The prevalence of “vertebral endplate signal changes” (VESC) and its association with low back pain (LBP) varies greatly between
studies. This wide range in reported prevalence rates and associations with LBP could be explained by differences in the definitions
of VESC, LBP, or study sample. The objectives of this systematic critical review were to investigate the current literature
in relation to the prevalence of VESC (including Modic changes) and the association with non-specific low back pain (LBP).
The MEDLINE, EMBASE, and SveMED databases were searched for the period 1984 to November 2007. Included were the articles that
reported the prevalence of VESC in non-LBP, general, working, and clinical populations. Included were also articles that investigated
the association between VESC and LBP. Articles on specific LBP conditions were excluded. A checklist including items related
to the research questions and overall quality of the articles was used for data collection and quality assessment. The reported
prevalence rates were studied in relation to mean age, gender, study sample, year of publication, country of study, and quality
score. To estimate the association between VESC and LBP, 2 × 2 tables were created to calculate the exact odds ratio (OR)
with 95% confidence intervals. Eighty-two study samples from 77 original articles were identified and included in the analysis.
The median of the reported prevalence rates for any type of VESC was 43% in patients with non-specific LBP and/or sciatica
and 6% in non-clinical populations. The prevalence was positively associated with age and was negatively associated with the
overall quality of the studies. A positive association between VESC and non-specific LBP was found in seven of ten studies
from the general, working, and clinical populations with ORs from 2.0 to 19.9. This systematic review shows that VESC is a
common MRI-finding in patients with non-specific LBP and is associated with pain. However, it should be noted that VESC may

Full‐thickness 5 mm punch biopsy wounds were made lateral to the sacrum in 48 smokers and 30 never smokers. After 1 week, the wounds were excised and fixed. The smokers were then randomized to continuous smoking or abstinence with a transdermal nicotine patch or a placebo patch. The sequence of wounding and excision was repeated after 4, 8, and 12 weeks. All excised tissue was stained with hematoxylin–eosin and immunohistochemically for macrophages (CD68), procollagen 1 N‐terminal propeptide (PINP) in fibroblasts, and endothelial cells (CD31). The cellularity was assessed and scored by two independent histopathologists, and for the analysis, proportional odds models and random effect models for repeated measurements were applied. Macrophages and PINP‐stained fibroblasts were reduced in the smokers' wounds (0.28 [0.14–0.58] [OR, 95%CI]; p=0.01 and 0.37[0.19–0.70]; p<0.01, respectively, when compared with never smokers' wounds). Inflammation scores were marginally affected. Following smoking cessation, inflammatory cell infiltration and macrophages in the wounds increased. PINP‐stained fibroblasts were unaffected. Neovascularization was not affected by smoking or abstinence. Wound inflammation and fibroblast proliferation were attenuated in smokers, suggesting delayed healing. Abstinence from smoking restores inflammation, but does not affect proliferation. These findings suggest a pathophysiologic mechanism for postoperative wound infection and dehiscence in smokers and why smoking cessation appears to reduce wound infection but not dehiscence. 相似文献

Lactococcus lactis strain Plasma (LC-Plasma) is reported to have anti-viral effects via direct activation of plasmacytoid dendritic cells, which upregulate the production of type I and III interferons. A randomized, placebo-controlled, double-blind, parallel group study was designed for elementary schoolchildren, grades 1 to 3, in Vietnam. LC-Plasma or a control were administered to schoolchildren as a beverage (1.0 × 1011 count LC-Plasma/day/person). The primary endpoint was to determine the efficacy of LC-Plasma in reducing the cumulative days absent from school due to upper respiratory disease (URID) and gastrointestinal disease (GID), and the secondary endpoint was to evaluate the potency of LC-Plasma on URID/GID symptoms and general well-being scores. LC-Plasma intake significantly reduced the cumulative days absent from school due to URID/GID (Odds ratio (OR) = 0.57, p = 0.004) and URID alone (OR = 0.56, p = 0.005); LC-Plasma also significantly reduced the number of cumulative fever positive days during the first 4 weeks of intervention (OR = 0.58, p = 0.001) and cumulative days with diarrhea during the last 4 weeks of the intervention period (OR = 0.78, p = 0.01). The number of positive general wellbeing days was significantly improved in the LC-Plasma group compared with the control throughout the intervention period (OR = 0.93, 0.93, p = 0.03, 0.04 in the first and last 4 weeks of the intervention, respectively). These data suggest that LC-Plasma seems to improve the health condition of elementary schoolchildren and reduces school absenteeism due to infectious disease, especially URID. 相似文献

Aim: This study investigated the effect of a 15‐week life style intervention (hypocaloric diet and regular exercise) on glucose tolerance, skeletal muscle lipids and muscle metabolic adaptations in 14 female and 9 male morbidly obese subjects (age: 32.5 ± 2.3 years, body mass index: 46.1 ± 1.9 kg m?2). Method: Before and after the life style intervention, an oral glucose tolerance test was performed and a muscle biopsy was obtained in the fasted state. Maximal oxygen uptake was measured by an indirect test. Results: After the intervention, body weight was decreased (P < 0.05) by 11 ± 1%, maximal oxygen uptake increased (P < 0.05) by 18 ± 5% and glucose tolerance increased (P < 0.05) by 12 ± 3%. Muscle glycogen was significantly increased by 47 ± 14%, but muscle ceramide and triacylglycerol content remained completely unchanged. No sex difference was observed for any of these parameters, but during submaximal exercise a marked decrease (P < 0.05) of 15 ± 2% in respiratory exchange ratio was seen only in females indicating an enhanced fat oxidation. Conclusion: Despite a marked weight loss and an improved aerobic capacity muscle ceramide and triacylglycerol remained unchanged after intensive life style intervention, and muscle lipids hence do not seem to play a major role for the improved glucose tolerance in these morbidly obese subjects. Interestingly, only the females improved fat oxidation during submaximal exercise after the intervention implying the presence of a sex‐dependent response to intensive life style adaptation. 相似文献

