腹腔镜手术治疗全内脏转位合并低位直肠癌1例及文献分析

目的 观察腹腔镜手术治疗全内脏转位合并直肠癌的疗效。 方法 回顾性分析2016年9月安徽医科大学第一附属医院胃肠外科收治的1例低位直肠癌合并全内脏转位并行腹腔镜手术治疗的患者临床资料,同时通过查阅复习相关文献,探讨其疗效及安全性。 结果 患者经检查提示全内脏转位合并直肠癌,行手术治疗,术后随访1年,患者均未出现相关并发症或肿瘤复发。经检索,全球报道的全内脏转位合并直肠癌患者15例,均接受手术治疗:其中6例为开放,7例腹腔镜,2例机器人。报道内容1例为手术录像,2例为护理体会,12例手术报道:其中1例为姑息性手术,14例为根治性手术,术后住院时间3~21 d,15例患者均未发生手术并发症。 结论 内脏转位可根据影像学检查明确诊断,腹腔镜下全内脏转位低位直肠癌手术安全有效,内脏转位一般不需处理,但术前明确诊断可减少其手术风险。     

Coexistence of salivary gland cysticercosis with squamous cell carcinoma of the mandible

Cysticercosis is a parasitic infestation caused by the pork tapeworm larval stage, Cysticercus cellulosae. The majority of the cases present in ocular, cerebral, and subcutaneous locations. We report the presence of cysticercosis inside the submandibular gland in association with squamous cell carcinoma of the inferior alveolar ramus of the mandible. To the best of our knowledge, this is the first case report documenting cysticercosis inside a salivary gland. A 65-year-old male presented with complaints of an ulcerative lesion on the inferior alveolar ramus present for 2 months. Histological examination revealed a keratinizing well-differentiated squamous cell carcinoma involving the alveolar margin and mandible. The histopathological examination of the submandibular gland revealed cysticercosis. This case emphasizes the importance of adequate sampling of all the tissues obtained for associated infectious disorders, more so in immunosuppressed patients, which will help the clinician to manage the case appropriately.     

Radiographic Outcomes in Treatment of Thumb Basal Joint Arthritis: Does Interposition of Acellular Dermal Allograft Prevent Metacarpal Settling?

Background  Osteoarthritis at the thumb carpometacarpal joint can have a profound impact on quality of life. Here, we evaluate radiographic outcomes in patients who have had open complete trapeziectomy, ligament reconstruction with tendon interposition, and acellular dermal matrix (GraftJacket) interposition—Group A, and compare them with those without GraftJacket interposition—Group B. Materials and Methods  Thirty patients who had undergone operative treatment for thumb basal joint arthritis by a single surgeon from 2009 to 2016 were identified, and charts were retrospectively reviewed for demographic data, surgical and radiographic outcomes, and complications. Results  There was no significant difference in pre- and postoperative radial abduction or pre- and postoperative palmar abduction. The difference in intraoperative joint space was significant ( p = 0.006), but the difference in postoperative joint space was not ( p = 0.310). The average amount of metacarpal settling was 6.9 versus 3.7 mm ( p = 0.035) (Groups A and B, respectively). Three patients in Group A developed an inflammatory reaction to the GraftJacket, and one required reoperation for allograft removal. Conclusion  This study suggests that thumb basal joint arthroplasty with GraftJacket interposition does not lead to more favorable radiographic outcomes at long-term follow-up. The increased costs associated with GraftJacket use may not be justified in light of these outcomes.     

Chronic meningitis in systemic lupus erythematosus: An unusual etiology

Chronic aseptic meningitis is a rare manifestation of systemic lupus erythematosus (SLE). Apart from immunological causes and drugs, the aseptic meningitis group can include some unidentified viral infections that cannot be detected by routine microbiological testing. It is imperative to do complete cerebrospinal fluid (CSF) workup before implicating the symptoms to disease activity or drugs, as untreated infections cause significant mortality in SLE. We present a case of young female with SLE who presented with chronic meningitis of an uncommon etiology.     

Olanzapine-induced restless leg syndrome: A case report and review of literature

Restless leg syndrome (RLS) is a common disorder causing sleep impairment. Antipsychotics particularly belonging to the first generation are a common cause of RLS. Whereas, RLS induced by olanzapine is rare, there are only a few cases reported earlier. We report a 38-year-old lady suffering from persistent delusional disorder who was prescribed olanzapine. She developed RLS after initiation of olanzapine which improved when switched over to risperidone. This report will caution clinicians about this side effect of a very commonly prescribed antipsychotic drug.KEY WORDS: Olanzapine, restless legs syndrome
