STUDY DESIGN: Biopsies were obtained from within radiographically successful human intervertebral body fusion cages to document the histology of remodeling bone graft. OBJECTIVES: The purpose of this study is to describe the tissue within successful human interbody cages with special reference to the viability of bone and the presence or absence of debris particles. SUMMARY OF BACKGROUND DATA: The use of interbody fusion cages is gaining rapid acceptance, but there is little histologic documentation of the nature of tissue within successful human interbody fusion cages. METHODS: Needle biopsies were obtained of tissue within radiographically successful intervertebral body fusion cages at the time of pedicle screw removal for back pain or fusion of adjacent spinal level in nine spinal levels of eight patients. Preoperative diagnoses of these eight adult patients included disease conditions in the sagittal plane: spondylosis (5), degenerative disc disease (6), failed laminectomy and discectomy (2), radiculopathy (1), and spondylolisthesis (1). In all cases the cages had been packed with autograft (iliac crest 7, local 1) at the time of insertion. Cage implantation was performed with anterior (anterior lumbar interbody fusion 4, corpectomy and plate fixation 1), and posterior (posterior lumbar interbody fusion 4), segmental instrumentation (plate 1, or pedicle screws 8). All cases except one cervical case had posterolateral fusion or bilateral facet fusion. The cages were composed of carbon fiber-reinforced polymer (Brantigan cage; DePuy AcroMed, Raynham, MA, n = 5) or titanium mesh (Harms Cage; DePuy AcroMed, Raynham, MA, n = 4). Cages had been in situ from 8 to 72 months (mean 28 months). All nine biopsies from eight patients were obtained from within the center of the cages. Specimens were decalcified, routinely embedded in paraffin, stained with hematoxylin and eosin, and viewed qualitatively with transmitted and polarized light. RESULTS: All needle biopsies were obtained from within the center of the cages, and no patient developed spinal instability after the biopsy. All nine biopsies showed small fragments of necrotic bone associated with viable bone and restoration of hematopoietic bone marrow. Numerous cement lines demarcated the edges of previous cycles of remodeling. The ratio of necrotic to viable bone varied greatly among cases. Small particles of debris were associated with four of the five carbon-fiber cages and one of the four specimens from titanium cages, but there was no visible bone resorption or inflammation. CONCLUSIONS: Autogenous bone graft was incorporated in these radiographically successful human intervertebral body fusion cages. A few debris particles were observed, but there was no histologic evidence of particle-induced bone resorption or inflammation.  相似文献

Objective:The Wavelia Microwave Breast Imaging (MBI) system, based on non-ionising imaging technology, has demonstrated exciting potential in the detection and localisation of breast pathology in symptomatic patients. In this study, the ability of the system to accurately estimate the size and likelihood of malignancy of breast lesions is detailed, and its clinical usefulness determined.Methods:Institutional review board and Health Products Regulatory Authority (HPRA) approval were obtained. Patients were recruited from the symptomatic unit to three groups; breast cancer (Group-1), unaspirated cysts (Group-2) and biopsied benign lesions (Group-3). MBI, radiological and histopathological findings were reviewed. MBI size estimations were compared with the sizes determined by conventional imaging and histopathology. A Quadratic Discriminant Analysis (QDA) classifier was trained in a 3D feature space to discriminate malignant from benign lesions. An independent review was performed by two independent breast radiologists.Results:24 patients (11 Group-1, 8 Group-2 and 5 Group-3) underwent MBI. The Wavelia system was more accurate than conventional imaging in size estimation of breast cancers. The QDA accurately separated benign from malignant breast lesions in 88.5% of cases. The addition of MBI and the Wavelia malignancy risk calculation was deemed useful by the two radiologists in 70.6% of cases.Conclusion:The results from this MBI investigation demonstrate the potential of this novel system in estimating size and malignancy risk of breast lesions. This system holds significant promise as a potential non-invasive, comfortable, and harmless adjunct for breast cancer diagnosis. Further larger studies are under preparation to validate the findings of this study.Advances in knowledge:This study details the potential of the Wavelia MBI system in delineating size and malignancy risk of benign and malignant breast lesions in a symptomatic cohort. Previous studies point towards differential connectivity patterns among basolateral (BLA) and centromedial (CMA) amygdala regions in patients with posttraumatic stress disorder (PTSD) as compared with controls. Here we describe the first study to compare directly connectivity patterns of the BLA and CMA complexes between PTSD patients with and without the dissociative subtype (PTSD+DS and PTSD−DS, respectively). Amygdala connectivity to regulatory prefrontal regions and parietal regions involved in consciousness and proprioception were expected to differ between these two groups based on differential limbic regulation and behavioral symptoms. PTSD patients (n=49) with (n=13) and without (n=36) the dissociative subtype and age-matched healthy controls (n=40) underwent resting-state fMRI. Bilateral BLA and CMA connectivity patterns were compared using a seed-based approach via SPM Anatomy Toolbox. Among patients with PTSD, the PTSD+DS group exhibited greater amygdala functional connectivity to prefrontal regions involved in emotion regulation (bilateral BLA and left CMA to the middle frontal gyrus and bilateral CMA to the medial frontal gyrus) as compared with the PTSD−DS group. In addition, the PTSD+DS group showed greater amygdala connectivity to regions involved in consciousness, awareness, and proprioception—implicated in depersonalization and derealization (left BLA to superior parietal lobe and cerebellar culmen; left CMA to dorsal posterior cingulate and precuneus). Differences in amygdala complex connectivity to specific brain regions parallel the unique symptom profiles of the PTSD subgroups and point towards unique biological markers of the dissociative subtype of PTSD.  相似文献

A method to extract genomic DNA from oocysts of Cryptosporidium parvum in human faecal material was developed and consisted of a simple alkali wash, freeze/boil technique. This method was simple, quick, sensitive, inexpensive and resulted in the production of genomic DNA which was free of PCR inhibitors and as such was a suitable template for the detection of C. parvum by various PCR amplification targets.  相似文献

PCR-IMS was used to detect Cryptosporidium spp. in environmental water samples in Northern Ireland which had previously tested negative by a conventional IFA staining method. Oocysts of C. parvum detected in river water and final treated sewage effluent collected from various sites along the river Lagan were identified as genotype 2 (animal origin) based on polymorphisms observed at the thrombospondin related adhesion protein gene locus. Similarly, genotype 1 (human origin) oocysts of C. parvum were detected in the marine filter feeder mussel, Mytilus edulis, collected from the shores of Belfast Lough. Detection of the human genotype of Cryptosporidium in mussels destined for human consumption identifies the organism's serious potential as a foodborne pathogen. This work highlights the possible value of monitoring filter feeder systems, in conjunction with specific molecular epidemiological tools, as an alternative monitoring system for the parasite within the aquatic environment.  相似文献
