INTRODUCTION
Ochronosis is a rare disorder which is defined as the deposition of metabolites of oxidation and polymerization of homogentisic acid, which have high affinity to collogen, in the connective tissues. It is a clinical condition characterized with ochronotic pigmentation of tissues, degenerative arthropathy of especially large joints and black discoloration of urine. In this paper we present a case of ochronosis diagnosed with biopsy and additional tests when a black discoloration of menisci and joint cartilage were detected during arthroscopic intervention for a degenerative meniscus tear.PRESENTATION OF CASE
A forty two year-old male patient was operated for lateral meniscus tear of his right knee. The arthroscopic examination of right knee revealed black colored synovial hypertrophy and torn lateral meniscus. Partial meniscectomy was performed. The diagnosis of ochronosis was made after histopathologic examination.DISCUSSION
Ochronotic pigment can accumulate in hyaline cartilage, tendon, skin, teeth, nail, sclera, tympanic membrane, heart valves, renal tubular cells, duramater, pancreas and walls of large arteries. In ochronosis the most frequently involved joints are knee and hip. In ochronotic arthropathy, articular cartilage become more sensitive to mechanical stresses. Our patient had meniscal tear, cartilage damage and black discoloration of synovial tissues and meniscus.CONCLUSION
Arthroscopy may be helpful in diagnosis of ochronotic arthropathy. 相似文献4.
The utilization of the metacarpal bones and interosseous muscles in the reconstruction of the hand should be based on the vascular anatomy of the metacarpal bones and the interosseous muscles. The authors studied the vascular anatomy of the metacarpal bones and the interosseous muscles to design a split metacarpal musculoosseous flap. Eighteen cadaveric hands from 9 cadavers were included in the study. The dorsal metacarpal arteries arise from the arch and course along the metacarpal bones closer to the ulnar borders of the bones supplying their periosteum through the muscular branches. Despite the indistinct pattern of muscular supply and anastomotic branches to the palmar surface, in all hands the arteries extend constantly along the metacarpal bone closer to the ulnar border. For defects or any pathology of the carpal bones, the metacarpal bones could be split at the ulnar border distally and a split metacarpal musculoosseous flap (based proximally depending on the dorsal metacarpal artery) could be performed (or based distally along with a distal intermetacarpal anastomosis). 相似文献

In the periphery, B cells differentiate in germinal centres (GCs) of secondary lymphoid organs. Isolated GC cells die quickly in vitro by apoptosis. Therefore, cell lines originating from follicular lymphomas, which are the malignant counterparts of GC B cells, would provide a stable in vitro model to study the immunobiology of GC B cells. We have established three novel human follicular lymphoma cell lines that were characterized with special reference to immunophenotypic features, response to B-cell receptor (BCR) triggering, response to cytokines and cytokine mRNA expression. One of the cell lines, HF-1A3, has a phenotype of a centrocyte. It expresses surface immunoglobulin G (sIgG) and dies by apoptosis following BCR cross-linking. Co-stimulation with interleukin-6 (IL-6), IL-15 or interferon-gamma (IFN-gamma) rescues HF-1A3 cells from BCR-induced apoptosis. The second cell line, HF-28, also represents phenotypically an IgG+ centrocyte. Ligation of its BCR leads to the cell-cycle arrest at G1 instead of apoptosis. HF-28 cells express both CD45RA and RO isoforms, which is unusual in B lymphocytes apart from plasma cells, thus suggesting a transition to plasma cell phenotype. The third cell line, HF-4.9, which phenotypically represents an sIgM+ centroblast, responds by proliferation to BCR cross-linking. These cell lines offer a unique in vitro model to study antigenic selection and cytokine-mediated growth regulation of human GC B cells. 相似文献
