OBJECTIVE: This study was undertaken to determine the relationship of umbilical cord pH and base excess (BE) values to adverse neonatal outcomes for a large tertiary hospital population delivering at term.Study design The perinatal/neonatal database of St. Joseph's Health Care, London, Canada, was used to obtain the umbilical cord pH and BE values, incidence of adverse neonatal outcomes, and patient demographics for all term (>/=37 weeks' gestation), singleton, liveborn infants with no major anomalies delivering between November 1995 and March 2002 (n=20,456). Statistical analyses included chi(2) analysis, logistic regression models to develop odds ratios and creation of receiver operating characteristic (ROC) curves with area under curve (AUC) calculations. RESULTS: Umbilical vein and artery pH and BE values for this tertiary care population averaged 7.33 +/- 0.06 and 7.24 +/- 0.07, and -4.5 +/- 2.4 and -5.6 +/- 3.0 mmol/L, respectively. Apgar less than 7 at 5 minutes, neonatal intensive care unit (NICU) admission, and assisted neonatal ventilation had significant inverse relationships with both umbilical artery and umbilical vein pH and BE (all P < .0001), with marginal increases in the incidences of these outcomes beginning with cord blood values close to the mean, and more substantial increases with cord values less than 1 or 2 SD below the mean, depending on the outcome studied. The ROC AUC for all these relationships were significant (P < .001) ranging from 0.76 to 0.79 when predicting Apgar less than 7 at 5 minutes to 0.68 to 0.70 when predicting NICU admission, and with cutoff cord blood values at which sensitivity and specificity were maximized again close to mean values. For each of these neonatal outcomes, the relation to cord blood values was similar with little difference in the data analysis whether using pH or BE values, and whether from the umbilical artery or vein. CONCLUSION: There is a progression of risk in term infants for Apgar less than 7 at 5 minutes, NICU admission, and need for assisted ventilation with worsening acidosis at birth, which begins with cord blood values close to mean values indicating a higher threshold for associated acidemia with these outcomes than is seen for more severe neonatal outcomes.  相似文献

Purpose

To compare the fetal outcome of preterm breech infants delivered vaginally (VD) or by cesarean section (CS).

Methods

A monocentric, retrospective consecutive case series of preterm breech deliveries between 24–37 gestational weeks over 10 years from 1/2000 to 12/2009 was performed in a perinatal care center (Level 1) at the University Clinic of Salzburg, Austria. Data from hospital database were statistically analyzed and compared regarding birth weight, head circumference, parity, transfer rate to neonatal intensive care unit (NICU), arterial and venous cord blood pH and base excess (BE), arterial cord blood pH ≤ 7.10 and BE ≤ ?11. Special focus was on fetal outcome of elective CS preterm breech deliveries with a non-urgent medical indication compared to VD.

Results

Among 22.115 deliveries, there were 346 live-born preterm singletons and twins in breech presentation (1.56 %), born between 24 + 0 and 37 + 0 gestational weeks. 180 CS and 36 vaginally delivered preterm breech infants were statistically evaluated. On comparing CS vs. VD for premature breech singletons, arterial cord blood pH and BE were lower in the VD group. VD twins had a lower arterial cord blood pH than CS twins. All other parameters were comparable. In preterm breech singletons with non-urgent CS, a statistical analysis was not possible due to small numbers. The VD twin group revealed lower values in birth weight, head circumference, arterial cord blood pH and BE, but no significant difference in venous cord blood pH and BE and transfer rate to NICU.

Conclusions

Although general recommendations regarding a superior mode of delivery for improved fetal outcome of preterm breech infants cannot be given, these data do not support a policy of routine CS.  相似文献

