Objectives

To evaluate the effect of biannual fluoride varnish applications in preschool children as an adjunct to school-based oral health promotion and supervised tooth brushing with 1000 ppm fluoride toothpaste.

Methods

424 preschool children, 2–5 year of age, from 10 different pre schools in Athens were invited to this double-blind randomized controlled trial and 328 children completed the 2-year programme. All children received oral health education with hygiene instructions twice yearly and attended supervised tooth brushing once daily. The test group was treated with fluoride varnish (0.9% diflurosilane) biannually while the control group had placebo applications. The primary endpoints were caries prevalence and increment; secondary outcomes were gingival health, mutans streptococci growth and salivary buffer capacity.

Results

The groups were balanced at baseline and no significant differences in caries prevalence or increment were displayed between the groups after 1 and 2 years, respectively. There was a reduced number of new pre-cavitated enamel lesions during the second year of the study (p = 0.05) but the decrease was not statistically significant. The secondary endpoints were unaffected by the varnish treatments.

Conclusions

Under the present conditions, biannual fluoride varnish applications in preschool children did not show significant caries-preventive benefits when provided as an adjunct to school-based supervised tooth brushing with 1000 ppm fluoride toothpaste.

Clinical significance

In community based, caries prevention programmes, for high caries risk preschool children, a fluoride varnish may add little to caries prevention, when 1000 ppm fluoride toothpaste is used daily.  相似文献

Objectives

In vitro studies show that milk or milk components may have cariostatic properties. However, the results of epidemiological studies on the association between intake of dairy products and dental caries have been inconsistent. The purpose of this cross-sectional study was to examine the association between intake of dairy products and the prevalence of dental caries in young children.

Methods

Study subjects were 2058 Japanese children aged 3 years. Information on diet was assessed with a self-administered brief diet history questionnaire for children. The consumption of dairy products was categorized into 3 levels in order to represent the tertiles as closely as possible. Dental caries was assessed by a visual examination. Adjustment was made for sex, toothbrushing frequency, use of fluoride, between-meal snack frequency, maternal smoking during pregnancy, environmental tobacco smoke exposure at home, and paternal and maternal educational levels.

Results

Compared with yogurt consumption at the lowest tertile (<1 time/week), its intake at the highest level (≥4 times/week) was significantly associated with a lower prevalence of dental caries, showing a clear dose–response relationship (adjusted prevalence ratio = 0.78, 95% confidence interval: 0.62–0.98, P for trend = 0.04). There were no material associations between intake of cheese, bread and butter, or milk and the prevalence of dental caries.

Conclusions

These data suggest that a high consumption of yogurt may be associated with a lower prevalence of dental caries in young children.  相似文献

Objectives

To investigate the remineralizing effects of fluoride mouthrinses used at different times and frequency in addition to fluoride toothpaste.

Methods

A randomized crossover single blinded study comprised 4 experimental phases of 21 days each. Twelve orthodontic volunteers were fixed with an orthodontic bracket containing an artificial carious enamel slab, which was from the same tooth in all 4 phases, and were randomly assigned to the following groups: (1) brushing with F toothpaste 2× per day (F- brush), (2) F- brush + rinsing with 0.05% NaF (F- rinse) after lunch, (3) F- brush + F-rinse before bedtime, and (4) F- brush + F- rinse 2× per day. Mean mineral gain after each phase was determined from mineral density profiles obtained using Micro-CT.

Results

The mean mineral gain in all treatments with F- brush and F-rinse were significantly greater than those in F- brush (p < 0.05). Moreover F- rinse 2× per day increased lesion remineralization more than F- rinse once a day.

Conclusions

The twice-daily use of 0.05% NaF mouthrinse combined with twice-daily regular use of fluoride toothpaste resulted in the greatest remineralization of incipient caries. These data indicate that rinsing frequency is a factor affecting the effectiveness of fluoride mouthrinse.

