Infective endocarditis (IE) caused by methicillin-resistant Staphylococcus aureus (MRSA) with reduced susceptibility to vancomycin and daptomycin has few adequate therapeutic options. Ceftaroline (CPT) is bactericidal against daptomycin (DAP)-nonsusceptible (DNS) and vancomycin-intermediate MRSA, but supporting data are limited for IE. This study evaluated the activities of ceftaroline, vancomycin, daptomycin, and the combination of ceftaroline plus daptomycin against DNS MRSA in a pharmacokinetic/pharmacodynamic (PK/PD) model of simulated endocardial vegetations (SEVs). Simulations of ceftaroline-fosamil (600 mg) every 8 h (q8h) (maximum concentration of drug in serum [Cmax], 21.3 mg/liter; half-life [t1/2], 2.66 h), daptomycin (10 mg/kg of body weight/day) (Cmax, 129.7 mg/liter; t1/2, 8 h), vancomycin (1 g) q8h (minimum concentration of drug in serum [Cmin], 20 mg/liter; t1/2, 5 h), and ceftaroline plus daptomycin were evaluated against 3 clinical DNS, vancomycin-intermediate MRSA in a two-compartment, in vitro, PK/PD SEV model over 96 h with a starting inoculum of ∼8 log10 CFU/g. Bactericidal activity was defined as a ≥3-log10 CFU/g reduction from the starting inoculum. Therapeutic enhancement of combinations was defined as ≥2-log10 CFU/g reduction over the most active agent alone. MIC values for daptomycin, vancomycin, and ceftaroline were 4 mg/liter, 4 to 8 mg/liter, and 0.5 to 1 mg/liter, respectively, for all strains. At simulated exposures, vancomycin was bacteriostatic, but daptomycin and ceftaroline were bactericidal. By 96 h, ceftaroline monotherapy offered significantly improved killing compared to other agents against one strain. The combination of DAP plus CPT demonstrated therapeutic enhancement, resulting in significantly improved killing versus either agent alone against 2/3 (67%) strains. CPT demonstrated bactericidal activity against DNS, vancomycin-intermediate MRSA at high bacterial densities. Ceftaroline plus daptomycin may offer more rapid and sustained activity against some MRSA in the setting of high-inoculum infections like IE and should also be considered.  相似文献

Stereotyped behaviors (e.g., body rocking) occur at high rates in individuals with mental retardation (e.g., Down syndrome). To determine if spontaneous stereotypy occurs in a murine model of Down syndrome, the home cage behavior of Ts65Dn and control mice was monitored during the dark cycle. Motor activity was further assessed in novel automated test chambers, with acoustic startle and rotor rod paradigms providing additional environmental challenges. Spontaneous stereotypy (repetitive jumping and cage top twirling) was observed in the home cage in approximately half of the Ts65Dn mice, compared with approximately 10% of diploid controls. Repetitive jumping was observed exclusively in the Ts65Dn mice. In the open field, although no differences were found between Ts65Dn and control mice, stereotypic Ts65Dn mice exhibited significantly less locomotor activity and rearing relative to control and nonstereotypic Ts65Dn mice. Ts65Dn mice attained significantly lower rotor rod speeds but did not differ from controls in the amplitude of the acoustic startle response. These environmental challenges did not increase stereotypy over home cage rates but induced stereotypy in two additional animals. The Ts65Dn model may aid in identifying genes associated with the development and expression of stereotypy.  相似文献

Angioplasty with stent placement is commonly used to treat coronary atherosclerosis. However, 20-40% of stainless steel stents restenose within 6 months via a prolonged inflammatory response mediated by monocytic infiltration and cytokine secretion. In the current study, we tested a hypothesis that blood flow and monocytes interact to alter stent corrosion. We assessed the effects of THP1 monocytes on the corrosion rate of 316L stainless steel (316LSS) under shear stress (0.5-50 dyn/cm(2)). In addition, THP1 cytokine secretion was determined using cytokine arrays and ELISA analyses. Data were compared using ANOVA and Tukey post hoc analysis (alpha = 0.05). Monocytes significantly lowered 316LSS corrosion rates without limiting current density. However, shear stress alone did not alter the corrosion rate of 316LSS. THP1 cells adhered to the 316LSS surface at all flow rates. Exposure to the 316LSS/corrosion test under high fluid flow rates increased (>twofold) the secretion of 7 of the 42 cytokines tested (angeogenin, GRO, I309, interleukin 8, interleukin 6, interleukin 1beta, and macrophage chemoattractant protein-1). Each of these cytokines play a role in wound healing, macrophage differentiation, and cell proliferation, all hallmarks of in-stent restenosis. Furthermore, only IL8 levels were significantly higher than any of the system controls during the 316LSS/corrosion test conditions. The IL8 levels from the 316LSS/corrosion tests were not significantly different from the +LPS control. Together, these data suggest that monocytic cells maybe activated by exposure to 316LSS stents and could contribute to in-stent restenosis and altered corrosion of the stent.  相似文献

Eating disorder prevention programs have yielded mixed results and are somewhat controversial, primarily because of claims they may produce iatrogenic effects. We used meta-analysis to evaluate the effectiveness of eating disorder prevention programs and investigate moderators of intervention effects. Overall, prevention programs had large effects on improving knowledge and small net effects on reducing maladaptive eating attitudes and behaviors. Studies targeting participants at a relatively higher risk for developing an eating disorder produced greater benefits. Concerns about iatrogenic effects of including psychoeducational material on eating disorders were not supported by the data. These findings challenge conclusions drawn in previous review articles regarding the ineffectiveness of prevention programs and support the ability of eating disorder prevention programs to demonstrate behavioral improvements.  相似文献

Herein we describe how the rotating disk electrode voltammetric technique can be used to examine the mechanism(s) of the inward transport of dopamine by the neuronal transporter for dopamine (DAT). The usefulness of making measurements kinetically resolving dopamine transport, interpretations of changes in Km and Vmax, approaches to defining pre-steady-state binding of dopamine to DAT, interactions between competing inhibitors, chemical modification of functional groups within DAT, and a presentation of a hypothetical multi-state model of dopamine transport are presented and discussed.  相似文献
