OBJECTIVE: To determine the effectiveness of a 10-day subcutaneous erythropoietin (rHuEpo) course of 300 units per kg per dose plus oral iron compared to oral iron alone in anemic infants during their convalescent phase of illness. STUDY DESIGN: Prospective, randomized trial performed at a 40-bed, teaching, referral, level III, neonatal intensive care unit. Infants with a gestational age at birth of less than 32 weeks, hematocrit of less than or equal to 28% with a corrected reticulocyte count of less than or equal to 5%, postconceptual age of less than 48 weeks or 5 months chronological age, and a diagnosis of anemia of prematurity were considered for inclusion. Major outcome parameters included hematocrit, corrected reticulocyte count and red cell transfusion requirements. RESULTS: A total of 60 infants were enrolled (n=30 per group). Infants randomized to rHuEpo had a significantly higher post-treatment hematocrit and corrected reticulocyte count than infants in the iron only group (p<0.001). There was a trend towards fewer red cell requirements in the rHuEpo group. CONCLUSIONS: The rHuEpo regimen studied here was associated with an acute improvement in hematocrit and corrected reticulocyte counts. This study did not demonstrate a statistically significant decrease in transfusion therapy, in part related to increased subsequent use of rHuEpo in the control group. Abstract: In this report, rat hypothalamic nitric oxide synthase (NOS) activity is shown to be partially inhibited by physiological concentrations of the pineal hormone melatonin. In vitro studies demonstrate that 1 nM melatonin, which approximates the physiological concentration of the hormone at night, significantly inhibited NOS activity. In vivo studies show that administering melatonin or collecting the hypothalamus from animals at night, when endogenous melatonin levels are elevated, results in a significant decrease of NOS activity. Results also show that calmodulin may be involved in this process since its presence in the incubation medium prevents the inhibitory effect of melatonin on NOS activity. 相似文献

The beStent is a new stainless steel, balloon-expandable mesh stent which has a unique serpentine design. Rotation of the unique low stress junctions upon expansion leads to orthogonal locking of the wires, maximizing radial strength and assuring zero shortening. The stent has delineating gold markers which assure precise positioning. We aim to present the initial acute results in a pilot registry for stent evaluation. Two hundred eighty-four stents were used in a total of 217 patients (age 57.9 ± 3.10 years; 178 males; 39 females) in seven centers, for variable indications. Stents of 15-, 25-, and 35-mm length were used. The arteries treated were the left anterior descending (n = 112, 42%), circumflex (n = 54, 20.2%), right coronary (n = 95, 35.5%), left main (n = 1, 0.4%), and vein graft (n = 5, 1.9%). Lesion types were: A in 42 patients (16.5%); B1 in 53 patients (20.7%); B2 in 81 patients (31.8%); and C in 79 patients (31%). One hundred fifty-nine patients required one stent, 40 patients required two stents, and 18 patients required three or more stents. Anticoagulation protocol included procedural heparin with aspirin with/without ticlopidine. Smooth angiographie results were obtained in all cases with no plaque herniation. Acute angiographic success was obtained in 97% of the patients, and acute clinical success in 95% of the patients. Complications within 30 days were: 3 deaths (1.4%) (2 noncardiac); 2 (0.9%) myocardial infarctions; and 2 (0.9%) stent thromboses. Therefore, the beStent is useful in treatment of complex lesions of variable length and complexity, providing excellent acute results with a low complication rate, in spite of unfavorable basic clinical and angiographie characteristics. 相似文献

Summary. Bone mineralisation during and after limb lengthening procedures on the femur or tibia using unilateral fixators has been
monitored quantitatively using dual energy X-ray absorptiometry (DEXA). We measured the bone mineral density (BMD) prospectively
in the newly formed callus, in the bone adjacent to the callus and in the proximal femur. In twenty-one patients we showed
a typical course with a peak value at 4 – 6 weeks after beginning distraction and a minimum value at maximum distraction.
In the consolidation period the BMD in the distraction gap increased until the fixator was removed. The BMD in the regenerated
bone increased faster in the regions of interest (ROI) opposite the fixator compared to those near it. Dynamisation caused
more homogeneous regeneration equalising V

in the different ROIs. The BMD in the proximal femur of the leg which was operated on decreased to 67% and in the opposite
leg to 87% of the preoperative value. DEXA provides a precise and quantitative assessment of callus and bone mineralisation
during limb lengthening and helps in understanding what is happening during these procedures.