Clinical significance

The rinsing frequency of NaF mouthrinse, when used with fluoride toothpaste, also affects the remineralization. This finding, if confirmed by a clinical study, would lead to a new recommendation for fluoride mouthrinse used in high caries risk patients who could benefit from using it twice a day.  相似文献

Objectives

This study aimed to evaluate posterior restorations placed in young adults, investigating the association between social determinants experienced during the life course and the quality of tooth fillings.

Methods

A representative sample (n = 720) of all 5914 individuals who were born in Pelotas in 1982 was prospectively investigated, and posterior restorations were assessed at 24 years of age. Exploratory variables included demographic and socioeconomic, oral health and dental service payment mode during the life course. Tooth-related variables (type of tooth, material and size of cavity) were also analysed.

Results

Multilevel logistic regression models showed that individuals who were always poor from birth to age 23 [odds ratio (OR) 2.35 (1.38–4.00)] and whose mothers had less years of education at their birth (OR 2.60 (1.44–4.68)) were with unsatisfactory restorations in posterior teeth more often. In addition, caries presence at age 15 (high decayed, missing, filled teeth (DMFT) tertile) (OR 1.95 (1.25–3.03)) and cavities with four or more surfaces (OR 18.67 (9.25–37.68)) were associated with the outcome.

Conclusions

These results show that socioeconomic characteristics of the individuals play an important role in restoration failures, reinforcing the need for preventive dental strategies and public policies to reduce inequalities as a major topic of oral health. In addition, the size of cavity appears as the most important determinant for restoration failure.

Clinical significance

Individual socioeconomic characteristics were associated with failure in posterior restorations in detriment of other clinical variables such as restorative material and type of tooth.  相似文献

Objectives

Inhibition of bacterial acid production by dental restorative materials is one of the strategies for secondary caries prevention. This study aimed to evaluate the effect of fluoride-releasing restorative materials on bacteria-induced pH fall at the bacteria–material interface.

Methods

Four fluoride-releasing restorative materials, glass-ionomer cement (GIC), resin-modified glass-ionomer cement (RMGIC), resin composite (RC) and flowable resin composite (FRC) were used. Each specimen was immersed in potassium phosphate buffer at pH 7.0 for 10 min and 4 weeks, and in potassium acetate buffer at pH 5.5 for 4 weeks. An experimental apparatus was made of polymethyl methacrylate and had a well with restorative materials or polymethyl methacrylate (control) at the bottom. The well was packed with cells of Streptococcus mutans, and the pH at the interface between cells and materials was monitored using a miniature pH electrode after the addition of 1% glucose for 90 min, and the fluoride released into the well was quantified using a fluoride ion electrode.

Results

The pH of GIC (4.98–5.18), RMGIC (4.77–4.99), RC (4.62–4.75) and FRC (4.54–4.84) at 90 min were higher than that of control (4.31–4.49). The fluoride amounts released from GIC were the highest, followed by RMGIC, RC and FRC, irrespective of immersion conditions. Saliva coating on materials had no significant effect.

Conclusions

The fluoride-releasing restorative materials inhibited pH fall at the bacteria–material interface. The degree of inhibition of pH fall seemed to correspond to the amount of fluoride detected, suggesting that the inhibition was due to the fluoride released from these materials.

Clinical significance

A little amount of fluoride actually released from the fluoride-releasing materials may have caries preventive potential for oral bacteria.  相似文献

Objectives

The aim of this study was to evaluate the anticaries effect of low-fluoride toothpastes combined with hexametaphosphate (HMP) on enamel demineralization.

Methods

Bovine enamel blocks were subjected to pH cycling and treatment with toothpaste's slurries (15 groups; 2×/day). Toothpaste mixtures contained the following: no fluoride (F) plus HMP (from 0 to 3.0%); 250 ppm F plus HMP (from 0 to 3.0%); 500 ppm F; 1100 ppm F; and a commercial toothpaste (1100 ppm F). After pH cycling, surface and cross-sectional hardness, as well as F present in the enamel were determined. The demineralization depth was analyzed using polarized light microscopy. The variables were subjected to 1-way ANOVA, followed by Student–Newman–Keuls’ test (p < 0.05).