OBJECTIVE: Our purpose was to examine regulatory linkages between fetal oxygenation and fetal and placental growth. We determined umbilical cord PO (2) and oxygen saturation, fractional oxygen extraction, and birth to placental weight ratio values in relation to size at birth for a large tertiary hospital population delivering at term. STUDY DESIGN: The computerized perinatal database of St Joseph's Health Care London, London, Ontario, was used to obtain the umbilical cord gases, pH, birth weight, placental weight, and other selected information for all term, singleton, liveborn infants between January 1990 and December 1999 (N = 27,043). Oxygen saturation values were calculated from the umbilical cord PO(2) and pH data with a previously derived empirical equation; fractional oxygen extraction values were calculated from the umbilical cord oxygen saturation data. Size at birth was divided into the following 5 birth weight categories using neonatal growth standards: fetal growth restriction, <3%; borderline fetal growth restriction, >or=3% and <10%; appropriate for gestational age, >or=10% and 90% and 97%. RESULTS: Infants in the borderline fetal growth restriction and fetal growth restriction groups had umbilical vein and artery PO(2) and oxygen saturation values that were stepwise lower than respective values for infants in the appropriate for gestational age group. Conversely, infants in the borderline large for gestational age and large for gestational age groups had umbilical vein PO(2) and oxygen saturation values that were stepwise higher than respective appropriate for gestational age group values; infants in these groups showed no change in arterial PO (2) and oxygen saturation values. Therefore infants in the borderline fetal growth restriction and fetal growth restriction groups had fractional oxygen extraction values that were stepwise higher than the appropriate for gestational age group value, whereas values for infants in the borderline large for gestational age and large for gestational age groups remained unchanged. Birth weight was disproportional to placental weight for infants in the borderline fetal growth restriction and fetal growth restriction groups when compared with that of the infants in the appropriate for gestational age group, with the birth to placental weight ratio values stepwise decreased. Conversely, birth weight was proportional to placental weight for infants in the borderline large for gestational age and large for gestational age groups with the birth to placental weight ratio values thus unchanged when compared with that of the infants in the appropriate for gestational age group. CONCLUSION: We conclude that fetal oxygenation is related to size at birth across the entire range of birth weights as studied at term from macrosomic to growth-restricted infants; this conclusion supports oxygen as a primary determinant of fetal growth. However, there are differences in the linkage between fetal oxygenation and metabolic rate or growth for these cohort groups that may relate to underlying etiologic processes.  相似文献

Purpose: To examine prematurity-associated neonatal outcomes in early preterm infants with metabolic acidemia compared to those without such acidemia.

Methods: We performed a retrospective cohort analysis to assess the impact of metabolic acidemia on prematurity-associated complications in a large cohort of singleton live-born infants with complete umbilical cord gas analyses delivered between 24 0/7 and 33 6/7 weeks. Metabolic acidemia was defined as an umbilical artery pH less than 7.0 plus a base deficit of 12?mmol/L or greater. Outcomes were adjusted for gestational age using logistic regression.

Results: Between 1 January 1988 and 31 December 2014, 6970 singleton early preterm infants were delivered at our hospital, of which 126 (1.8%) had metabolic acidemia. Neonatal mortality as well as prematurity-associated morbidities were significantly increased in the presence of metabolic acidemia. Included were ventilator requirement (73% versus 36%, p?p?p?=?0.036), and neonatal death (13% versus 4%, p?Conclusion: Metabolic acidemia significantly increases the risks related to prematurity in infants delivered prior to 34 weeks’ gestation.  相似文献

ObjectiveTo investigate the use of Kangaroo Mother Care (KMC) and its association with breastfeeding at 1 to 6 months of corrected age in mothers of very preterm (VPT) and preterm (PT) infants.DesignProspective longitudinal study.SettingNeonatal Intensive Care Units in four counties in Sweden.ParticipantsThe study included 103 VPT (<32 gestational weeks) and 197 PT (32‐36 gestational weeks) singleton infants and their mothers.MethodsData on KMC, measured in duration of skin‐to‐skin contact/day during all days admitted to a neonatal unit, were collected using self‐reports from the parents. Data on breastfeeding were obtained by telephone interviews.ResultsVPT dyads that breastfed at 1, 2, 5, and 6 months had spent more time in KMC per day than those not breastfeeding at these times. A trend toward significance was noted at 3 and 4 months. In the PT dyads no statistically significant differences were found in the amount of KMC per day between those dyads that breastfed and those that did not.ConclusionsThis study shows the importance of KMC during hospital stay for breastfeeding duration in VPT dyads. Hence, KMC has empowering effects on the process of breastfeeding, especially in those dyads with the smallest and most vulnerable infants.  相似文献

