Context

Little is known about patient and physician factors that affect decisions to pursue more or less aggressive treatment courses for patients with advanced illness.

Objectives

This study sought to determine how patient age, patient disposition, and physician religiousness affect physician recommendations in the context of advanced illness.

Methods

A survey was mailed to a stratified random sample of U.S. physicians, which included three vignettes depicting advanced illness scenarios: 1) cancer, 2) heart failure, and 3) dementia with acute infection. One vignette included experimental variables to test how patient age and patient disposition affected physician recommendations. After each vignette, physicians indicated their likelihood to recommend disease-directed medical care vs. hospice care.

Results

Among eligible physicians (n = 1878), 62% (n = 1156) responded. Patient age and stated patient disposition toward treatment did not significantly affect physician recommendations. Compared with religious physicians, physicians who reported that religious importance was “not applicable” were less likely to recommend chemotherapy (adjusted odds ratio [OR] 0.39, 95% CI 0.23–0.66) and more likely to recommend hospice (OR 1.90, 95% CI 1.15–3.16) for a patient with cancer. Compared with physicians who ever attended religious services, physicians who never attended were less likely to recommend left ventricular assist device placement for a patient with congestive heart failure (OR 0.57, 95% CI 0.35–0.92). In addition, Asian ethnicity was independently associated with recommending chemotherapy (OR 1.72, 95% CI 1.13–2.61) and being less likely to recommend hospice (OR 0.59, 95% CI 0.40–0.91) for the patient with cancer; and it was associated with recommending antibiotics for the patient with dementia and pneumonia (OR 1.64, 95% CI 1.08–2.50).

Conclusion

This study provides preliminary evidence that patient disposition toward more and less aggressive treatment in advanced illness does not substantially factor into physician recommendations. Non-religious physicians appear less likely to recommend disease-directed medical treatment in the setting of advanced illness, although this finding was not uniform and deserves further research.  相似文献

Significant progress has been made in radiation protection for children during the last 10 years. This includes increased awareness of the need for radiation protection for pediatric patients with international partnerships through the Alliance for Radiation Safety in Pediatric Imaging. This paper identifies five areas of significant progress in radiation safety for children: the growth of the Alliance; the development of an adult radiation protection campaign Image Wisely?; increased collaboration with government agencies, societies and the vendor community; the development of national guidelines in pediatric nuclear medicine, and the development of a size-based patient dose correction factor by the American Association of Physicists in Medicine, Task Group 204. However, many challenges remain. These include the need for continued education and change of practice at adult-focused hospitals where many pediatric CT exams are performed; the need for increased emphasis on appropriateness of pediatric imaging and outcomes research to validate the performance of CT studies, and the advancement of the work of the first pediatric national dose registry to determine the "state of the practice" with the final goal of establishing ranges of optimal CT technique for specific scan indications when imaging children with CT.  相似文献

OBJECTIVE: The purpose of this study was to evaluate the CT appearance, management, and temporal course of persistent pulmonary interstitial emphysema in neonates. MATERIALS AND METHODS: Criteria for inclusion in the study group included neonates with a history of prematurity who required ventilation for lung disease, development of hyperexpanded radiolucent lung lesions after typical radiographic findings of pulmonary interstitial emphysema, and CT documentation of lung abnormalities. Radiographs and CT scans were reviewed for the anatomic distribution, appearance, and presence of classic lines or dots within a radiolucent mass. We compared the management (surgical vs nonsurgical) and the temporal course in nonsurgical cases for patients in the United States and patients outside the United States. RESULTS: From seven institutions, we identified 17 patients who had persistent pulmonary interstitial emphysema with CT documentation. On CT, all lesions consisted of hyperexpanded cystic radiolucencies. Distribution was single-lobe in nine patients (left upper lobe, n = 5; left lower lobe, n = 3; right middle lobe, n = 1), multilobar in eight patients, and bilateral in six patients. Fourteen patients (82%) showed the characteristic line-and-dot pattern. All patients were initially treated conservatively; nine eventually underwent surgical resection with confirmation at pathology. Of 10 patients who underwent nonsurgical treatment for at least 1 year, lesions resolved in four patients, decreased in size in three asymptomatic patients, and enlarged in three patients, with eventual resection in two. Surgical resection was performed in 89% (8/9) of patients treated in the United States and in 13% (1/8) of patients treated outside the United States. CONCLUSION: Our study found that 82% of patients with persistent pulmonary interstitial emphysema had characteristic CT findings (central lines and dots surrounded by radiolucency). Although most patients in this series eventually underwent surgical resection, initial treatment can be conservative. The decision to perform surgery may have reflected the local medical culture.  相似文献

The purpose of this study was to assess the current status of patient radiation exposure tracking internationally, gauge interest and develop recommendations for implementation. A survey questionnaire was distributed to representatives of countries to obtain information, including the existence of a patient exposure tracking program currently available in the country, plans for future programs, perceived needs and goals of future programs, which examinations will be tracked, whether procedure tracking alone or dose tracking is planned, and which dose quantities will be tracked. Responses from 76 countries, including all of the six most populous countries and 16 of the 20 most populous, showed that although no country has yet implemented a patient exposure tracking program at a national level, there is increased interest in this issue. Eight countries (11%) indicated that such a program is actively being planned and 3 (4%) stated that they have a program for tracking procedures only, but not for dose. Twenty-two (29%) feel that such a program will be "extremely useful", 46 (60%) "very useful" and 8 (11%) "moderately useful", with no respondents stating "Mildly useful" or "Not useful". Ninety-nine percent of countries indicated an interest in developing and promoting such a program. In a first global survey covering 76 countries, it is clear that no country has yet achieved exposure tracking at a national level, although there are successful examples at sub-national level. Almost all have indicated interest and some have plans to achieve dose tracking in the near future.  相似文献