Results

In the absence of fluoride, 0.5% HMP promoted the lowest mineral loss and its effect was similar to that of a 250 ppm F toothpaste (p > 0.05). The combination of 0.5% HMP and 250 ppm F resulted in lower mineral loss (p < 0.05) and similar lesion depth when compared to the 1100 ppm F toothpaste (p > 0.05).

Conclusion

To conclude, the combination of 0.5% HMP and 250 ppm fluoride in a toothpaste has a similar inhibitory effect on enamel demineralization in vitro when compared to a toothpaste containing 1100 ppm F.

Clinical significance

The anticaries effect of toothpaste containing 250 ppm F combined with 0.5% HMP was similar to that of a 1100 ppm F toothpaste, despite the 4-fold difference in F concentration. Although such effects still need to be demonstrated in clinical studies, it may be a viable alternative for preschool children.  相似文献

Background

Occlusal surfaces of erupting and newly erupted permanent molars are particularly susceptible to caries.The objective of the study was to assess and compare the effect of a single application of 38% SDF with ART sealants and no treatment in preventing dentinal (D3) caries lesions on occlusal surfaces of permanent first molars of school children who participated in a daily school-based toothbrushing program with fluoride toothpaste.

Methods

The prospective community clinical trial in the Philippines was conducted over a period of 18 months and included 704 six- to eight-year-old school children in eight public elementary schools with a daily school-based fluoride toothpaste brushing program. Children were randomly assigned for SDF application or ART sealant treatment. Children from two of the eight schools did not receive SDF or ART sealant treatment and served as controls. SDF or ART sealant treatment was applied on sound occlusal surfaces of permanent first molars. Surfaces that were originally defined as sound at baseline but which changed to dentinal (D3) caries lesions were defined as surfaces with new caries (caries increment). Non-compliance to the daily toothbrushing program in three schools offered the opportunity to analyze the caries preventive effect of SDF and sealants separately in fluoride toothpaste brushing and in non-toothbrushing children.

Results

In the brushing group, caries increment in the SDF treatment group was comparable with the non-treatment group but caries increment in the sealant group was lower than in the non-treatment group with a statistically significant lower hazard ratio of 0.12 (0.02-0.61). In the non-brushing group, caries increment in the SDF treatment group and the sealant group was lower than the non-treatment group but the hazard ratio was only statistically significant for the sealant group (HR 0.33; 0.20-0.54). Caries increment was lower in toothbrushing children than in non-toothbrushing children. Hazard ratios reached statistical significance for the non-treated children (HR 0.43; 0.21-0.87) and the sealant-treated children (HR 0.15; 0.03-0.072).

Conclusions

A one-time application of 38% SDF on the occlusal surfaces of permanent first molars of six- to eight-year-old children is not an effective method to prevent dentinal (D3) caries lesions. ART sealants significantly reduced the onset of caries over a period of 18 months.

Objectives

Glycemic index (GI) refers to the effect of food on glycemia and insulinemia. It is widely used in nutrition and diabetology to advise patients on adequate diets to maintain constant glucose and insulin blood levels. As GI is mainly related to the metabolic availability of glucides, it is believed it might influence the activity of oral bacteria. The aim of this work was to assess the relationships between the consumption of food with high GI and the Decayed, Missing, Filled Teeth (DMFT) index.

Methods

The trial was carried out in the Dental Clinic of the University of L’Aquila, during the academic year 2006-2007, in 272 students (115 males and 148 females; mean age, 24.2±2.6 and 23.7±3.3 years, respectively), from the University School of Medicine. The consumption of food with a high GI was investigated with the Food Frequency Questionnaire (FFQ) and the Decayed, Missing, Filled Teeth (DMFT) index.

