Preexpanded distant "super-thin" intercostal perforator flaps for facial reconstruction without the need for microsurgery.

BACKGROUND: Concept of the 'super-thin perforator flap' was introduced in 1994 by authors. Since then, various types of 'super-thin perforator flaps' were applied successfully especially for contour sensitive reconstruction such as face and neck. METHODS: Eleven patients requiring large flaps who presented with extensive disfiguring facial scar (male: seven cases, female: four cases). On the consideration of flaps' colour, texture and thickness requirements, the authors selected 'super-thin' anterior intercostal perforator flaps (AICP, range from 4 x 14 cm to 25 cm x 9 cm) for reconstruction purpose. First, tissue expanders (volume range from 800 cc to 1200 cc) were carefully inserted under the AICP. After the flaps were expanded for 2 months, distant scars were removed and the covering super-thinned flaps were transferred into recipient site. Two weeks later, pedicles in the anterior chest were cut down and flaps were transferred to replace all the left scars. RESULTS: Flap were survived without any complications. The colour, texture and thickness of the transferred flap were satisfactory, shrink of flaps were not observed after long term follow-up. The authors present a method of facial reconstruction that has the advantages of creating a large amount of thin tissue of both good colour and texture, without the need of microsurgery and few disadvantages of donor-site morbidity. The disadvantages are three-staged procedures, complications of tissue expansion and uncomfortable compulsory posture for patients. In our opinion, this is an alternative method of choice for reconstructing all large defects in the lower two-thirds of the face.

目的探讨颈椎病的临床及影像学表现,以期提高对本病的认识。方法对86例颈椎病X线,CT,MR影像学表现进行分析。结果86例中,颈椎病主要影像学改变为颈椎生理曲度异常,椎体和钩突关节骨质增生,椎间隙变窄,椎管狭窄和颈部韧带骨化。结论颈椎病影像学改变具有一定的特征性,有助于颈椎病的诊断,为临床治疗提供了依据和参考价值。

不同时辰刺激大鼠的交叉上核对血浆中皮质酮含量的影响

应用拉丁方设计，在不同体重组的大鼠，于不同时辰，对交叉上核采取不同强度的刺激，用放射免疫法测定外周血中皮质酮的含量。其结果显示：各体重组间无显著性差异，不同时辰血浆皮质酮的平均含量各组呈现基本相同的节律性变化，无论何时辰刺激交叉上核使之兴奋，均可显著提高血浆皮质酮的含量。完全损毁交叉上核后７２小时血浆皮质酮含量和正常对照组无显著性差异，且其节律性也没有出现明显变化。     

枯草杆菌BF-7658α-淀粉酶生产菌株所产蛋白酶热稳定性研究

我国α—淀粉酶(EC3.2.1.1)生产是用枯草杆菌BF—7658菌株,它产生的酶是个复杂酶系。其中混杂的蛋白酶未见酸性蛋白酶。发酵液经热处理后,测蛋白酶活力。结果,碱性蛋白酶耐热性比中性蛋白酶强。发酵液经55℃,30分钟热处理后,碱性蛋白蛋残存活力与室温(20～25℃)下碱性蛋白酶活力相差不显著(P>0.05),而中性蛋白酶则相差显著(P<0.05)。用电泳法观察发酵液蛋白酶电泳谱型,显示两条蛋白酶区带。     

宫颈癌的治疗进展

官颈癌是最常见的妇科恶性肿瘤之一,其发病率及死亡率仅次于乳腺癌。对官颈癌的治疗,除应力求根除疾病,提高生存率外,还应尽可能争取保留患者的生殖内分泌功能,特别是对年轻的患者。为提高宫颈癌患者的治疗效果,化疗已成为中、晚期的重要治疗手段。

Comparison of clinical outcomes of frozen-thawed blastocysts transfer in natural and hormonally controlled cycles

Objectives:To assess the clinical outcomes of frozen-thawed blastocysts transfer in natural and hormonally controlled cycles.Methods:A retrospective analysis of natural and hormonally controlled cycle for 246 frozen-thawed blastocyst transfer cycles,the clinical pregnancy rate,implantation rate,early abortion rate were compared.Results:Of the 192 hormonally controlled cycles,the cancel rate,clinical pregnancy rate per ET,implantation rate and abortion rate were 7.3%(14/192),53.9%(96/178),38.8%(131/338)and 11.5%(11/96)respectively,whereas in 54 natural cycles,these rates were 16.7%(9/54),68.9%(31/45),52.9%(45/85)and 16.1%(5/31)respectively.There was no significant difference between the two groups with regard to the clinical pregnancy and abortion rate per ET,but the cancel rate and implantation rate were higher in natural cycles.However,the pregnancy and implantation rates of patients without PCOS in hormonal control cycles(57.2%,40.9%)were similar with those in natural cycles(P>0.05).Conclusion:These findings suggested that both hormonally controlled and natural cycles had similar pregnancy outcomes in frozen-thawed blastocysts transfer.     

类风湿性关节炎患者氧化还原物水平的测定

OBJECTIVE: To examine the oxidation or reduction products in patients with rheumatism arthritis (RA), and investigate the relationship between oxidation or reduction products and occurrence and development of RA. METHODS: The serum levels of total ascorbic acid (TAA), dehydroascorbic acid (DHAA)/TAA, vitamin E, advanced oxidation protein products (AOPP) and malondialdehyde (MDA) were detected by high-performance liquid chromatography with electrochemical detection in 83 RA patients and 30 healthy adults. Correlation analysis of AOPP, MDA and hs-CRP was performed. RESULTS: Compared with normal control group, significantly higher serum MDA, DHAA/TAA, and AOPP levels were detected in RA patients (P<0.05), but vitamin E showed no significant difference (P<0.05). Linear regression analysis showed that MDA (P<0.01) was positively but AOPP (P>0.05) negatively correlated to hs-CRP. CONCLUSIONS: Oxidation or reduction products in serum of RA patients increases significantly, which may be an important mechanism for the occurrence and development of RA. Serum AOPP and MDA levels can reflect the oxidation status in RA patients.
