目的探讨腹腔镜内环结扎术治疗儿童复发性腹股沟斜疝的优势及手术经验。 方法选择2013年1月至2018年1月,广州市妇女儿童医疗中心收治的34例复发性腹股沟斜疝患儿为研究对象。回顾性分析其临床病例资料。根据本次入院采取的手术治疗方式,将其分为腹腔镜组(n=19,采用腹腔镜内环结扎术)和开放手术组(n=15,采用疝囊高位结扎术)。采用独立样本t检验及χ2检验,对2组患儿一般临床资料、术中及术后情况等,进行统计学分析。本研究符合2013年修订的《世界医学协会赫尔辛基宣言》要求,所有患儿监护人均于术前签署手术治疗知情同意书。 结果①开放手术组患儿术后,3例出现阴囊血肿,1例出现医源性隐睾。2组患儿术后均未发生伤口感染、睾丸萎缩、阴囊积液及再次复发等其他并发症。②2组患儿中,男性患儿所占比例、月龄及初次手术为腹腔镜手术所占比例比较,差异均无统计学意义(P>0.05)。腹腔镜组患儿的手术时间、术中出血量、住院时间及术后并发症发生率分别为(30.0±10.7) min、(1.0±0.3) mL、(1.2±0.4) d、0,均显著短于或少于开放手术组的(44.7±24.5) min、(2.3±1.3) mL、(3.3±1.0) d、26.7%;其住院费用为(7 547±820)元,则显著高于开放手术组的(5 488±1 218)元,并且差异均有统计学意义(t=-2.347、P=0.044,t=-3.859、P=0.002,t=-8.171、P<0.001,P=0.029,t=5.580、P<0.001)。 结论腹腔镜内环结扎术治疗儿童复发性腹股沟斜疝,具有安全有效、术中情况良好、术后并发症少等优势,但住院费用相对偏高。  相似文献

Objective To evaluate the efficacy and safety of lenalidomide plus dexamethasone (LD) in patients with proliferative glomerulonephritis with monoclonal immunoglobulin deposits (PGNMID). Methods The clinicopathological data of PGNMID patients who were treated with LD protocol from January 2010 to October 2019 were retrospectively analyzed. Results All of 6 patients received LD treatment for≥3 months after renal biopsy in Jinling Hospital. During the follow-up period of 6 to 19 months, 3 patients achieved renal remission, and the renal remission rate was 50%(3/6). Light microscopy showed membranoproliferative glomerulonephritis and immunofluorescence showed single kappa type IgG3 was deposited in the mesangial region and the vascular loop. Before taking LD scheme, the median urinary protein were 7.76(1.27, 14.57) g/24 h, the median serum creatinine was 118.5(70.7, 289.1) μmol/L, and the median albumin was 34.5(22.4, 37.5) g/L. The concentration of serum free kappa and lambda light chain was increased in 5 patients, but the serum free light chain ratio was normal. Hypocomplementemia was detected in two cases. Six patients underwent bone marrow flow cytometry, and 2 patients had elevated monoclonal plasma cells, accounting for 0.7% and 0.5%, respectively. Immunofixation electrophoresis suggested that 1 patient had positive serum M protein for kappa type IgG3. At the last follow-up, median urine protein was 3.33(0.33, 11.23) g/24 h, median serum creatinine was 108.7(80.4, 160.9) μmol/L, and median albumin was 35.9(24.5, 45.6) g/L. The concentration of serum free light chain in 4 patients from 5 patients with elevated serum free light chain was lower than that before taking the drug. Decreased level of serum complement in two cases returned to normal after treatment. The M spike did not turn negative during the follow-up in one patient. Adverse events included anemia, neutropenia, limb numbness and upper respiratory tract infection. Conclusion This study reports for the first time that LD protocol may be effective in treating PGNMID, but more attention should be paid to the hematological adverse events of lenalidomide.  相似文献

