Increasingly, the medical community is recognizing how oral health relates to overall health and engaging in oral health promotion. In 2012, the Massachusetts Medical Society set out to explore how to prioritize oral health as a part of its public health agenda. It began with an Oral Health Forum, which evolved into a Committee on Oral Health. This committee is the first of its kind in a state medical society and is comprised of a diverse group of medical and dental professionals. In this article, we review the process by which the committee was established, report on projects, and reflect on lessons learned.  相似文献

Purpose

PET image resolution is variable across the measured field-of-view and described by the point spread function (PSF). When accounting for the PSF during PET image reconstruction image resolution is improved and partial volume effects are reduced. Here, we evaluate the effect of PSF-based reconstruction on lesion quantification in routine clinical whole-body (WB) PET/CT imaging.

Materials and methods

41 oncology patients were referred for a WB-PET/CT examination (Biograph 40 TruePoint). Emission data were acquired at 2.5 min/bed at 1 h pi of 400 MBq [18F]-FDG. Attenuation-corrected PET images were reconstructed on 336 × 336-matrices using: (R1) standard AW-OSEM (4 iter, 8 subsets, 4 mm Gaussian) and (R2) AW-OSEM with PSF (3 iter, 21 subsets, 2 mm). Blinded and randomised reading of R1- and R2-PET images was performed. Individual lesions were located and counted independently on both sets of images. The relative change in PET quantification (SUVmax, SUVmean, volume) of lesions seen on R1 and R2 is reported as (R2 − R1)/R1. Furthermore, SUVmax and SUVmean was measured for a 3 cm spherical norm region in the right lobe of the healthy liver for R1 and R2.

Results

Clinical reading revealed 91 and 103 positive lesions for R1 and R2, respectively. For all lesions SUVmax (R2) was higher than SUVmax (R1). Regression analysis indicated that the relative increase in SUVmax (and SUVmean) decreased with lesion size, whilst it increased with increasing radial distance from the centre of the field of view (FOV). There was no significant difference in SUVmean in homogenous liver tissue between R1 and R2.

Conclusion

In whole-body FDG-PET/CT using routine clinical protocols, PSF-based PET reconstruction increases lesion detection and affects SUVmax measurements compared to standard AW-OSEM PET reconstruction.  相似文献

BackgroundCoronary computed tomography (CT) angiography can be associated with high radiation exposure. Reduction of tube voltage from 120 kV to 100 kV can reduce the dose by up to 40%, but it also increases image noise.ObjectiveWe aimed to find a patient-specific predictor of image noise to determine the use of reduced tube voltage.MethodsContrast-enhanced coronary dual-source CT angiography data sets [prospectively electrocardiogram (ECG)–triggered axial and retrospectively ECG-gated spiral acquisition, rotation of 280 milliseconds, 2 × 128 × 0.6 mm collimation, 100 kV, 320 mAs] of 165 patients (age, 54 ± 13 years) for the detection of coronary artery stenoses were analyzed. Image noise was measured in the aortic root. Influence of body weight, height, body mass index, thoracic cross sectional area, as well as the area of the thoracic solid tissue were analyzed.ResultsMean image noise in the aorta was 35.1 ± 8.9 HU. Mean dose length product was 207 ± 184 cm · cGy with an average effective dose of 2.9 ± 2.6 mSv. The patient cohort was divided into tertiles according to image noise. Numerous parameters, including BMI and body weight, were significantly different between the highest and lowest tertiles. In multivariable regression analysis, the area of the thoracic solid tissue was the only independent predictor of image noise (P < 0.0001).ConclusionsThe area of the thoracic solid tissue at the level of the aortic root predicts image noise and may hence be used for the decision to reduce tube voltage from 120 kV to 100 kV.  相似文献

Six patients suffering from major depression were treated with electroacupuncture. During 4 weeks of treatment, the total CPRS-S-A score decreased from 23.8 to 13.4 (p=0.0095). A decrease of neuropeptide Y (NPY) in plasma during the first 2 weeks of treatment was noted in five of the patients, all being women (p=0.0431). The decrease was negatively correlated with age (rs=-0.29; p=0.046). The results are in line with a putative antidepressive effect of electroacupuncture, along with an influence on NPY in plasma.  相似文献

BACKGROUND: In atopic eczema (AE), skin colonization with Staphylococcus aureus is known to play a major triggering and possibly pathophysiological role. METHODS: In this open-labeled controlled side-to-side comparative trial, affected sites (flexures of both elbows) in 15 patients diagnosed as having generalized or localized AE were evaluated regarding S. aureus colonization and clinical severity of AE over a 2-week period. Flexures of the elbows were covered with silver-coated textiles on one arm and cotton on the other for 7 days followed by a 7-day control period. RESULTS: A highly significant decrease in S. aureus colonization could be seen on the site covered by the silver-coated textile already 2 days after initiation lasting until the end of the treatment. Seven days after cessation, S. aureus density remained significantly lower compared to baseline. In addition, significantly lower numbers of S. aureus were observed on the silver-coated textile site in comparison to cotton at the end of treatment as well as at the time point of control. Clinical improvement correlated with the reduction of S. aureus colonization. CONCLUSION: A superior improvement achieved by silver-coated compared to cotton textiles paralleled a potent anti-S.-aureus effect.  相似文献

Detection of Epstein-Barr virus (EBV) DNA in the cerebrospinal fluid (CSF) is associated with acquired immunodeficiency syndrome (AIDS)-related brain lymphoma. Real-time polymerase chain reaction (PCR) was performed to quantify EBV DNA in CSF and plasma from 42 patients with AIDS-related non-Hodgkin's lymphoma (NHL). Twenty patients had primary central nervous system lymphoma (PCNSL) and 22 systemic NHL, including 12 with central nervous system involvement (CNS-NHL). As controls, 16 HIV-infected patients with other CNS disorders were examined. EBV DNA was detected in the CSF from 16/20 (80%) patients with PCNSL, 7/22 (32%) with systemic NHL, 8/12 (67%) with CNS-NHL, and 2/16 (13%) of the controls. The viral EBV DNA levels were significantly higher in the CSF from patients with PCNSL or CNS-NHL compared to patients with systemic NHL or controls. EBV DNA was detected in plasma from 5/16 (31%) patients with PCNSL, 9/16 (56%) with systemic NHL, 4/9 (44%) with CNS-NHL, and 4/15 (27%) controls. No difference in plasma viral load was found between patient groups. From the patients with CNS-NHL, plasma samples drawn prior to CNS involvement contained significantly higher EBV DNA levels than those from systemic NHL patients without subsequent CNS involvement. EBV DNA levels in the CSF, but not in plasma, from patients treated with antiherpes drugs were significantly lower than in untreated patients. High CSF EBV DNA levels were found in HIV-associated brain lymphomas and the viral load can be clinically useful. High plasma EBV DNA levels might predict CNS involvement in systemic NHL.  相似文献
