Kruskal-Wallis test at a 5% level of significance, showing the differences among all materials (p < 0.001). The crescent order of microleakage was MTA < Vitremer < Super EBA < amalgam. Higher leakage levels were observed in the first millimeter sections of amalgam, Vitremer and MTA, when compared with the third millimeter section (p < 0.05).  相似文献