Needle-free injection (NFI), as an alternative drug delivery strategy, owns great potential. It is able to reduce complaints about needle phobia and avoid the occurring of accidental needle stick injuries. The nozzle diameter is inherently important in determining the injection dose, injection depth, and pain associated with NFIs. In this work, needle-free injectors with nozzle diameters of 0.17, 0.20, 0.30, 0.40, and 0.50 mm were studied in the simulation and experiment. This article optimizes the mathematical model for spring-powered NFI by considering the hydraulic loss due to the abrupt change in the nozzle exit area and the friction force between the piston and ampoule. We explore the dispersion pattern in gels with different nozzle diameters. Mice insulin injection was conducted to investigate the pharmacological effect of different injection methods. The experimental results show that there is the best dispersion effect and available injection depth while the nozzle diameter is 0.30 mm, which is in agreement with the result predicted by the mathematical model. Also, there is a satisfactory pharmacological effect on the mice insulin injection under the same injection condition. Undoubtedly, the mathematical model is capable of predicting the suitable nozzle diameter under the given conditions.  相似文献

The chemical constituents of 95% EtOH extract of yacon leaves were separated to yield two new sesquiterpene lactones, together with three known compounds. The two new compounds were characterized to be 8β-angeloyloxy-13-methoxyl-11, 13-dihydromelampolid-14-oic acid methyl ester (1) and 8β-(3-methylbut-2-enoyl) oxy-13-methoxyl-11, 13-dihydromelampolid-14-oic acid methyl ester (2) on the basis of NMR spectra, HR-MS and other spectroscopic methods. The cytotoxicity of compounds 1–5 were evaluated on human hepatoma cell Bel-7402 and all the compounds showed moderate cytotoxicity.  相似文献

Cancer metastasis is one of the biggest challenges in cancer treatments since it increases the likelihood that a patient will die from the disease. Therefore, the availability of techniques for the early detection and quantification of tumors is very important. We have prepared cyanine 7.5 NHS ester (Cy7.5) and folic acid (FA) conjugated biodegradable mesoporous silica nanoparticles (bMSN@Cy7.5-FA NPs) (~100 nm) for visualizing tumors in vivo. The fluorescence spectra revealed that the emission peak of bMSN@Cy7.5-FA NPs had a red-shift of 1 nm. Confocal immunofluorescent images showed that bMSN@Cy7.5-FA NPs had an excellent targeting ability for visualizing cancer cells. In vivo fluorescence imaging has been conducted using an orthotopic model for pancreatic cancer within 48 h, and the fluorescence intensity reached a maximum at a post injection time-point of 12 h, which demonstrated that the use of bMSN@Cy7.5-FA NPs provides an excellent imaging platform for tumor precision therapy in mice.  相似文献

目的研究顺铂(cisplatin CDDP)和5-氟脲嘧啶(5-fluorouracil,5-FU)联合术前诱导化疗对口腔舌鳞癌的抗瘤作用.方法 对36例口腔舌鳞癌患者采用顺铂和氟脲嘧啶联合术前诱导化疗,总计量CDDP 80 mg·m-2、5-FU 2.0 g,在4 d内平均给予.结果以肿瘤体积缩小50%以上或症状明显缓解为有效,总有效率为66.7%,化疗的主要毒副反应为胃肠反应91.7%.结论 CDDP和5-FU联合诱导化疗方案对舌癌能够取得明显的近期疗效.  相似文献

This study used RNA interference (RNAi) to explore the effect of NO and inducible nitric oxide synthase (iNOS) on apoptosis and proliferation in the tongue squamous carcinoma cell line Tca8113. Tca8113 cells were transfected with the plasmid pGenesil-1, which expresses iNOS short hairpin RNA (shRNA), or the negative control plasmid pSilencer-HK, and the transfected cells were compared with untransfected cells. The expression of iNOS was detected by histochemistry, and apoptosis was detected by flow cytometry. The expression of iNOS was significantly lower in the pSilencer-iNOS group than in the pSilencer-HK and empty control groups. The apoptosis rate was significantly higher in the pSilencer-iNOS group than in the pSilencer-HK and empty control groups. Growth monitoring showed that proliferation was also inhibited in cells transfected with pSilencer-iNOS. RNAi gene silencing decreased iNOS gene expression, induced apoptosis, and suppressed proliferation in Tca8113 cells.  相似文献
