Introduction: Surgery in patients with head and neck cancers is frequently complicated by multiple stages of procedure that includes significant surgical removal of all or part of an organ with cancer, tissue reconstruction, and extensive neck dissection. Postoperative wound infections, termed ‘surgical site infections’ (SSIs) are a significant impediment to head-and-neck cancer surgery and recovery, and need to be addressed.
Areas covered: Approximately 10–45% of patients undergoing head-and-neck cancers surgery develop SSIs. SSIs can lead to delayed wound healing, increased morbidity and mortality as well as costs. Consequently, SSIs need to be avoided where possible, as even the surgery itself impacts on patients’ subsequent activities and their quality of life, which is exacerbated by SSIs. Several risk factors for SSIs need to be considered to reduce future rates, and care is also needed in the selection and duration of antibiotic prophylaxis.
Expert commentary: Head and neck surgeons should give personalized care especially to patients at high risk of SSIs. Such patients include those who have had chemoradiotherapy and need reconstructive surgery, and patients from lower and middle-income countries and from poorer communities in high income countries, who often have high levels of co-morbidity because of resource constraints. 相似文献
The nuclear content, area and perimeter of the nucleus of lymphocytes in the C.S.F. were determined quantitatively by means of image analysis technique. 26 cases of central nervous system lymphocytic leukemia (CNLL), and 8 suspected cases were studied, other 56 cases who did not have leukemic and neoplastic diseases and had normal C.S.F. lymphocytes were taken as a control. Our data showed that all the mean nuclear content (MNC), mean nuclear area (MNA), mean nuclear perimeter (MNP), and the maximum and minimum nuclear contents of the 2 groups of former patients were obviously higher than those of the contral (P less than 0.01). These results presented suggestion that the image analysis technique can be used for differentiating the leukemic lymphocyte from normal one especially in suspected cases, and thus the diagnosis of CNLL might be improved. 相似文献
Immigrants, and especially those recently arrived in host countries, are a high-risk group for tuberculosis. In 2006 in France, nearly half of the new cases of tuberculosis were reported in this group. The incidence rate among people born abroad was about seven times higher than that observed in people born in France (38.9/105 vs 5.2/105). The incidence was also the highest in districts with a high proportion of socioeconomically vulnerable population, such as Paris and greater Paris area. The characteristics of French born and non-French born cases differed. Patients born abroad were less likely to have pulmonary tuberculosis (68.2% vs 78.2; p < 0.001) and were more likely to live in institutions (such as migrant shelters) and to harbor multiresistant tuberculosis strains. Background: The overexpression of p53 has been found to be correlated with prognosis of some carcinomas, including gastric cancer, but
no studies have reported on its relationship to the location of gastric cancer. In the present study, we compared the p53
expression of proximal and distal gastric cancer concerning histopathology and prognosis.
Methods: A total of 170 tumors in the patients with proximal (80 cases) and distal (90 cases) gastric cancer were studied by immunohistochemical
methods.
Results: p53 immunopositivity was detected in 28.8% of all tumors. The p53-positive expression in proximal gastric cancer was higher
than in distal gastric cancer (38.8% vs. 20.0%, p<0.05). A 5-year survival analysis showed that there is no significant difference
between tumors that are p53 positive and p53 negative. No correlation was found between p53 expression and histopathology
of gastric cancer.
Conclusion: p53 nuclear staining is not useful as a prognostic indicator or as a parameter in gastric cancer. 相似文献