We have recently shown that the gene BRD1 is associated with schizophrenia and bipolar affective disorder and that the BRD1 protein (BRD1) which is expressed in neurons may occur in a short and a long variant. The aim of the study was to generate
polyclonal antibodies against new BRD1 epitopes enabling discrimination between the long and short BRD1 variants, and elucidate
the BRD1 distribution in several human tissues, including the CNS. Polyclonal rabbit antibodies were raised against three
different BRD1 epitopes. One (67) was specific for the long BRD1 variant, whereas the two others (63/64 and 65/66) like the
original monoclonal mouse antibody (K22) were predicted to stain both variants. Immunohistochemical staining procedures were
subsequently performed on paraffin-embedded human cerebral cortex and microarray slides containing 30 different human tissues.
Western blotting confirmed the predicted specificity of the developed antibodies. K22, 63/64 and 65/66 displayed a similar
neuronal staining pattern characterized by a distinct but weak nuclear staining, while the surrounding cytoplasm and proximal
dendrites were more intensely stained. Interestingly, staining with 67 generated in contrast primarily an intense nuclear
staining. The new antibodies resulted, furthermore, in a prominent neuroglial reaction characterized by staining of cell bodies,
nuclei and glial processes. The tissue microarray analysis revealed that BRD1 was widely distributed in human tissues. The
particular expression profile, e.g., the degree of nuclear and/or cytoplasmatic staining, seemed, however, to be highly tissue
dependent. These results suggest a general role of BRD1 in the cell and stress that the two BRD1 variants may play different

Objective
The present study evaluated ankle stiffness in adults with and without neurological disorders and investigated the accuracy and reproducibility of a clinically applicable method using a dynamometer.Methods
Measurements were obtained from 8 healthy subjects (age 39.3), 9 subjects with spastic cerebral palsy (CP) (age 39.8) and 8 subjects with multiple sclerosis (MS) (age 49.9). Slow and fast dorsiflexion stretches of the ankle joint were performed to evaluate passive muscle-tendon-joint stiffness, reflex mediated stiffness and range of movement (ROM), respectively. Intra/inter-rater reliability for passive and reflex mediated ankle muscle stiffness was assessed for all groups.Results
Subjects with CP and MS showed significantly larger values of passive stiffness in the triceps surae muscle tendon complex and smaller ROM compared to healthy individuals, while no significant difference in reflex mediated stiffness. Measurements of passive muscle-tendon-joint stiffness and reflex mediated stiffness showed good to excellent inter- and intra-rater reliability (ICC: 0.62–0.91) in all groups.Conclusion
Increased stiffness was found in subjects with CP and MS with a clinically applicable method that provides valid and reproducible measurement of passive ankle muscle-tendon-joint stiffness and reflex mediated stiffness.Significance

Studies of connective tissue from
patients with inguinal hernia have shown that smoking may be associated
with hernia formation due to a defective connective tissue metabolism.
Whether smoking is a risk factor for recurrence, too, was examined in
this study. From December 1990 through December 1995, 649 patients
underwent hernia repair as open sutured repair (Cooper ligament or
abdominal ring repair) or as open mesh repair. Five hundred forty-four
eligible patients were evaluated for recurrence 2 years
postoperatively. Association between recurrence and 17 patient-,
disease-, and intraoperative variables were analyzed by multiple
logistic regression. The results showed that smoking was significantly
and independently associated with recurrence compared to nonsmoking
[odds ratio (OR = 2.22; 95% confidence interval (95% CI) =
1.19–4.15)]. Open sutured repair compared to open mesh repair was the
most significant predictor for recurrence (OR = 7.23; 95% CI
= 3.01–17.37). Surprisingly, local anesthesia was associated with a
higher risk of recurrence compared to general anesthesia (OR =
2.44; 95% CI = 1.19–5.09). Potential confounders and other risk
factors for hernia recurrence such as age, alcohol consumption,
previous surgery, and anatomical characteristics of the hernia were
adjusted for in the analysis. In conclusion, smoking is an important
risk factor for recurrence of groin hernia, presumably due to an
abnormal connective tissue metabolism in smokers. 相似文献

Nanoscale structural analyses of biomineralized materials can frequently help elucidate important structure-function relationships in these complex organic-inorganic composites. Atomic force microscope (AFM) imaging of the exterior surface of trabecular bone reveals a densely woven structure of collagen fibrils, banded with a 67-nm periodicity, and densely packed mineral plates. The mineral plates on the collagen fibrils overlap and exhibit a large range of plate diameters from 30 to 200 nm. On the collagen fibrils, small nodular features, spaced 20-30 nm, run perpendicular to the fibrils. In some cases, these nodules are also seen on filaments extending between collagen fibrils. We hypothesize that these protrusions are noncollagenous proteins such as proteoglycans and may have collapsed into compact structures when the sample was dried. AFM images of pristine fractured surfaces reveal a dense array of mineral plates. In a few isolated locations, short sections of bare collagen fibrils are visible. In other regions, the existence of the underlying collagen fibrils can be inferred from the linear patterns of the mineral plates. Fractured samples, rinsed to remove mineral plates, reveal separated collagen fibrils on the fractured surfaces. These fibrils are often covered with protrusions similar to those observed on the exterior surfaces but are less organized. In addition, as on the exterior surfaces, there are sometimes small filaments extending between neighboring collagen fibrils. These studies provide important insights into the nanostructured architecture of this complex biocomposite. 相似文献
