Ischemic stroke occurs in 0.2-0.4% of patients undergoing left heart catheterization, and is responsible for 5-10% of the mortality associated with the procedure. The main predisposing factors for this complication are female gender, complex atherosclerotic plaques in the ascending aorta, and peripheral arterial disease. The possibility of timely intervention with reperfusion therapy supports close clinical monitoring during the immediate post-catheterization period. The cardiologist should be familiar with the various types of stroke reperfusion therapy and its indications according to the time interval between catheterization and the stroke. Background Reliable closure of the translumenal incision is one of the main challenges facing natural orifice translumenal endoscopic
surgery (NOTES). This study aimed to evaluate the use of an automated flexible stapling device (SurgASSIST) for closure of
the gastrotomy incision in a porcine model.
Methods A double-channel gastroscope was advanced into the stomach. A gastric wall incision was made, and the endoscope was advanced
into the peritoneal cavity. After peritoneoscopy, the endoscope was withdrawn into the stomach. The SurgASSIST stapler was
advanced orally into the stomach. The gastrotomy edges were positioned between the opened stapler arms using two endoscopic
grasping forceps. Stapler loads with and without a cutting blade were used for gastric closure. After firing of the stapler
to close the gastric wall incision, x-ray with contrast was performed to assess for gastric leakage. At the end of the procedure,
the animals were killed for a study of closure adequacy.
Results Four acute animal experiments were performed. The delivery and positioning of the stapler were achieved, with technical difficulties
mostly due to a short working length (60 cm) of the device. Firing of the staple delivered four rows of staples. Postmortem
examination of pig 1 (when a cutting blade was used) demonstrated full-thickness closure of the gastric wall incision, but
the cutting blade caused a transmural hole right at the end of the staple line. For this reason, we stopped using stapler
loads with a cutting blade. In the three remaining animals (pigs 2–4), we were able to achieve a full-thickness closure of
the gastric wall incision without any complications.
Conclusions The flexible stapling device may provide a simple and reliable technique for lumenal closure after NOTES procedures. Further
survival studies are currently under way to evaluate the long-term efficacy of gastric closure with the stapler after intraperitoneal
interventions.

The chondroid syringoma is a rare benign tumor, also called mixed cutaneous tumor by
the presence of epithelial and mesenchymal components, consisting of sweat elements
in cartilaginous, collagenous, myxoid or osseous stroma, among others. It mainly
affects middle-aged men and is characterized by asymptomatic and slowgrowing, dermal
or subcutaneous nodules. The most common locations are the head and neck. It is rare
on the extremities. There are reports of malignant variants predominantly in women,
located on the extremities. We report a case of a female patient with a lesion on the

In humans, type I interferon (IFN) is a family of 17 cytokines, among which the alpha subtypes and the beta subtype are differentially expressed. It has been suggested that IFN-beta activates a specific signaling cascade in addition to those activated by all type I IFNs. Nevertheless, no true biological relevance for a differential activity of alpha and beta IFN subtypes has been identified so far. Because type I IFNs are critical for the regulation of osteoclastogenesis in mice, we have compared the effect of IFN-alpha2 and IFN-beta on the differentiation of human monocytes into osteoclasts. Primary monocytes undergoing osteoclastic differentiation are highly and equally sensitive to both alpha2 and beta IFNs as determined by measuring the induction levels of several IFN-stimulated genes. However, IFN-beta was 100-fold more potent than the alpha2 subtype at inhibiting osteoclastogenesis. Expression profiling of the genes differentially regulated by IFN-alpha2 and IFN-beta in this cellular system revealed the chemokine CXCL11 as the only IFN-induced gene differentially up-regulated by IFN-beta. We show that recombinant CXCL11 by itself inhibits osteoclastic differentiation. These results indicate that autocrine-acting CXCL11 mediates, at least in part, the regulations of osteoclastogenesis by type I IFNs. 相似文献

Leishmania (Viannia) braziliensis, a protozoan parasite widespread in the New World, is responsible for the infection of different mammal orders, including humans. This species is considered to be a major etiological agent of American cutaneous leishmaniasis. A proteomic study was carried out to identify proteins expressed by L. (V.) braziliensis. One hundred and one spots representing 75 protein entries were identified by MALDI-TOF-TOF. Isoelectric point values estimated by gel electrophoresis matched closely with predicted values, although some discrepancies existed suggesting that post-translational protein modifications may be common in L. braziliensis. Moreover, 20 hypothetical proteins were experimentally identified. Identified proteins were classified into 15 groups according to biological process. Among the proteins identified, approximately 40% have not been previously reported in a proteomic map of Leishmania. In addition, a number of potential virulence factors and drug targets were identified in this protein map, including some proteins associated with the metastatic phenotype. This study describes the first compilation of a proteomic reference map for L. braziliensis (pI 4-7, M(r) 10-130 kDa) and provides a very useful tool for comparative studies of strains isolated from patients presenting different clinical manifestations of leishmaniasis as well as a potential tool to identify markers for clinical diagnosis, therapeutics, and prognosis. 相似文献
