Subjective leisure experiences of older Australians

Background:  Previous studies into leisure have employed methodologies that may understate the significance of experiential components. This exploratory study investigated the leisure experiences of retired Australians over 65 years of age.
Methods:  Five semistructured interviews were used to explore the leisure experience of older people from metropolitan Adelaide. Data were coded and analysed thematically.
Results:  Relaxation and engrossment emerged as commonly expressed experiences, yet were found to emerge as a result of engagement in occupations predefined as leisure. Experiences of freedom from both necessary duties and a sense of obligation were discussed as potential determinants of leisure consciousness.
Practice implications:  This study highlights a need for therapists to actively access clients' subjective leisure experiences to enable engagement in personally meaningful leisure occupations.     

Diagnosis of ligament rupture of the ankle joint: Physical examination, arthrography, stress radiography and sonography compared in 160 patients after inversion trauma

We prospectively enrolled 160 consecutive patients with inversion trauma of the ankle in a diagnostic protocol that included physical examination within 2 days and at 5 days after trauma, arthrography, stress radiography, and ultrasonography. 135 patients had pathological lateral ligament laxity on the later physical examination or lateral ligament rupture diagnosed on arthrography and they were operated on. 122 of these patients had ligament ruptures.

At clinical follow-up after a minimum of half a year, all of the patients who were not operated on had stable joints without signs of previous ligament ruptures.

Delayed physical examination at 5 days after the injury led to the highest overall sensitivity (96%) and specificity (84%) for the detection of a ligament rupture. Additional diagnostic procedures, at a considerable cost, yielded little additional information.     

The etiology, treatment, and prevention of nonorganic failure-to-thrive in infants

Nonorganic failure-to-thrive is a medical-psychological disorder reflecting lack of growth in an infant without apparent physical causes. Children who fail to thrive as infants are at high risk for developmental delays, personality problems, abuse, and death. This article focuses on environmental failure-to-thrive, describing the behavioral characteristics of the nonthriving infant and the family milieu. Aspects of early environments of NOFT infants are profiled, specific intervention strategies are discussed, and recommendations regarding the promotion of intense, consistent multi-disciplinary intervention strategies are advanced.

Xenotransplantation is being developed in the hope of resolvingthe critical shortage of donor organs for transplantation. TheEurotransplant waiting lists [1] for donor organs of variouskinds number almost 16 000 patients and the US lists [2] morethan 90 000 patients. Renal transplantation, for instance, cost-effectivelyconfers a significant survival advantage [3] and improvementof quality of life [4]. But whereas currently, in Europe, nearly12 000 end-stage renal disease patients await a suitable donor,only 3383 kidney transplants were performed in 2005, with anaverage waiting time of 1174 days [1]. Substantial researchefforts are being made in the field of xenotransplantation,and the immunological barriers are gradually being elucidated.Pig-to-human xenogeneic     

推动医疗改革应制定分区医疗系统战略

医疗改革是一个世界性难题，各国在医疗改革中都会遇到各自相关的问题。改革艰巨，但并不是不可能完成，本文作者认为，通过关注以分区的方式提供医疗服务可以让医疗改革取得实效。本文从划定分区边界的方式出发，介绍了制定分区医疗战略的五个步骤，并讨论了各医疗系统成功实施各自战略所需采取的措施，这也可以为我国正在推行的医疗改革提供一定的借鉴和参考。