目的:探讨妊娠期糖尿病(gestational diabetes mellitus,GDM)新生儿脐血血气分析情况及可能影响因素。方法:选择2015年6-8月同济大学附属第一妇婴保健院分娩的单胎GDM孕妇(GDM组)152例和单胎正常孕妇(对照组)152例。152例GDM孕妇中,阴道分娩组69例,剖宫产组83例;非胰岛素治疗组141例,胰岛素治疗组11例。比较2组新生儿脐血血气分析和结局,以及不同分娩方式和治疗方式对GDM新生儿各项指标的影响。结果:GDM组脐静脉血碱剩余(BE)值较正常对照组低,差异有统计学意义(t=2.702,P=0.007)。不同分娩方式GDM新生儿脐动脉血pH值、脐静脉血pH值、脐动脉血BE值、脐静脉血BE值、脐静脉二氧化碳分压[p(CO2)]比较,差异有统计学意义(均P<0.05);但2组新生儿结局差异无统计学意义(均P>0.05)。是否采用胰岛素治疗的GDM新生儿脐血血气分析和结局比较差异无统计学意义(均P>0.05)。结论:血糖控制良好的GDM新生儿脐血血气分析与正常妊娠者类似,分娩方式及是否采用胰岛素治疗均不影响GDM新生儿结局。  相似文献

Objective: Randomized controlled trials have demonstrated that delayed umbilical cord clamping (DCC) in preterm infants results in improved neonatal outcomes, including increased hematocrit, and decreased rates of intraventricular hemorrhage (IVH) and packed red blood cell transfusions. We hypothesized that implementation of a DCC policy in preterm infants would result in similarly improved outcomes, despite initial clinician resistance.

Study design: A DCC policy (30–60?s) for singleton infants <35 weeks gestation was implemented in September 2011. We conducted a pre-test/post-test analysis of neonatal outcomes among singletons delivered between 24 0/7 weeks and 34 6/7 weeks gestation from 2009 to 2013 (2 years pre-implementation and 2 years post-implementation). The primary outcomes were rates of policy compliance and four neonatal outcomes.

Results: Despite multiple routes of policy dissemination, DCC was attempted in only 49% of the deliveries. In spite of this, infants delivered post-policy implementation (n?=?196) had a significant decrease in IVH, significant increase in initial hematocrits, and improved temperatures compared with infants delivered pre-implementation (n?=?204).

Conclusion: After implementation of a DCC policy, preterm singleton infants had improved temperatures, increased hematocrits and a decreased prevalence of IVH without significant differences in adverse outcomes, suggesting that the benefits of DCC outweighed the risks.  相似文献

Imprecise diagnosis of birth asphyxia coupled with uncertainties about causal factors for neurologic abnormalities in the newborn have greatly fueled the current litigation crisis in obstetrics. Our goal was to more precisely define birth asphyxia based on fetal condition as measured by umbilical artery blood pH, Apgar scores, and neurologic condition of newborns. We selected for study 2738 patients with singleton pregnancies with cephalic presentations who were delivered of infants at term to avoid complications such as prematurity, which may affect infant outcome independent of birth condition. The basis for study of these particular patients were defined criteria for high risk and an indicated arterial cord pH value. A total of five infants demonstrated cerebral dysfunction as evidenced by seizures during the neonatal period. Infection was linked to seizures in three of these infants; one infant had neonatal asphyxia and only one infant's clinical course could be attributed solely to birth events (uterine rupture). Stratification of umbilical artery blood pH values, Apgar scores, and combinations of these dependent variables in relation to newborn clinical outcomes revealed that infants must be severely depressed at delivery before birth asphyxia can be reliably diagnosed. Such depression includes Apgar scores less than or equal to 3 at 1 and 5 minutes plus umbilical artery pH values less than 7.00.  相似文献

OBJECTIVE: To correlate umbilical blood gas variables with neonatal seizures in neonates with significant acidosis at birth (pH < or = 7.1). METHODS: We reviewed the maternal and neonatal charts of 238 patients at a gestational age of 32 weeks or more with cord gases done at delivery and an umbilical artery pH of 7.1 or less. All infants transferred to the neonatal intensive care unit were studied, and those with neonatal seizures secondary to hypoxic ischemic encephalopathy were identified. We used the perinatal outcome of early neonatal seizures secondary to hypoxic ischemic encephalopathy to divide the patients into two groups. The relationship between the umbilical artery parameters of pH, base deficit, partial oxygen pressure (pO(2)), partial carbon dioxide pressure (pCO(2)), and the neonatal outcome of seizures were determined with Student t tests and multiple logistic regression analysis. RESULTS: Umbilical artery base deficit, pO(2), and pCO(2) were significantly elevated in newborns who had seizures, whereas cord pH was decreased. Using multiple regression analysis the variable neonatal seizure was predicted only by low umbilical artery pH. A pH of less than 7 was more sensitive (73.8%) than a base excess of -16 (52.5%) in predicting the development of neonatal seizures. CONCLUSION: Our data suggest that severe fetal acidemia identified by a pH less than 7.0 was the most important umbilical blood gas variable for predicting early onset of neonatal seizures.  相似文献