Results

In some experimental groups, the consumption of food with a high GI was directly proportional to DMFT.

Conclusions

An inappropriate intake of food with a high GI seems to be linked with dental caries. This data suggests that collaboration among medical doctor, dental hygienist, and nutritionist would be useful for the patient for the prevention not only of metabolic diseases, but dental pathology as well.  相似文献

Objective

This in vitro study aimed to investigate the preventive effect of brushing with anti-erosive toothpastes compared to a conventional fluoride toothpaste on dentine erosion.

Materials and methods

Bovine dentine specimens (n = 12 per subgroup) were eroded in an artificial mouth (6 days, 6 × 30 s/day) using either citric acid (pH:2.5) or a hydrochloric acid/pepsin solution (pH:1.6), simulating extrinsic or intrinsic erosive conditions, respectively. In between, the specimens were rinsed with artificial saliva. Twice daily, the specimens were brushed for 15 s in an automatic brushing machine at 2.5 N with a conventional fluoride toothpaste slurry (elmex, AmF) or toothpaste slurries with anti-erosive formulations: Apacare (NaF/1% nHAP), Biorepair (ZnCO3-HAP), Chitodent (Chitosan), elmex Erosionsschutz (NaF/AmF/SnCl2/Chitosan), mirasensitive hap (NaF/30% HAP), Sensodyne Proschmelz (NaF/KNO3). Unbrushed specimens served as control. Dentine loss was measured profilometrically and statistically analysed using two-way and one-way ANOVA followed by Scheffe‘s post hoc tests. RDA-values of all toothpastes were determined, and linear mixed models were applied to analyse the influence of toothpaste abrasivity on dentine wear (p < 0.05).

Results

Dentine erosion of unbrushed specimens amounted to 5.1 ± 1.0 μm (extrinsic conditions) and 12.9 ± 1.4 μm (intrinsic conditions). All toothpastes significantly reduced dentine erosion by 24–67% (extrinsic conditions) and 21–40% (intrinsic conditions). Biorepair was least effective, while all other toothpastes were not significantly different from each other. Linear mixed models did not show a significant effect of the RDA-value of the respective toothpaste on dentine loss.

Conclusion

Toothpastes with anti-erosive formulations reduced dentine erosion, especially under simulated extrinsic erosive conditions, but were not superior to a conventional fluoride toothpaste.  相似文献

Objectives

The aim of this study was to assess the preventive effect of a fluoride-, stannous- and chitosan-containing (F/Sn/chitosan-) toothpaste (TP) on initial enamel erosion and abrasion.

Methods

In total, 150 human premolar enamel specimens were ground, polished and divided into 5 toothpaste/rinse groups (n = 30): (G1) placebo-TP/tap water, (G2) sodium fluoride (NaF-) TP/tap water, (G3) F/Sn/chitosan-TP/tap water, (G4) F/Sn/chitosan-TP/Sn-rinse, (G5) NaF-TP/NaF-rinse. The 8-day erosion–abrasion cyclic treatment (one cycle/day) consisted of incubating the samples in artificial saliva (30 min), then submitting the samples to toothbrush abrasion (2 min incubation in toothpaste slurry; brushing with 20 toothbrush strokes) and rinsing (2 min; 10 ml) with the respective solution: tap water (G1–G3), Sn-rinse (G4) or NaF-rinse (G5). Afterwards, the samples were submitted to erosion (2 min; 30 ml 1% citric acid, pH = 3.6). Surface microhardness (SMH) was measured initially and after every abrasion and erosion treatment. Enamel substance loss was calculated after each abrasion. Non-parametric ANOVA followed by Wilcoxon rank tests were used for analysis.

Results

G1 presented the greatest SMH decrease, while G4 presented the least SMH decrease (p < 0.001). G3 had a similar SMH decrease to G2 and G5. Substance loss was significantly lower in G4 than all other groups (p < 0.05), closely followed by G3. Both G2 and G5 showed similar calculated enamel substance loss to G1.

