Real-time 3-dimensional echocardiography (RT3DE) can provide a unique combination of accurate left atrial (LA) volume quantification and rapid, automatic assessment of LA function. The aim of the study was to evaluate the changes in LA volumes and function in patients with atrial fibrillation (AF) undergoing radiofrequency catheter ablation (RFCA) using RT3DE; 57 consecutive patients referred for RFCA were studied. Paroxysmal AF was present in 43 patients (75%) and persistent AF in 14 (25%). After a mean follow-up of 7.9 +/- 2.7 months, patients were divided into 2 groups: successful RFCA (SR group) and recurrence of AF (AF group). RT3DE was performed before, within 3 days, and 3 months after RFCA to assess LA volumes (maximum, minimum, and preA) and LA functions (passive, active, and reservoir). A total of 38 patients (67%) had successful RFCA (SR group). Immediately after RFCA, no significant changes in LA volumes and function were observed. After 3 months, a significant reduction in LA volumes (maximum: 26 +/- 8 to 23 +/- 7 ml/m(2), p <0.01) was noted only in the SR group, with a significant improvement in LA active (22 +/- 8% to 33 +/- 9%, p <0.01) and reservoir functions (116 +/- 45% to 152 +/- 54%, p <0.01). Conversely, the AF group showed a trend towards a deterioration of LA volumes and function. In conclusion, in patients who maintain sinus rhythm after RFCA, a significant reverse remodeling and functional improvement of the left atrium is observed using RT3DE.  相似文献

Several techniques for treatment of atrial fibrillation (AF) have been developed, including the direct placement of radiofrequency for lesions at open heart surgery. Detailed evaluation of left atrial (LA) function has not been performed after these procedures and has not been compared in patients with chronic AF. We compared the atrial function of patients with sustained sinus rhythm (SR) after linear ablation with a group who underwent direct-current cardioversion and a group of normal controls to investigate the measurable deleterious effects, if any, on atrial function after the surgical procedure. Twenty-one consecutive patients who had maintained SR for >6 months after a linear radiofrequency ablation (LRFA) procedure were studied. As control subjects, we examined 33 patients with chronic AF successfully restored to SR by cardioversion who maintained SR for >6 months and 42 age-matched normal subjects. LA function was decreased in both the LRFA and cardioverted AF groups compared with normal controls. The parameters of LA function, atrial fraction, LA ejection fraction, and the A' velocity were lowest in the LRFA group, intermediate in the cardioverted AF group, and highest in the normal controls (LA function: 15.8 +/- 10%, 26 +/- 10%, 33 +/- 7%; p = 0.0001; LA ejection fraction: 31 +13%, 41 +/- 12%, 51 +/- 9%; p = 0.0001; A' velocity: 4.2 +/- 1.4, 7.6 +/- 2.2, 9.5 +/- 1.9 cm/s; p = 0.0001). LA volumes were increased in the LRFA and cardioverted AF groups compared with normals (62.8 +/- 22 vs 70.6 +/- 17 vs 38.7 +/- 9.8 ml; p = 0.0001). Thus, although LA function is restored and maintained after LRFA has been performed during open heart surgery, LRFA use is associated with a measurable decrease in LA function over and above that found after conventional cardioversion.  相似文献

Radiofrequency catheter ablation has been demonstrated to be effective in the treatment of patients with atrial fibrillation. However, its impact on left atrial (LA) function has not been widely studied. The purpose of the present study was to evaluate the impact of radiofrequency catheter ablation on LA function in patients with atrial fibrillation. Thirty-eight patients with symptomatic drug-refractory atrial fibrillation were treated with circumferential pulmonary vein ablation (CPVA). LA volumes and function were assessed with real-time 3-dimensional echocardiography before and 6 months after the procedure. The effectiveness of CPVA was evaluated at 6-month follow-up. Recurrence of the arrhythmia was defined as any documented (clinically or on 24-hour Holter electrocardiography) atrial tachyarrhythmia lasting>30 seconds after the first 12 weeks after the procedure. CPVA induced a reduction of maximum LA volume (from 55+/-15 to 48+/-16 ml, p<0.001), without impairment in LA function, measured as the active emptying percentage of total volume (32+/-29% vs 39+/-33%, p=NS). At follow-up, 21 patients (61.8%) had no recurrences. Maximum LA volumes were significantly larger in patients who presented with recurrences compared with those who did not (64+/-18 vs 50+/-11 ml, p=0.01). In conclusion, CPVA induces a reduction in LA volume without a deleterious impact on function, and, of importance, real-time 3-dimensional echocardiography is a useful noninvasive imaging tool to follow up LA remodeling and function in these patients.  相似文献