The aim of the present study was to assess if there was an association between low catecholamine levels at birth in the premature infant and the development of TTN. Blood samples were collected at delivery from the umbilical artery of all preterm infants with a gestational age less than 36 weeks for determination of pH and catecholamine levels (noradrenaline and adrenaline). Amongst non-asphyxiated infants only, cord pH greater than 7.25 and/or Apgar score greater than 7 at five minutes, catecholamine levels were compared between the 10 infants who developed transient tachypnoea of the newborn (TTN) and 13 controls of a similar gestational age range (31-35 weeks) who developed no respiratory distress in the neonatal period. Infants who developed TTN were more often delivered without labour, 8 of 10 compared to 2 of 13 controls (p less than 0.01). There were no significant differences in adrenaline levels between the two groups. Noradrenaline levels, however, were significantly lower in the infants who developed TTN, being a median of 3.1 nmol/l (range 1.07-5.85 nmol/l) compared to a median of 6.4 nmol/l (range 2.38-22.83) in the controls (p less than 0.01). Infants who were delivered following labour had significantly elevated noradrenaline levels compared those delivered without labour (elective delivery) (p less than 0.05). These results suggest that low noradrenaline levels in preterm infants may explain the association in this group of TTN and "elective" delivery.  相似文献

Objective: The objective of this study is to assess retrospectively the predictive value of fetal scalp pH and base excess (BE) for fetal acidosis and poor neonatal outcome in term, low-risk, spontaneous deliveries with suspicious or pathological intrapartum cardiotocography (CTG) tracings.

Methods: Umbilical artery pH and BE values obtained immediately after delivery and Apgar score were the outcomes under consideration. Statistics included receiver-operating characteristic curve (ROC) and multiple logistic regression analysis.

Results: Four hundred and six deliveries were included in the study. Scalp pH?p?p?Conclusion: Our study supports the consideration of both scalp pH and BE values, when fetal blood sampling (FBS) is used.  相似文献

The purpose of this clinical study is to investigate the diagnostic value of plasma volume (PV), nonstress test (NST), contraction stress test (CST), and umbilical artery Doppler (UAD) in detecting fetal compromise in 81 patients (83 fetuses) at risk for fetal growth retardation. Neither PV nor UAD studies were used in the clinical management. There were two stillbirths and three neonatal deaths for a perinatal mortality of 6%. Twenty-seven infants (32.5%) were small for gestational age (SGA), seven (8.6%) had cord pH 7.20 or less, and five (6.2%) had 5-minute Apgar scores less than 7. Overall, PV had the highest sensitivity and NST the highest specificity regarding delivery of SGA infants. The positive and negative predictive values for infants with low cord pH and low Apgar scores were similar among the various tests. There were nine fetuses with zero or reverse diastolic flow: seven were SGA (four perinatal deaths) and all of them had both nonreactive NST and positive CST. The other two infants were appropriate for gestational age with all other tests being normal. Antepartum fetal heart rate testing appears to be similar to other tests in predicting poor fetal outcome in high-risk pregnancies.  相似文献

It is becoming increasingly apparent that Apgar scores are generally lower in otherwise uncomplicated preterm newborns than in term newborns. However, there is little information regarding normal values for umbilical artery blood gas measurements in the preterm infant. The present study included 77 otherwise uncomplicated preterm infants and 1292 uncomplicated term infants. Although preterm infants did have significantly lower 1- and 5-minute Apgar scores, there was no significant difference in the frequency of acidemia (umbilical artery pH below 7.20). The mean pH was 7.29 in preterm infants and 7.28 in term infants. Mean values for pCO2, pO2, HCO3, and base deficit were similar in each group. There were no significant differences in umbilical artery blood gas values in the 77 preterm infants according to birth weight groups. Umbilical cord blood acid-base determination may prove a useful adjunct in assessing the condition of the newborn preterm infant.  相似文献