Conclusion

The treatment with F/Sn/chitosan-TP and tap water provided a similar SMH decrease to both NaF-TP groups, but significantly lower substance loss. F/Sn/Chitosan-TP and Sn-rinse showed a better preventive effect, which promoted less SMH decrease and reduced substance loss.

Clinical significance

The toothpaste containing fluoride, stannous and chitosan shows promising results in reducing substance loss from erosion and abrasion. The combination of this toothpaste with the stannous-containing rinse showed even better prevention against erosion–abrasion.  相似文献

BackgroundDietary intake of sugar-sweetened beverages (SSBs) has increased during the past 30 years, and SSB intake is associated with caries.
ConclusionsSSBs vary in the quantity and nature of sugars they contain; however, all of the sugars are fermentable by oral bacteria and potentially cariogenic.Practical ImplicationsOral health care practitioners should assess patients’ SSB intakes by asking questions regarding the quantity, frequency and duration of SSB intake. In response to individual behaviors, practitioners should tailor recommendations when providing anticipatory guidance to help patients decrease their exposure to SSBs for caries prevention. In light of the association between SSB intake and caries risk, as well as the increased consumption of SSBs, all dental practices should assess patients’ SSBs intake.  相似文献

Objectives

The purpose of this study was: (a) to compare the visual caries classification system ICDAS II with conventional (CR) and digital radiography (DR) for diagnosis of non-cavitated caries on free proximal surfaces, (b) to examine the potential of micro-computed tomography (MCT) to substitute histological examination for the in vitro caries assessment.

Methods

Both proximal surfaces of 20 teeth received classification separately by two examiners by means of the diagnostic modalities examined. The teeth were sectioned and assessed for depth of the lesion. The modalities were compared in terms of degree of inter-examiner agreement, sensitivity, specificity, accuracy, positive and negative predictive value and validity. Two diagnostic thresholds were applied: no caries versus all caries scores (D1), and no dentin caries versus dentin caries (D3).

Results

The weighted kappa values for inter-examiner reproducibility for all diagnostic modalities were 0.51–0.81. Visual examination (ICDAS II) reached significant higher sensitivity (0.92–0.96) and negative predictive value (0.9–1) than radiography. Likewise, the radiographic modalities presented significantly higher specificity (0.93–1) and positive predictive values (0.92–1) than the ICDAS II criteria. The overall accuracy performance of radiographic modalities was related to the diagnostic threshold. MCT did not agree with histological validation at each disease severity scale.

Conclusions

The ICDAS II criteria are a promising tool for caries diagnosis on free proximal surfaces. DR and CR radiography present comparable performance. Furthermore, MCT is not capable of constituting a reliable alternative to histological examination for caries research.  相似文献

Objective

The aim of this 2-year cohort study (2003 to 2005) was to investigate how caries experience, at initial lesions (early or non-cavited lesions) and cavited stages, predicts caries increment in permanent teeth in 7-10- year-olds.

Material and Methods

The random sample of 765 children attending public schools in the city of Piracicaba, SP, Brazil, was divided into two groups: 423 children aged 7-8 years and 342 children aged 9-10 years. All subjects were examined by a calibrated examiner, using dental mirror and ball-ended probes, after tooth brushing and air-drying in an outdoor setting, based on the World Health Organization criteria. Active caries with intact surfaces were also recorded as initial lesion (IL). Univariate analysis was used for statistical analysis (Odds Ratios and Chisquare).

Results

The association between the DMFT (decayed, missing and filled teeth) increment and the presence of IL was significant only for 9-10-year-old children. The children with DMFT>0 at baseline were more prone to have DMFT increment, with the highest risk for caries increment occurring in children aged 7-8 years.

