Les auteurs étudient les voies d'accès à des 2-polyaza-arylindane-1,3-diones et à leurs dérivés diversement substitués sur l'hétérocycle et l'homocycle ainsi que la pharmacomodulation de leur activité anti-inflammatoire. Ces β dicétoénamines fournissent en milieu alcalin des anions multidents conduisant en présence d'agents électrophiles à une C, N ou O-substitution; la mise en œuvre de la réaction de Mitsunobu a abouti à une N-(ω-amino-)-alkylation régiospécifique ou régiosélective. Parmi les diverses modifications structurales, la N-éthylation de la molécule de base, éventuellement couplée avec une méthoxylation en 5 sur l'homocycle s'est avérée efficace pour l'émergence d'une activité anti-inflammatoire en séries pyridazinyle, pyrimidinyle et tétrazolyle. Parmi les 3 molécules les plus actives 31, 43 et 50, la seconde a fait l'object d'une étude plus approfondie.  相似文献

La synthèse d'une série de 3-nitro et 3-amino-1,4-diméthyl-9H-carbazoles diversement substitués en positions 6 et 8 est décrite. Leur activité cytotoxique, évaluée in vitro au moyen de la culture clonogénique de la leucémie L1210, dépend fortement de la nature et de la position des substituants. Le plus cytotoxique d'entre eux, le dérivé 3-amino 6-hydroxy, présente une cytotoxicité comparable à celle de l'acétate de N2-méthyl-9-hydroxy ellipticinium (NMHE). Ces résultats et ceux présentés dans les deux publications précédentes, permettent une discussion détaillée des relations structure-activité dans la série du 9H-carbazole et du 1,4-diméthyl 9H-carbazole et suggèrent, pour les dérivés du 3-amino 1,4-diméthyl 9H-carbazole, un mécanisme d'action proche de celui des dérivés de l'ellipticine.  相似文献

Nutrition is a special treatment, between care and cure. Besides being a fundamental physiological need, it is a powerful emotional and symbolic vector regardless of age, and a treatment for the sick person. Nutrition, hydration and feeding are natural inalienable rights of the human being and therefore of human rights. Thus, this right has found its place, namely or not, in the Declarations and international Conventions of human rights since 1948. The right to receive adequate nutritional support is part of the fundamental principle of safeguarding the respect of the human being dignity and integrity, entered in French positive law in 1994 (first bioethics law), and since then regularly reaffirmed in all texts. Nutritional treatment is the tool of nutrition right. Badly applied, it needs to be widely recognized, defended, guaranteed and protected. We recount the emergence of this right from its sources, including in international law, and will show how it was introduced in the treatment relation.  相似文献

Successful ageing promotion is a major goal of care in geriatrician patients, even in old age. Cardiovascular prevention strategy, diabetes care or kidney protection involve dietary management. However, mortality and morbidity risk factors are modified in the oldest old, and on the other hand, dietary restrictions could impair health status of the older. Studies devoted to the oldest old are scarce. In most situations, exercise training promotion seems to better address the prevention goals than do diets.  相似文献

In the past 10 years several controlled studies have been published on the use of oral BCAA supplementation in patients with cirrhosis and chronic encephalopathy. Differences in trial design, end-point and treatment make any meta-analysis of the results impossible. From a review of the literature it appears that BCAA are safe and constitute a useful, non-comagenic source of nitrogen. Their superiority over other nitrogen sources (casein) is doubtful, but in long-term studies there seems to be an advantage in terms of encephalopathy, nutritional parameters and possibly survival. Given their low cost/effectiveness, other studies are justified to identify patients who may benefit most from long-term BCAA treatment.  相似文献

Diverse animal models have been used to model the complex metabolic changes involved in cancer-associate malnutrition. Transplantable tumors without anti-tumor therapy are useful to reveal catabolic mechanisms, if conducted within the limits of clinically relevant tumor burden. The few studies in which a treatment (surgery, chemotherapy) has been used suggest that malnutrition is the result of tumor-associated processes (capture of substrates; secretion of catabolic factors, alteration of the hormonal environment), of the anti-neoplastic treatments, or of interactions between these. Current approaches in nutritional intervention are focussed on the complex interplay between host and tumor, by the provision of specific amino acids (i.e., arginine, glutamine), as well as polyunsaturated n-3 fatty acids. Feeding route and diet formulation are critical elements of the research approach. Further evolved animal models representing the clinical diversity of cancer and treatments, and the key nutrients, will form a basis to develop the optimal nutritional support for the cancer patient.  相似文献

Within the framework of the French National Programme on Nutrition and Health, a survey was conducted in middle and high schools of the French Aquitaine region with the aim of describing the food supply within middle and high schools in 2004-2005, before and after the application of regulation of snack machins in September 2005 of the recent French law of public health.The results show that:

20 % of schools offered free snacks, specially bread (67 %) and fruit (39,3 %).
-
63 % of the answering schools had food or drink sales, concerning pastries (74,6 %), chocolate bars (30 %) and soft drinks (25,4 %).
-
In 2004-2005, 44 % of the high schools had snack machins. In Septembre 2005, each school (99 %) had removed on snack machines.
These results highlight the importance of the actions implemented in the programme of “Nutrition, prevention and health of children and teenagers in Aquitaine”, especially actions focussing on food supply within schools.  相似文献