INTRODUCTION: Information about focal atrial fibrillation (AF) originating from the right atrium has not been well described. The purposes of this study were to demonstrate the electrophysiologic characteristics and radiofrequency catheter ablation in patients who had right atrial focal AF. METHODS AND RESULTS: From January 1996 to September 1998, 172 patients with clinically documented attacks of paroxysmal AF were referred to this institution for electrophysiologic study and/or radiofrequency catheter ablation. Anterior free wall, crista terminalis, and right and left superior pulmonary veins were mapped simultaneously. Eight patients (4.7%) had right atrial focal AF, consistent activation sequence, irregular fibrillation interval (mean fibrillation interval: 164 +/- 11 msec), and episodes of exit block from the initiating foci observed. The presumed ablation site was chosen on the basis of the earliest bipolar activity relative to an atrial electrogram reference during the initiation of AF. After application of 2 +/- 1 radiofrequency pulses, AF was eliminated without recurrence during the follow-up period (mean: 14 +/- 8 months; range: 3 to 25). Twenty-four-hour Holter monitoring showed that the number of atrial premature beats decreased significantly at the 3-month follow-up (4,216 +/- 411 vs 135 +/- 14 beats/day). CONCLUSION: Right atrial focal AF is one subgroup of focal AF, and it can be cured by radiofrequency catheter ablation.  相似文献

AIMS: We determined late atrial function following a surgical linear endocardial radiofrequency (RF) ablation procedure that aimed to restore and maintain sinus rhythm (SR) in atrial fibrillation (AF). We tested the hypothesis that successful restoration of SR is accompanied by measurable mechanical atrial function that is at normal or near normal levels. METHODS: Forty-seven patients who underwent the surgical RF procedure at least 6 months previously (median 2.86 years; range: 0.6-4.2 years) were studied using an array of echocardiographic variables. Two patient groups (SR restored [RF-SR], persistent AF [RF-AF]) and an age matched control group were studied. Among the echocardiographic variables measured were left atrial (LA) size and volume, LA active fractional emptying and mitral annular displacement corresponding to atrial contraction (A' velocity) by Doppler tissue imaging. RESULTS: At long term follow up 29/47of patients who underwent the RF procedure were in SR with atrial contraction present echocardiographically. Of the patients initially restored to SR, the proportion remaining in SR at 3 years was 79% (SE 9%). The atrial-emptying fraction was reduced in comparison to that seen in normal controls (27+/-14% vs 46+/-10%). The A' velocity was decreased in the surgical RF cohort vs controls (4.4+/-1.3 vs 9.7+/-1.7cm/s; P=0.0001). Despite LA size preoperatively being similar in both surgical groups, atrial size decreased in those in whom SR was restored (48.6+/-7.6 vs 44.8+/-4.7mm; P=0.0001) but increased in those in whom AF persisted (48.2+/-8.1mm vs 52.3+/-7.8mm; P=0.0001). CONCLUSION: The radial pattern of linear radiofrequency ablation used in the present study resulted in restoration of SR and atrial function. Procedural success was independent of preoperative atrial size. Restoration of SR results in 'reverse' atrial remodelling and improved atrial function. However atrial function remains modestly impaired, either due to the ablation lesions or pre-existing atrial disease.  相似文献

