The large number of pharmacological agents available to treat type 2 diabetes (T2D) makes choosing the optimal drug for any given patient a complex task. Because newer agents offer several advantages, whether and when sulphonylureas (SUs) should still be used to treat T2D is controversial. Published treatment guidelines and recommendations should govern the general approach to diabetes management. However, expert opinions can aid in better understanding local practices and in formulating individual choices. The current consensus paper aims to provide additional guidance on the use of SUs in T2D. We summarize current local treatment guidelines in European countries, showing that SUs are still widely proposed as second-line treatment after metformin and are often ranked at the same level as newer glucose-lowering medications. Strong evidence now shows that sodium-glucose co-transporter-2 inhibitors (SGLT-2is) and glucagon-like peptide-1 receptor agonists (GLP-1RAs) are associated with low hypoglycaemia risk, promote weight loss, and exert a positive impact on vascular, cardiac and renal endpoints. Thus, using SUs in place of SGLT-2is and GLP-1RAs may deprive patients of key advantages and potentially important cardiorenal benefits. In subjects with ascertained cardiovascular disease or at very high cardiovascular risk, SGLT-2is and/or GLP-1RAs should be used as part of diabetes management, in the absence of contraindications. AbstractObjective: The Fragile X Mental Retardation-1 (FMR1) premutation is a common genetic abnormality, affecting ~1:150 women in the United States. Clinical neuropsychologists are becoming increasingly aware of their role in the clinical management of the FMR1 premutation, which is associated with risk for a range of cognitive, executive, neuromotor, and psychological impairments, including neurodegenerative disease. This study investigated atypical eye contact as a critical neuropsychological phenotype associated with the FMR1 premutation. Methods: Thirty-eight women with the FMR1 premutation and 27 control women engaged in a 20-min conversational sample with an examiner. Eye contact quality was coded from the videotaped samples by blinded coders. Mixed models tested group differences in eye contact during the beginning and the end of the conversation. Social anxiety and broad autism phenotype (BAP) traits were tested as predictors of eye contact quality across the groups. Results: Women with the FMR1 premutation exhibited significantly reduced eye contact during both the beginning and the end of the social interaction, despite a ‘warm-up’ effect where eye contact improved by the end of the interaction. Eye contact quality was not associated with social anxiety or BAP traits. Conclusions: This study supports reduced eye contact as a phenotypic feature of the FMR1 premutation, which presents independent of social anxiety and the BAP. These findings contribute to a growing understanding of the neuropsychological phenotype of the FMR1 premutation, which has public health implications given that >1 million individuals in the United States carry this genetic abnormality. 相似文献

Colorimetric and fluorescent probes based on small organic molecules have become important tools in modern biology because they provide dynamic information concerning the localization and quantity of the molecules and ions of interest without the need for genetic engineering of the sample. In the past five years, these probes for ions and molecules have attracted great attention because of their biological, environmental and industrial significance combined with the simplicity and high sensitivity of absorption and fluorescence techniques. Moreover, pyrazole derivatives display a number of remarkable photophysical properties and wide synthetic versatility superior to those of other broadly used scaffolds. This review provides an overview of the recent (2016–2020) findings on chemosensors containing pyrazole derivatives (pyrazoles, pyrazolines and fused pyrazoles). The discussion focuses on the design and physicochemical properties of chemosensors in order to realize their full potential for practical applications in environmental and biological monitoring (sensing of metal ions, anions, explosives, and biomolecules). We also present our conclusions and outlook for the future.This review provides an overview on the recent (2016–2020) findings on chemosensors based on pyrazole derivatives.