Conclusion

The predictors of caries increment were the presence (at baseline) of caries experience in permanent teeth for both age groups (7-8; 9-10-year-olds) and the presence of the IL (at baseline) for 9-10-year-olds.  相似文献

Objectives

A history of caries in the primary molars is associated with an advanced emergence of their permanent successors. Hence, caries in the primary molars may have an impact on the emergence order of the permanent teeth. The aim of the present study was to fully investigate the variability in permanent tooth emergence, taking into account the (caries) status of the primary molars.

Methods

For this purpose data available from the Signal Tandmobiel® project were used. In this prospective longitudinal survey data were collected from a representative sample of 4468 children, examined yearly by trained dentist-examiners. Bayesian statistical analyses taking into account the interval-censored character of the data were performed.

Results

56% of all examined primary molars were sound; between 2.5 and 7.2% of the first and second primary molars were extracted due to caries. When both primary molars were sound, the most prevalent emergence order was ‘4–3–5–7’ (first premolar–canine–second premolar–second molar) in the maxilla and ‘3–4–5–7’ in the mandible. When both maxillary primary molars were affected by caries (i.e., decayed, filled or extracted due to caries), the sequence ‘4–5–3–7’ was the most prevalent whereas sequences ‘3–4–5–7’ and ‘4–3–5–7’ were less prevalent. When both mandibular primary molars were affected by caries, the prevalence of sequences ‘4–3–5–7’, ‘4–3–7–5’ and ‘4–5–3–7’ increased whereas the prevalence of sequences ‘3–4–5–7’ and ‘3–4–7–5’ decreased.

Conclusions

A history of caries in the primary molars is associated with an altered emergence order of canines, premolars and second molars.  相似文献

Background

One of the dental health goals of Health Japan 21, in which the Japanese government clarified its health policy, was to ensure the use of fluoride toothpaste in 90% or more of schoolchildren. This goal was not achieved. The aim of this cross-sectional questionnaire study was to evaluate the characteristics of parents whose children use non-fluoride toothpaste.

Methods

In December 2010, questionnaire forms were sent to 18 elementary schools or school dentists. Students (6-12 years old) were asked to take the forms home for their parents to fill in, and to bring the completed questionnaire to school. The collected questionnaires were mailed from schools to the author’s institution by the end of March 2011. The relationship between fluoride in toothpaste and reasons for choice of toothpaste, the child’s toothbrushing habits, and attitude toward child caries prevention was examined in the 6,069 respondents who answered all the questions for the analyses and indicated that their children use toothpaste.

Results

Non-fluoride toothpaste users accounted for 5.1% of all toothpaste users. Among the children using non-fluoride toothpaste, significantly greater numbers gave ‘anti-gingivitis’, ‘halitosis prevention’ or ‘tartar control’ as reasons for choice of toothpaste; did not give ‘has fluoride’, ‘is cheaper’ or ‘tastes good’ as reasons for choice of toothpaste; or used toothpaste sometimes, or were in 4th - 6th grades. There was no significant relationship between use of non-fluoride toothpaste and measures taken for caries prevention in children. Multilevel (first level: individual, second level: school) logistic regression analysis indicated that use of non-fluoride toothpaste was significantly related to: giving ‘anti-gingivitis’ (odds ratio: 1.44) as a reason for choice of toothpaste; not giving ‘has fluoride’ (0.40), ‘tastes good’ (0.49) or ‘is cheaper’ (0.50) as the reason for choice of toothpaste; to toothbrushing less often (twice a day: 1.34, once a day or less: 1.46) and to using toothpaste less often (sometimes: 1.39).

Conclusions

It is necessary to teach parents that dental caries is the dental health issue with the highest priority for children, and therefore fluoride toothpaste should be used.

Objective

This study aimed to evaluate the impact of toothpaste slurry abrasivity and toothbrush filament diameter on abrasion of eroded dentin in vitro.

