Using evidence to inform clinical practice shortly after traumatic events

Despite ongoing gaps in our knowledge, there is now considerable research evidence available to inform clinical decisions and treatment plans shortly after traumatic events. The author uses two fictional cases to illustrate an evidence‐based approach to clinical management following traumatic events. The importance of fully assessing individuals and tailoring management plans to address their specific needs optimally is highlighted by considering two distinct, but familiar types of presentation.     

Rectus femoris sheath: a new localization of hip synovial cyst

Iliopsoas bursitis is the classical manifestation of synovial cysts of the hip. We report the first observation of a new localization of these cysts: the sheath of rectus femoris muscle. The patient presented with a hip osteoarthritis and an important mass of the front of the thigh. Computed tomography was useful in establishing the diagnosis and hip arthrography confirmed the communication between the joint and the rectus femoris sheath.     

Disseminating early interventions following trauma

Dissemination of effective early interventions following trauma is necessary to ensure the provision of the best possible care in a timely manner. To achieve this, agreement from all key stakeholders is required regarding the messages to be disseminated and the means of dissemination. This article is based on a National Institute of Mental Health sponsored symposium on the dissemination of early interventions at the 21st annual meeting of the International Society for Traumatic Stress Studies in Toronto in 2005. The current knowledge base regarding the effectiveness and dissemination of early interventions (defined here as those that are begun within the first 3 months after exposure) following trauma for children, adolescents, and adults is considered.     

Male sterility associated with familial translocation heterozygosity: t(8;15) (q22;p11).

During the investigation of the family of a subject consulting for primary sterility, the same oligoteratospermia was found in two of his brothers. The three karyotypes of these subjects exhibited an equilibrated reciprocal autosomal translocation t(8;15) (q22;p11), which was also detected in their mother. The karyotypes of the remaining siblings, one brother and one sister, were normal. The semen analysis of the sterile subjects suggests that the block of gamete production occurs at the beginning of spermiogenesis. The chromosomal anomaly, which has no effect on the reproduction of the mother, leads to sterility of the male offspring bearing it.     

Néphropathies glomérulaires au cours de la polyarthrite rhumatoïde non traitée Revue de la littérature à propos de deux cas

Two cases of classical rheumatoid arthritis complicated by the development of a glomerulopaty independently of drug treatment are presented. Renal biopsy showed membranous glomerulonephritis in one case and fusion of foot processes in the second. The literature is reviewed and the possible relationships between rheumatoid arthritis and these glomerulopathies are discussed. The authors conclude to the absence of causal relationship between these two diseases.
