OBJECTIVES: We analyzed the methods and outcomes of urethroplasty in men with complex urethral disruptions. METHODS: The medical records of 40 men with complex urethral disruptions were analyzed. Surgical methods were individualized according to stricture location, severity and length of the stricture, bladder neck characteristics and presence of complicating factors. Patients were divided into four groups based on the above characteristics. RESULTS: End-to-end urethroplasty performed in six patients with short bulbar strictures (<3 cm) was successful in all. Elaborated perineal repair was performed in 10 patients with intermediate (3-6 cm) strictures with or without complicating factors. Elaborated perineal repair with urethral substitution was performed in nine patients with long segment stricture (>6 cm). Abdominal transpubic repair was successfully applied to patients with rectourethral fistula or lacerated bladder neck. Success rate of anastomotic urethroplasty was 95% while over all success rate was 85%. CONCLUSION: Guidelines for urethral reconstruction of complex urethral disruptions are predicated on stricture length, location, bladder neck characteristics and associated complicating factors. End-to-end urethroplasty with stricture excision is highly reliable for short strictures for which previous operative repair have failed. Elaborated perineal repair is extremely versatile for intermediate and longer strictures with associated complicating factors. Abdominal transpubic urethroplasty is effective for patients with rectourethral fistula or lacerated bladder neck.  相似文献

OBJECTIVE: Recent studies of patients with juvenile bipolar disorder report low rates of recovery and high rates of chronicity. However, we lack data on the short-term outcome. This study examines the pattern of recovery from the index episode in an aggressively treated juvenile sample. METHOD: We assessed 25 subjects (< 16 years) with a diagnosis of mania, using the Diagnostic Interview for Children and Adolescents-Revised) (DICA-R), Young Mania Rating Scale (YMRS), and Children's Global Assessment Scale (CGAS) at intake and at 3 and 6 months. We studied the time taken to recover from the index episode, the level of functioning, and the factors predicting them. RESULTS: After 6 months, 24 (96%) subjects had recovered from the index manic episode. The median time to recovery was 27 days. Total episode length was significantly longer among those with previous affective episodes. CONCLUSIONS: The findings suggest that juvenile-onset mania has high rates of recovery and low rates of chronicity. These differences from the existing literature need further exploration.  相似文献

A commercially available ADAC Pinnacle(3) radiation treatment planning system has been used to model electron beams from a Varian Clinac 2300C/D in the energy range of 6 to 22 MeV. Prior to clinical use, the dosimetric characteristics of the beams have to be modeled accurately. As a first step for beam modeling, a number of dose profile and depth dose measurements were taken at standard source-to-surface distance (SSD) of 100 cm. Dose profiles and depth dose measurements at extended SSDs up to 120 cm are important for ascertaining accuracy of the model, as well as their clinical usefulness in the treatment of some sites (e.g., head-and-neck tumors). Modeled and measured beam data were compared. Over 98% of comparison points (modeled vs. measured) at 100-cm SSD were within 2.5% or 2.5 mm. At 110 cm SSD, over 98% of compared points were within 4% or 4 mm, and at 120-cm SSD, over 98% of compared points were within 5% or 5 mm. Overall, more than 98% of compared points were within 4% or 4 mm. Better models were produced for lower energies (6 to 15 MeV) than higher energies (18 and 22 MeV). For 6, 9, 12, and 15 MeV, 89% of compared points were within 2% or 2 mm. For 18- and 22-MeV electron energies, 75% and 67%, respectively, were within 2% or 2 mm. These results are consistent with the recommendations of AAPM Task Group Report 53.  相似文献

The effects of acute and chronic stages of carrageenan-induced air-pouch inflammation on the pharmacokinetics of prednisolone were studied in male Wistar rats. Chronic inflammation produced a significant increase in the area under the curve (AUC) of prednisolone compared to control animals (6594 ± 2144 vs 3530 ± 2164 µg · hr/ L). The effect of acute inflammation was not significant (AUC = 4996 ± 3813). Both acute and chronic inflammation also reduced thein vitro plasma protein binding of prednisolone, the reduction being much greater after chronic inflammation. The AUC of free prednisolone after chronic inflammation was 3141 µg · hr/L, compared to 1121 µg · hr/L in the control group and 1823 µg · hr/L after acute inflammation. The mean values of half-life and apparent volume of distribution at steady-state in each group were similar. These results indicate that prednisolone must be used with caution in the treatment of inflammatory diseases because of higher free concentrations of the steroid.  相似文献

Purpose:To analyze the reasons for delay in cataract surgery in patients with advanced cataracts during the COVID-19 pandemic.Methods:This was a prospective, cross-sectional, multicenter questionnaire study which included patients with mature cataract, nuclear sclerotic cataract grade IV, and cataracts with best corrected visual acuity (BCVA) <5/60, during the COVID-19 pandemic from December 2020 to April 2021. Reasons for delay in presentation to the hospital were analyzed.Results:One thousand four hundred seventy two patients were recruited with advanced cataracts. Absence of ophthalmic care nearby (44.2%), lack of awareness regarding elective surgeries (42.6%), lack of public transportation (37%), fear of contracting COVID-19 (23.4%), and waiting for outreach camps (20.4%) were found to be the reasons behind the delay in cataract surgery. 53.7% of the patients had worsening of defective vision and 55.3% of them had difficulty in carrying out activities of family living. 30.8% of the patients faced difficulty in commuting and 8.4% of the patients suffered a fall during this pandemic due to worsening of the visual acuity.Conclusion:The lockdown imposed during the pandemic has created a significant backlog of patients who are progressing to advanced cataracts due to lack of ophthalmic care nearby, lack of awareness regarding elective surgeries, lack of public transportation, and no outreach camps. Proactive measures to deal with this backlog are of utmost need to prevent blindness due to cataract.  相似文献

Corrosion is frequently viewed as a catastrophic and unavoidable disaster in marine applications. Every year, a huge cost is incurred on the maintenance and repair of corrosion-affected equipment and machinery. In the marine environment, as-cast nickel–aluminium bronze (NAB) is susceptible to selective phase corrosion. To solve this problem, chromium-reinforced nickel–aluminium bronze was fabricated using the friction stir process (FSP) with improved microstructures and surface properties. A slurry erosion–corrosion test on as-cast and FSPed composites demonstrated that the developed surfaced composite has lower erosion and corrosion rates than the as-cast NAB alloy. The erosion–corrosion rate increased with a decrease in the impact angle from 90° to 30° for both as-cast NAB and prepared composites, exhibiting a shear mode of erosion. The specimens at impact angle 30° experienced more pitting action and higher mass loss compared with those at impact angle 90°. Due to increases in the mechanical properties, the FS-processed composite showed higher erosion resistance than the as-cast NAB alloy. Furthermore, corrosion behaviour was also studied via the static immersion corrosion test and electrochemical measurements under 3.5 wt.% NaCl solution. In a static immersion corrosion test, the FSPed composite outperformed the as-cast NAB composite by a wide margin. The FSPed composite also demonstrated a reduced electrochemical corrosion rate, as revealed by the polarization curve and electrochemical impedance spectroscopic (EIS) data. This reduced rate is attributed to the formation of a Cr oxide film over its surface in the corrosive environment.  相似文献