Methods

Eroded dentin samples (hydrochloric acid, pH 2.6, 15 s) were brushed with 40 strokes in an automatic brushing machine using manual toothbrushes with different filament diameter (0.15, 0.20 or 0.25 mm). The toothbrushes were applied with a control slurry free of abrasive particles (RDA-value 10) or toothpastes slurries with different abrasivity (RDA-values 20, 50 or 100). Each erosive–abrasive cycle was followed by storage of the dentin samples in artificial saliva for 3 h. After each 4 cycles, the samples were stored in artificial saliva for 15 h. After 60 cycles, dentin loss was measured by profilometry and statistically analysed by ANOVA and linear regression analysis.

Results

Dentin loss increased along with the RDA-value of the toothpaste slurries. The impact of the filament diameter on dentin loss was less evident compared to the RDA-value. However, toothbrushes with smaller filament stiffness caused higher dentin wear in all toothpaste slurry groups (RDA 20, 50 and 100) except for the paste-free control group (RDA 10).

Conclusion

Abrasion of eroded dentin increased along with the RDA-value of the toothpaste slurry and with decreasing filament diameter of the toothbrush.  相似文献

Background

With rapid urbanization in China, an increasing number of rural adults have migrated to cities to seek job opportunities, leaving their school-aged children behind. These left-behind children (LBC) without one or both parents usually receive less attention from their caregivers. Whether the parental migration affects the children’s oral health is not well understood. This study aimed to explore the differences in dental caries status and oral health-related behaviors between children with different parental migration experiences in a rural area of Southern China.

Methods

A cross-sectional study was conducted in Luchuan County of Guangxi Province in 2015. A total of 1085 school children aged 8–12 participated in this study. Participants’ demographic characteristics, parental migration information, and eating and oral hygiene habits were collected using a self-administered questionnaire. Dental caries of permanent teeth was examined using the decayed, missing, and filled tooth (DMFT) index recommended by the World Health Organization. Dental caries experience and oral health-related behaviors were compared between LBC and non-LBC, as well as children with different experiences of parental migrations. The impact of various parental migration attributes on LBC oral health outcomes was examined by univariate and multivariate analyses.

Results

Among the school-aged children examined, 60.9% of them were LBC. Only 29.7% of the children brushed their teeth regularly; 86.5% of them did not know what fluoride toothpaste was. Caries prevalence was 51.4% for LBC and 40.8% for non-LBC (p <?0.001). The LBC experienced a greater DMFT mean (1.20?±?1.59) compared to the non-LBC (0.85?±?1.30) (p <?0.001). Oral health-related behaviors were not significantly different between LBC and non-LBC. Dental caries experience and oral health-related behaviors were not related to the type or duration of parental migration. Multiple regression analyses showed that parental migration was one of significant predictors of children’s caries outcome; LBC had a higher risk to caries than non-LBC (95% CI =1.26, 2.09).

Conclusions

These findings indicate that parental migration could be a significant risk factor for caries development among 8- to 12-year-old school children in rural China.

Objectives

The aim of this clinical study was to evaluate the retention rate and caries-prevention effect of a flowable composite compared to a conventional resin-based sealant in a young population over a 24-month period.

Methods

Thirty-four patients, ranging in age from 16 to 22 years, diagnosed with at least 2 non-cavitated pit-and-fissure caries in the first and second molars were selected for this randomized split-mouth design trial. A total of 220 sealants, were placed in 117 upper molars and 103 lower molars. The teeth were sealed with a flowable resin composite (Tetric Evo Flow) or a sealant material (Helioseal F). Each restoration was independently evaluated in terms of retention and the presence of caries at baseline and at 1, 6, 12, and 24 months. Data were analyzed using non-parametric Mann–Whitney U and Friedman 1-way ANOVA tests at p < 0.05.