INTRODUCTION: Understanding the structural remodeling and reverse remodeling of the left atrium (LA) and pulmonary vein (PV) after radiofrequency ablation of atrial fibrillation (AF) may provide important insights into the mechanism and management of AF. This study used magnetic resonance angiographic (MRA) images to investigate changes in PV and LA morphologies before and more than 1 year after ablation. METHOD AND RESULTS: Forty-five patients (36 men and 9 women, mean age 60 +/- 13 years) who underwent MRA before and more than 12 months (mean 21 +/- 11) after ablation of paroxysmal AF were included in the study. The patients were divided into two groups: group I included 35 patients without AF recurrence, and group II included 10 patients with late (>1 month postablation) recurrence of AF. The sizes of the LA and nonablated PV were compared before and after ablation. In group I, significant reduction of ostial area of both superior PVs was noted (left superior PV: from 2.85 +/- 0.67 to 2.59 +/- 0.73 cm2; right superior PV: from 2.89 +/- 0.85 to 2.60 +/- 0.73 cm2, both P <0.001). Geometric alteration toward a round shape was noted in the ostia of superior PVs during follow-up (eccentricity of right superior PV and left superior PV decreased from 0.31 +/- 0.10 to 0.22 +/- 0.13 and from 0.27 +/- 0.11 to 0.19 +/- 0.13, respectively, both P <0.01). However, LA volume showed only borderline reduction (from 61.52 +/- 19.06 to 56.64 +/- 17.13 mL, P=0.05). In group II, significant dilation of the LA (from 61.14 +/- 17.54 to 78.73 +/- 25.27 mL, P=0.004) and right superior PV (from 3.41 +/- 1.12 to 4.08 +/- 1.31 cm2, P=0.016) was noted during follow-up. Ostial area and eccentricity of the left superior, left inferior, and right inferior PVs and LA were similar before and after ablation. CONCLUSION: Structural remodeling of the superior PVs and LA can be reversible after successful ablation without AF recurrence; however, late recurrence of AF is associated with progressive LA dilation.  相似文献

The effect of successful catheter ablation of overt accessory pathways on the incidence of atrial fibrillation (AF) was studied in 129 symptomatic patients with (n = 75) or without (n = 54) previous documented AF. Fourteen had had ventricular fibrillation. Factors predictive of recurrence were examined, including electrophysiologic parameters. Atrial vulnerability was defined as induction of sustained AF (greater than 1 minute) using single, then double, atrial extrastimuli at 2 basic pacing cycle lengths. When compared to patients with only reciprocating tachycardia, patients with clinical AF included more men (77 vs 54%, p = 0.008) and were older (35 +/- 12 vs 29 +/- 12 years, p = 0.01). They had a significantly shorter cycle length leading to anterograde accessory pathway block (252 +/- 42 vs 298 +/- 83 ms, p less than 0.001), greater incidences of atrial vulnerability (89 vs 24%, p less than 0.001) and subsequent need for cardioversion (51 vs 15%, p less than 0.001). After discharge, the follow-up period was 35 +/- 12 months (range 18 to 76); 7 patients with previous spontaneous AF (9%) had recurrence at a mean of 10 months after ablation. Age, presence of structural heart disease accessory pathway location, atrial refractory periods and accessory pathway anterograde conduction parameters were not predictive of AF recurrence. Persistence of atrial vulnerability after ablation was the only factor associated with further recurrence of AF. Atrial vulnerability was observed after ablation in only 56% of patients with previous AF versus 89% before ablation. BACKGROUND: The effects of left atrial (LA) circumferential ablation on LA function in patients with atrial fibrillation (AF) have not been well described. OBJECTIVES: The purpose of this study was to determine the effect of LA circumferential ablation on LA function. METHODS: Gated, multiphase, dynamic contrast-enhanced computed tomographic (CT) scans of the chest with three-dimensional reconstructions of the heart were used to calculate the LA ejection fraction (EF) in 36 patients with paroxysmal (n = 27) or chronic (n = 9) AF (mean age 55 +/- 11 years) and in 10 control subjects with no history of AF. Because CT scans had to be acquired during sinus rhythm, a CT scan was available both before and after (mean 5 +/- 1 months) LA circumferential ablation (LACA) in only 10 patients. A single CT scan was acquired in 8 patients before and in 18 patients after LACA ablation. Radiofrequency catheter ablation was performed using an 8-mm-tip catheter to encircle the pulmonary veins, with additional lines along the mitral isthmus and the roof. RESULTS: In patients with paroxysmal AF, LA EF was lower after than before LACA (21% +/- 8% vs 32 +/- 13%, P = .003). LA EF after LA catheter ablation was similar among patients with paroxysmal AF and those with chronic AF (21% +/- 8% vs 23 +/- 13%, P = .7). However, LA EF after LA catheter ablation was lower in all patients with AF than in control subjects (21% +/- 10% vs 47% +/- 5%, P < .001). CONCLUSION: During medium-term follow-up, restoration of sinus rhythm by LACA results in partial return of LA function in patients with chronic AF. However, in patients with paroxysmal AF, LA catheter ablation results in decreased LA function. Whether the impairment in LA function is severe enough to predispose to LA thrombi despite elimination of AF remains to be determined.  相似文献