Résumé. Le processus de la minéralisation peut précisément être étudié d’une manière quantitative pendant et après la distraction du cal au moyen de l’absorptiométrie en énergie-binaire (DEXA) en créant une petite radioexposition. Des patients (n = 21) avec une distraction du cal, auxquels nous avons mesuré prospectivement la densité minérale de l’os (bone mineral density = BMD) dans et autour du cal nouvellement formé, étaient traités avec des fixateurs externes unilatéraux. La minéralisation a montré une courbe typique avec une première pointe de valeur [0,365 +/– 0,196 g/cm 2 (30,9% de la première valeur)] à 4 – 6 semaines après le début de la distraction. Une valeur minimale apparut à la distraction maximale. Dans la période de consolidation la BMD est montèe jusqu’au moment de l’enlèvement du fixateur à 1,020 +/– 0,234 g/cm2 (87%). La minéralisation du cal, mésurée grace à la technique des ?regions of interest (ROIs)?, augmenté plus vite aux ROIs éloignées qu’aux ROIs très proches du fixateur. De la dynamisation du fixateur résulte une vitesse de minéralisation plus homogène. Nous considérons la DEXA comme une méthode précise pour étudier les processus de la minéralisation et du développement du cal pendant la distraction avec un fixateur unilatéral. En tenant compte des limites de la technique, la DEXA – qui nous livre des valeurs quantitatives – nous aide à comprendre ce qui se passe pendant la distraction.

N -Acetyl-5-methoxytryptamine, commonly known as melatonin, was isolated and structurally identified by a dermatologist, Aaron B. Lerner, in 1958. His interest stemmed from the fact that melatonin has potent skin-lightening effects in amphibians and some other non-mammalian species. In humans, however, melatonin is not capable of lightening skin or in reducing pigmentation. While melatonin synthesis is the best known to occur in the pineal gland, it is also produced in several other organs including the skin. Melatonin is a powerful, multifaceted direct free radical scavenger and indirect antioxidant. Numerous studies have shown that melatonin production wanes with increasing age and its loss has been speculated to be consequential in free radical-mediated cellular and organ deterioration that occurs in the elderly. Additionally, a number of free radical-related diseases, e.g. Alzheimer's disease, Parkinsonism, cataract formation, etc., may in part be a consequence of the loss of endogenous melatonin production. The administration of melatonin in animal models of these diseases typically defers their progression and limits their severity. Likewise, due to its antioxidant properties and via other mechanisms, melatonin may reduce skin aging. Melatonin, when applied topically to the skin of humans, has obvious protective effects against ultraviolet (UV) light-induced erythema. Also, lipid peroxidation in human skin fibroblasts due to their exposure to UV-B is reduced when melatonin is present. 相似文献

Neuropathology in non-human immunodeficiency virus-infected drug addicts: hypoxic brain damage after chronic intravenous drug abuse 总被引:3,自引:0,他引:3
Neuropathological studies were carried out on 180 human immunodeficiency virus-seronegative intravenous drug addicts. The
findings in victims of acute heroin intoxication (n = 116) were congestion (99.1%), capillary engorgement (68.1%), and/or perivascular bleeding (68.1%) – hemodynamic processes
attributable to toxic primary respiratory failure. In a high percentage of these cases (88%), cerebral edema was also present.
In 18 cases of acute heroin intoxication who survived for periods of hours or days, the sole postmortem finding was ischemic
nerve cell damage, resembling that typically seen in systemic hypoxia. Semiquantitative analysis revealed nerve cell loss
in the hippocampal formation and/ or Purkinje cell layer in 26% of the 162 chronic drug abusers. By contrast, in nearly 80%
of these cases, the hippocampus showed enhanced expression of glial fibrillary acid protein by astrocytes and/or a proliferation
of microglia, demonstrated by CD68 expression. Since such reactive processes are produced by primary neuronal damage, it can
be assumed that chronic intravenous drug abuse results in obviously ischemic nerve cell loss. This could be demonstrated in
the hippocampus, but it must also occur throughout the whole brain. The demonstration of ischemic nerve cell damage and neuronal
loss or secondary reactive alterations has not been described previously.