Results

Tetric Evo Flow showed complete retention with 100%, 95.5%, 93.8%, and 88.5% at 1, 6, 12, and 24-month evaluations, respectively, while Helioseal F retention rates were 98.1%, 95.5%, 94.8%, and 85.4%, respectively, for the same evaluation periods. At the 24-month recall, 4 (4.2%) total losses were observed in subjects treated with Tetric Evo Flow and 2 total losses (2.1%) for Helioseal F, respectively. No significant differences were observed between the materials in retention rates or caries incidence for each evaluation period (p > 0.05).

Conclusion

Placement of flowable composite as fissure sealants in the younger population seems to be as effective as conventional fluoride containing fissure sealants for the prevention of fissure caries.

Clinical relevance

The use of a flowable composite as a fissure sealant material, in conjunction with a total-etch, single bottle adhesive, yielded better retention than did the conventional fluoride containing resin-based fissure sealant over a 24-month period in young patients.  相似文献

Objectives

This randomized, cross-over in situ study investigated the impact of sodium fluoride dose and concentration in milk on caries lesion rehardening, fluoridation and acid resistance.

Methods

Twenty-eight subjects wore two gauze-covered enamel specimens with preformed lesions placed buccally on their mandibular partial dentures for three weeks. Participants used fluoride-free dentifrice throughout the study and consumed once daily one of the five study treatments: no fluoride in 200 ml milk (0F-200), 1.5 or 3 mg fluoride in either 100 (1.5F-100; 3F-100) or 200 ml milk (1.5F-200; 3F-200). After three weeks, specimens were retrieved. Knoop hardness was used to determine rehardening and resistance to a secondary acid challenge. Enamel fluoride uptake (EFU) was determined using a microbiopsy technique.

Results

A linear fluoride dose–response was observed for all study variables which exhibited similar overall patterns. All the treatments resulted in rehardening, with 0F-200 inducing the least and 3F-100 the most. Apart from 1.5F-200, all the treatments resulted in statistically significantly more rehardening compared to 0F-200. The fluoride doses delivered in 100 ml provided directionally although not statistically significantly more rehardening than those delivered in 200 ml milk. EFU data exhibited better differentiation between treatments: all fluoridated milk treatments delivered more fluoride to lesions than 0F-200; fluoride in 100 ml demonstrated statistically significantly higher EFU than fluoride in 200 ml milk. Findings for acid resistance were also more discerning than rehardening data.

Conclusions

The present study has provided further evidence for the anti-caries benefits of fluoridated milk. Both fluoride dose and concentration appear to impact the cariostatic properties of fluoride in milk.  相似文献

Objectives

This study aimed to investigate the patient and tooth factors associated with selection of restorative material in direct posterior restorations in young adults from a population-based birth cohort.

Methods

A representative sample (n = 720) of all 5914 births occurring in Pelotas in 1982 were prospectively investigated, and posterior restorations were assessed in 2006, when the patients were 24 years old. Tooth-related variables (individual level) included restorative material (amalgam or composite), type of tooth, size of cavity, and estimated time in mouth. Data regarding demographic and socio-economic characteristics, oral health, and service utilization patterns during the life course were also assessed (contextual level).

Results

Logistic Regression Multilevel models showed that individuals who have accessed dental services by private insurance by age 15 [odds ratio (OR) = 1.66 (0.93–2.95)] and who had a higher dental caries index at age 15 (high DMFT tertile) [OR 2.89 (1.59–5.27)] presented more amalgam restorations in the posterior teeth. From tooth-level variables, the frequency of amalgams decreases with increasing number of surfaces enrolled in the cavity preparation (p < 0.001) and was almost 5 times greater in molars than in premolars.

Conclusions

The present findings suggest that variables related to type of dental service, dental caries (higher DMFT index), and cavity characteristics (tooth type, size) determine the choice of dentists for restorative materials. Other individual characteristics such as demographic and socioeconomic status have not influenced this choice.

Clinical significance

This is the first population-based study that assesses the determinant factors for the choice of dentists for composite or amalgam in posterior direct restorations, showing that, independently of socioeconomic and demographic characteristics, type of payment of dental services and clinical factors are associated with this choice.  相似文献
