Echocardiographic Guidance for Transcatheter Coil Embolization of Congenital Coronary Arterial Fistulas in Children

Congenital coronary arterial fistulas are rare anomalies that have traditionally been managed by surgical ligation. However, in recent years endovascular therapy has been employed with encouraging results. Between 1993 and 1996, we performed transcatheter coil embolization of coronary arterial fistulas to the right atrium or ventricle in four children ranging in age from 4.5 to 9.8 years. Cardiac and coronary arterial anatomy were diagnosed correctly on the preoperative echocardiogram in all patients, including the origin, course, and termination of the fistulas. The fistula was occluded completely in three of the patients, whereas trivial residual flow remained in the fourth. Transesophageal echocardiography was useful for monitoring the embolization procedure. In one of the patients, the fistula reopened while the child was on overnight heparin, although the magnitude of flow was less than that before the embolization. At follow-up ranging from 10 to 43 months, there was no flow through the fistula in any patient. We present our experience with these patients, with a focus on the importance of echocardiographic evaluation before, during, and after transcatheter therapy of coronary arterial fistulas.     

Current status of paediatric heart, lung, and heart-lung transplantation.

Since the first successful heart transplantation by Christian Barnard in 1967, there have been over 60 000 heart transplants performed worldwide. Around 350 paediatric heart transplants are now performed annually and approximately 10% of these are in the UK.     

Viscous corneal protection by sodium hyaluronate, chondroitin sulfate, and methylcellulose

The authors' study of the viscosities of various concentrations of sodium hyaluronate, chondroitin sulfate, and methylcellulose revealed that sodium hyaluronate and methylcellulose are pseudoplastic fluids in contrast to chondroitin sulfate, which is a Newtonian fluid. Pseudoplastic fluids are ideal for maintaining the anterior chamber, since they are more viscous at rest. Intermediate viscosity preparations of these three agents used as a thin endothelial coating gave excellent protection from intraocular lens abrasion. A highly viscous agent, eg, sodium hyaluronate 1%, in a thin layer produced extensive endothelial cell damage because it transmitted excessive shear force to the endothelium. A highly viscous agent, sodium hyaluronate 1% in a thick layer produced a physical barrier to compression with little endothelial damage. A low-viscosity agent, balanced salt solution provided insufficient protection against intraocular lens abrasion.     

Rheumatoid arthritis in the Pima Indians of Arizona: an assessment of the clinical components of the New York criteria.

When a prevalence study of Rheumatoid Arthritis (RA) was made in the adult Pima Indian population living on the Gila River Reservation, a high prevalence was found using the New York criteria (5-9 per cent). This was mainly due to the high frequency of limitation of motion which brought in many undesirable subjects. After excluding it as a component of New York criteria we found a prevalence for RA of 3 per cent with a predominance among the females (3-8 as against 2-0 per cent in males). The group so defined fulfilled the requirements of the Rome criteria, showed a higher concordance with serological or radiological evidence of RA, and appeared to identify subjects in whom the experienced clinical rheumatologist would more often agree with the diagnosis.     

Clinical Studies of Cefazolin and Comparison with Other Cephalosporins

Cefazolin, a new cephalosporin derivative, was studied in the treatment of 105 hospitalized patients with a variety of infections including endocarditis, pneumonia, and urinary and soft tissue infections, and was found to be effective in 104 patients. Cefazolin was also tested in vitro and shown to be effective against staphylococci, pneumococci, Escherichia coli, Klebsiella sp., and Proteus mirabilis by agar dilution method. It was shown to produce high serum levels when administered in a 250- to 1,000-mg intramuscular dose and was well tolerated and free from renal toxicity. Comparison of the results of this study with those from our prior studies on cephaloridine revealed equivalent antibiotic potency, good tolerance to both the agents when given intramuscularly, superior, average blood levels with cefazolin, equal clinical efficacy, and absence of renal toxicity with cefazolin (unlike cephaloridine). Similarly, the results of treatment of pneumococcal pneumonia with intramuscular cefazolin were found to be superior to those for oral cephalexin.