OBJECTIVE: This study was undertaken to evaluate the association between umbilical cord interleukin-6 (IL-6) levels and neonatal morbidity in infants born at less than 32 weeks' gestation. STUDY DESIGN: Umbilical cord plasma IL-6 levels and neonatal outcomes were assessed in 309 infants born between 24 weeks and 0 days' and 31 weeks and 6 days' gestation. RESULTS: Mean IL-6 levels were higher in spontaneous (n = 193, 355 +/- 1822 pg/mL) compared with indicated preterm births (n = 116, 37 +/- 223 pg/mL, P < .0001). Adjusting for gestational age, a progressive relationship was noted between increasing IL-6 levels and increased risk of neonatal systemic inflammatory response syndrome (SIRS). IL-6 levels beyond the 90th percentile (> or =516.6 pg/mL) were also significantly associated with periventricular leukomalacia (PVL; odds ratio [OR] 15, 95% CI 2-149) and necrotizing enterocolitis (NEC; OR 6, 95% CI 1.1-33). In the multivariate analysis, an IL-6 level 107.7 pg/mL or greater (determined by receiver operating curve analysis) remained a significant independent risk factor for PVL (OR 30.3, 95% CI 4.5-203.6). CONCLUSION: Umbilical cord IL-6 levels are higher in preterm infants born after spontaneous preterm labor or premature rupture of membranes. Elevated IL-6 levels are associated with an increased risk for SIRS, PVL, and NEC in infants born at less than 32 weeks' gestation.  相似文献

Apgar scores are used routinely to assess early neonatal status, but are less accurate in the preterm neonate because of developmental immaturity. Attention has been directed to umbilical cord gases as a method of neonatal evaluation. Using a retrospective chart review of all viable preterm births (24-36 weeks' gestation) between January 1986 and December 1989, we tabulated the umbilical cord gas indices of these infants. Fetuses with lethal congenital anomalies and those with abnormal heart rate tracings on admission were excluded from the data base, leaving 1872 infants. Cord arterial blood gas values were available for analysis in 74.4% of cases and cord venous gas values in 81.8%. The mean (+/- standard deviation [SD]) arterial and venous umbilical cord blood gas values for the preterm infants, were, respectively: pH, 7.26 +/- 0.08 and 7.33 +/- 0.07; oxygen pressure, 19.0 +/- 7.9 and 29.2 +/- 9.7 mmHg; carbon dioxide pressure, 53.0 +/- 10.0 and 43.4 +/- 8.3 mmHg; bicarbonate, 24.0 +/- 2.3 and 22.8 +/- 2.1 mEq/L; and base excess, -3.2 +/- 2.9 and -2.6 +/- 2.5 mEq/L. Acidemia was defined statistically as 2 SDs or more below the population mean. The incidence of 5-minute Apgar scores below 7 in the preterm infants was 8.5% and within this group, 17.8% were acidemic (arterial pH 7.10 or lower). More than 82% of neonates with 5-minute Apgar scores less than 7 had normal umbilical cord blood gases. The absence of fetal pulmonary maturity in patients with preterm premature rupture of the membranes (PPROM) is often considered an indication for conservative management. The purpose of this study was to examine the value of biochemical pulmonary maturity assessment for the prediction of neonatal outcome in patients with PPROM between 32 and 34 weeks' gestation. Pregnancies complicated by PPROM at 32 to 34 weeks' gestation that delivered from January 1995 to May 2000 and had biochemical pulmonary maturity assessment were reviewed. Patients with medical disorders, multiple gestations, fetal growth restriction or structural anomalies, or evidence of intra-amniotic infection were excluded. Neonatal outcome measures were compared between patients with mature and immature pulmonary indices. During this time period, 244 patients with PPROM at 32-34 weeks' gestation were delivered; 78 patients met inclusion criteria (n = 41 patients with mature indices and n = 37 patients with immature indices). There were no cases of perinatal death or sepsis. There was no difference in major neonatal morbidities including need for mechanical ventilation, grade 2 or 3 necrotizing enterocolitis, grade 3 or 4 intraventricular hemorrhage, or seizures. After controlling for confounding factors including gestational age at PPROM and delivery, latency period, group B streptococcus (GBS) vaginal colonization, corticosteroid therapy, neonatal sex, mode of delivery, fetal indications for delivery, and umbilical cord pH, biochemical pulmonary maturity was not predictive of major neonatal morbidity. In our population, biochemical pulmonary maturity status does not appear to be predictive of neonatal morbidity in pregnancies complicated by PPROM at 32-34 weeks' gestation.  相似文献

