Objective
To detect if strain ultrasound elastography and strain ratio have additional value to the conventional grey scale ultrasound in predicting thyroid malignancy.Patients and methods
This study included 92 thyroid nodules from 62 patients (the mean age was 40.64?±?13.93). Morphologic aspects of the thyroid nodule in conventional grey scale ultrasonography and elastographic examinations with elastography score and strain ratio (SR) were performed for all nodules. The final diagnosis was confirmed by fine needle aspiration biopsies in 72 nodules and by excisional biopsies in 20 nodules.Results
We found that combination of both conventional ultrasound and strain elastography score have the best diagnostic performance with sensitivity, specificity, PPV, NPV and accuracy accounting for 80%, 97%, 57%, 99% and 96% respectively. The means SR for benign nodules (1.37?±?0.56) was significantly lower than that for malignant nodules (3.0?±?0.71) [p-value .003].The optimal SR cutoff is 2.5 with estimated 80% sensitivity, 98% specificity, PPV 67%, NPV 99% and accuracy 97%.Conclusion
The clinical application of elastography score and SR should be carried out hand in hand with conventional sonographic assessment of thyroid nodules to achieve the best diagnostic performance. 相似文献992.

Purpose
To demonstrate role of PET-CT (positron emission tomography-computed tomography) in screening for local and distant metastatic deposits from primary renal cell carcinoma and enhancing its advantages over other imaging modalities.Materials and methods
Twenty patients were evaluated as post nephrectomy screening for renal malignancy. Positive cases were interpreted whenever a focal area of FDG uptake more than the surrounding tissue whether in or outside abdominal cavity. Final diagnosis after correlation with other conventional radiological modalities as CT, MRI with the gold standard was histopathological diagnosis.Results
In current study, there were different sites of distant metastasis of renal cell carcinoma as follow: lung was the commonest (8 patients = 40%), followed with bones (7 patients = 35%), lymph nodes (6 patients = 30%), liver (4 patients = 20%), suprarenal gland (4 patients = 20%) and last were peritoneal deposits (2 patients = 10%).Conclusion
PET-CT had limitations in evaluating RCC metastasis due to its inability for adequate characterization of small metastatic lesions, however positive PET-CT results as a good predictive for evaluating metastatic deposits. Combined antiretroviral therapy (cART) increased the life expectancy of people living with Human Immunodeficiency Virus (HIV) (PLHIV) and remarkably reduced the morbidity and mortality associated with HIV infection. Consequently, PLHIV are experiencing non-acquired immunodeficiency syndrome (AIDS) associated comorbid conditions including diabetes, hyperlipidemia, hypertension, and cardiovascular disease. The aim of this study is to determine the frequency of non-AIDS associated comorbid conditions among a cohort of PLHIV on cART in Lebanon.Data were collected between November 2018 and December 2019 from 105 voluntary participants. A standardized questionnaire was used to collect demographic and behavioral data including lifestyle, smoking, physical activity, substance use and abuse in addition to co-infections and family history of non-communicable diseases. Moreover, data on occurrence and treatment of cardiovascular disease, hypertension, diabetes, lipid and metabolic disorders as well as mental health were collected. Blood samples were used to assess the levels of fasting blood sugar (FBS), glycosylated hemoglobin (HbA1C), triglycerides (TG), low-density lipoprotein (LDL), high-density lipoprotein, total cholesterol, and serum creatinine.Hypertension (29.5%) and hyperlipidemia (29.5%) followed by diabetes (23.7%) and cardiovascular disease (9.7%) were mainly reported among study participants. Higher rate of comorbid conditions was observed among participants >40 years of age than those ≤40 years with both hypertension and hyperlipidemia most commonly reported. Older age (odds ratio [OR] 7.6; 95% CI: 1.83-31.98; P = .005) is associated with higher odds of having hyperlipidemia. Moreover, participants on cART for ≥10 years are 5 times more likely to have hyperlipidemia (OR 5; 95% CI: 1.08-22.73; P = .039). Our results also showed that study participants did not experience anxiety, depression or somatic symptoms and that there was no association between these mental disorders and older age or comorbidities.Our results provide important information on HIV trends and associated comorbidities in Lebanon and can be used to improve the management of non-communicable diseases among PLHIV. 相似文献

Purpose
To evaluate hypotheses which explain phenotypic variability in sex determining region Y positive 46,XX males. We investigate two 46,XX males without gonadal ambiguities.Methods
Cytogenetic and molecular analyses were used to identify the presence of Y chromosome material and to map the translocation breakpoint. Finally, the pattern of X chromosome inactivation was studied using the methylation assay at the androgen receptor locus.Results
The presence of Y chromosome material, including the sex determining region Y gene, was demonstrated in both men. However, the amount of translocated Y chromosome material differed between the patients. Different X chromosome inactivation patterns were found in the patients; random in one patient and non-random in the other.Conclusions
We found a lack of association between phenotype and X chromosome inactivation pattern. Our cytogenetic and molecular analyses show support for the position effect hypothesis explaining the phenotypic variability in XX males. 相似文献998.

The seeds of Peganum harmala L. (Pgh) (Zygophyllaceae) have been used in Moroccan traditional medicine for treatment of a various diseases and to relieve dolorous process. The major objective of this paper was to investigate the mechanism of the analgesia induced by alkaloid extract of Peganum harmala. In the present work, the antinociceptive action was assayed in several experimental models in mice: writhing, formalin, and hot plate tests. The alkaloid extract (12.5 and 25mg/kg) and in a dose-dependent manner significantly reduced the nociception by acetic acid intraperitoneal injection (p<0.001). In the formalin test, the extract also significantly reduced the painful stimulus in both phases of the test (p<0.001). Treatment with the extract when given by (i.p. or i.c.v.) or with morphine (10mg/kg, i.p.) produced a significant increase of the reaction time in hot plate test. These result showed that the alkaloid extract of Pgh contains active analgesic principles acting both centrally and peripherally. Furthermore, this antinociceptive effect has been avoided by naloxone at a dose of 1mg/kg in the first phase of formalin and hot plate tests indicating that this extract act partly through an opioid-mediated mechanism. In conclusion, the alkaloid extract of Peganum harmala seems to have both central and peripheral antinociceptive activities which may be mediated by opioid receptors. 相似文献

Research has focused on creating new methodologies such as supervised machine learning algorithms that can easily calculate the mechanical properties of fiber-reinforced concrete. This research aims to forecast the flexural strength (FS) of steel fiber-reinforced concrete (SFRC) using computational approaches essential for quick and cost-effective analysis. For this purpose, the SFRC flexural data were collected from literature reviews to create a database. Three ensembled models, i.e., Gradient Boosting (GB), Random Forest (RF), and Extreme Gradient Boosting (XGB) of machine learning techniques, were considered to predict the 28-day flexural strength of steel fiber-reinforced concrete. The efficiency of each method was assessed using the coefficient of determination (R2), statistical evaluation, and k-fold cross-validation. A sensitivity approach was also used to analyze the impact of factors on predicting results. The analysis showed that the GB and RF models performed well, and the XGB approach was in the acceptable range. Gradient Boosting showed the highest precision with an R2 of 0.96, compared to Random Forest (RF) and Extreme Gradient Boosting (XGB), which had R2 values of 0.94 and 0.86, respectively. Moreover, statistical and k-fold cross-validation studies confirmed that Gradient Boosting was the best performer, followed by Random Forest (RF), based on reduced error levels. The Extreme Gradient Boosting model performance was satisfactory. These ensemble machine learning algorithms can benefit the construction sector by providing fast and better analysis of material properties, especially for fiber-reinforced concrete. 相似文献

Non-Hodgkin's lymphoma (NHL) is the most common malignancy referred to our institute which is the largest tertiary referral cancer centre in Saudi Arabia, The proportion of follicular low grade NHL appears to be extremely small in this population (<5% of all NHL). To date, there is no data available regarding any correlation between bcl-2 gene rearrangement and different cell types of nodal and extranodal NHLs in Saudi patients. We used a sequential polymerase chain reaction (PCR) technique to determine the frequency of bcl-2/J(H) recombination occurring via the major breakpoint region (mbr) in 16 GI tract NHLs including 4 MALT lymphomas and 13 follicular (nodal) NHLs. The results showed only 2/13 (15%) nodal follicular NHLs with bcl-2/J(H) fusion DNA whereas 9/16 (56%) of the extranodal NHLs with at least 2 of them exhibiting MALT characteristics were positive for the bcl-2 gene rearrangement. A breakdown of the proportion of extranodal NHLs of different cell types showing bcl-2 rearrangement via mbr was as follows: 5/8 diffuse large non-cleaved cell (DLNCC), 1/3 diffuse small cleaved cell (DSCC), 1/1 follicular small cleaved cell (FSCC) and 2/4 MALTs. The PCR amplified bcl-2/J(H) fusion DNA from 5 randomly selected tumors (2 MALTs, 1 DLNCC, 1 DSCC and 1 nodal follicular lymphoma) were cloned and sequenced. All 5 of them showed different bcl-2/J(H) N-regions confirming the clonality of each tumor sample. The data indicating a very low incidence of bcl-2 translocation in nodal follicular NHLs and a surprisingly high incidence of it in extranodal NHLs are intriguing, and quite contrary to the findings in Western patients. These unusual observations warrant further studies and may suggest that different genetic events are involved in the development of extranodal NHLs including MALT and follicular center-cell NHLs in Saudi patients. 相似文献