One- and five-minute Apgar scores and umbilical cord arterial pH values were compared in preterm newborn infants of various gestational ages. The more premature the infant, the more likely the Apgar score was low in the presence of a pH greater than or equal to 7.25. Conversely, the closer to term, the more frequently an infant with a pH of less than 7.25 had an Apgar score of greater than or equal to 7. Therefore, in preterm infants, there is little congruity between the Apgar score and umbilical cord pH. Based on these findings, it is not appropriate to label preterm newborn infants as asphyxiated based on a low Apgar score.  相似文献

OBJECTIVE: To study the relationship between cord blood hepatic enzymes and obstetric and neonatal outcome in a Chinese population. MATERIALS AND METHODS: The study group consisted of 288 low-risk Chinese women with singleton term pregnancies. The following enzymes were assayed in cord blood: lactate dehydrogenase (LDH), glutamyl transferase (GGT), aspartate aminotransferase (AST) and alanine transferase (ALT). These were correlated to maternal and neonatal characteristics. RESULTS: A strong correlation was noted between cord blood AST and LDH (R = 0.582, p < 0.01), which was absent amongst those infants delivered by elective cesarean section. LDH, AST and ALT were negatively correlated with cord arterial pH and base excess (BE). GGT was inversely related only to gestational age (R = -0.18, p < 0.01). Both LDH and AST were weakly correlated with the duration of the first and second stages of labour. LDH was most closely linked to arterial pH, whereas AST was related to both arterial BE and duration of the second stage. CONCLUSIONS: The reference values are comparable to those published for Caucasian populations. There are moderate elevations in LDH and AST associated with the onset of labour and changes in acid-base status.  相似文献

OBJECTIVE: To determine neonatal outcome at 7 months of corrected age in very low birth weight (VLBW) infants with placental chorioamnionitis. METHODS: We conducted a cohort study of 287 VLBW infants delivered as a result of preterm premature rupture of membranes (PROM) or preterm labor. Control subjects (n = 123) had placentas with absent umbilical cord inflammation and absent or low-grade membrane inflammation. Case subjects (n = 164) had moderate membrane inflammation or any umbilical cord inflammation. Neonatal and 7-month outcomes were compared. A power analysis showed that 98 total subjects were needed to reject the two-sided null hypothesis with a difference in mean Bayley index scores of at least 8. RESULTS: Infants in the study group had significantly more preterm PROM, antenatal antibiotics, lower birth weight, lower gestational age, longer duration of ruptured membranes, and clinical chorioamnionitis. Intraventricular hemorrhage occurred more commonly in infants with placentas demonstrating chorioamnionitis (relative risk = 1.6, 95% confidence interval 1.1, 2.4, P =.013). One hundred sixty-seven (69%) of the 243 surviving infants had 7-month follow-up. There was no difference between cases and controls in mean Bayley mental developmental index (93 compared with 90, P =.25), psychomotor developmental index (89 compared with 90, P =.68), or in the number of infants that were developmentally delayed. Objective: To determine which parameter of the umbilical arterial cord gas analysis, pH, base deficit (BD) or lactate has a bigger predictive ability for neonatal morbidity at term.

Method: We conducted a four-year retrospective cohort study including all non-anomalous, singleton, vertex, term births with neonatal acidemia (umbilical arterial cord gas pH?≤?7.1). The primary outcomes were a composite neurological morbidity and a composite systemic morbidity. The predictive ability of lactate, BD and pH was compared using receiver operator characteristic (ROC) curves. Optimal cutoff values of lactate, BD and pH were estimated based on their maximal Youden Index.

Results: We identified 466 acidemic neonates who had paired and validated cord blood gas data. The ROC curve analysis revealed that pH, BD and lactate had a similar predictive ability for neurological (AUC: 0.81; 0.78; 0.83, respectively) and systemic neonatal morbidity (AUC: 0.77; 0.82; 0.82, respectively). The combination of pH?≤?7.0 and lactate?≥?7.0?mmol/L presented similar validity to that of pH?≤?7.0 and BD?≥?12?mmol/L, but both were comparable to pH alone.

Conclusions: pH, BD and lactate have similar predictive ability for adverse neonatal outcomes among acidemic neonates. Umbilical arterial lactate could replace BD as a measure of the metabolic component in acidemic neonates. However, neither BD nor lactate demonstrated in this study to improve the predictive ability of pH alone for short-term neonatal outcomes.  相似文献