PURPOSE: The aim of the study was to identify variables associated with successful long-term maintenance of sinus rhythm (SR) after a single ablation of long-lasting persistent atrial fibrillation (AF). METHODS: Complex left atrial (LA) ablation was performed in 100 patients. Restoration of SR by ablation was the desired procedure endpoint. RESULTS: SR was restored by ablation in 38 patients during the first procedure. Following one ablation, 50 patients remained in SR for 31 +/- 14 months. SR maintenance was associated with shorter duration of the persistent AF (median 14 vs. 22 months; P = 0.05), lower proportion of the LA points exhibiting voltage <0.2 mV (median 20% vs. 33%; P = 0.006), and higher proportion of LA points showing voltage >1 mV (median 15% vs. 11%; P = 0.02). CONCLUSION: Among clinical variables, shorter duration of persistent AF and higher voltage recorded around the LA predicted long-term maintenance of SR after single ablation.  相似文献

It has been demonstrated that successful cavotricuspid isthmus ablation of typical atrial flutter combined with atrial fibrillation (AF) sometimes influences the preablation history of paroxysmal AF. However, the effectiveness of only isthmus ablation on AF itself is unclear. Endocardial catheter mapping during induced AF was performed around the tricuspid annulus using duodecapolar clectrode catheters in 39 patients with drug-refractory paroxysmal AF. Isthmus ablation was performed in 16 patients (41%) in whom catheter mapping during AF showed an organized activation pattern around the tricuspid annulus. During a mean follow-up of 12.3 months, isthmus ablation was successful in preventing AF in 12 (75%) patients, 8 without medication and 4 with a previously ineffective drug. This success group had a significantly higher F wave amplitude in lead V1 (0.29+/-0.10 vs 0. 15+/-0.04 mV, p < 0.01), a higher left ventricular ejection fraction (74+/-9 vs 58+/-2%, p < 0.05), and a smaller left atrial dimension (35+/-6 vs 43+/-4 mm, p < 0.05) than the failure group. Isthmus ablation may be effective in preventing paroxysmal AF with an organized activation pattern around the tricuspid annulus. F wave amplitude, left ventricular ejection fraction, and left atrial dimension were significant predictors of success.  相似文献

The present study analyzed the recurrence rate of idiopathic atrial fibrillation (AF) after elimination by radiofrequency ablation of the substrate for a regular tachycardia. Forty consecutive patients with idiopathic AF and a history of regular palpitations or documented regular tachyarrhythmias were prospectively included. Regular tachyarrhythmias were induced in 82.5% of patients: atrial flutter (42.4% of the inducible arrhythmias), atrial tachycardia (24.2%), atrioventricular (AV) nodal reentry (18.2%), AV reentry through a concealed accessory pathway (9.1%), and AV nodal reentry associated with right atrial tachycardia (6.1%). Dual AV node physiology with single or dual AV node echoes was demonstrated in 6.1% of patients without inducible arrhythmias. During follow-up (92 +/- 11 months), AF recurred in 19.2% of patients after successful radiofrequency ablation and in 69.2% after unsuccessful or not performed procedures (p <0.05). Left atrial diameter did not change after successful ablation but increased significantly in the population without elimination of the substrate (initial diameter 37.5 +/- 2 mm, final diameter 46.4 +/- 3.2 mm; p <0.05). In conclusion, the systematic search for the substrate of regular tachyarrhythmias followed by their elimination by catheter ablation reduces the recurrence of idiopathic AF in patients with a history of regular palpitations or documented regular tachyarrhythmias.  相似文献

目的:探讨已达到消融终点的长程持续性心房颤动(房颤)患者复发的危险因素。方法:纳入达到消融终点的长程持续性房颤患者256例,消融终点定义为双侧肺静脉电隔离,二尖瓣峡部和左心房顶部线性消融双向阻断且碎裂电位消失。根据随访结果将患者分为房颤复发组(n=43)和无复发组(n=213)。通过多因素 COX 回归分析探讨房颤复发的独立危险因素。结果:经过(19.5±3.6)个月随访,与无复发组相比,房颤复发组患者右心房内径较大,为(53.31±6.55)mm 对(48.74±5.87)mm;房颤持续时间较长,为(81.83±45.75)个月对(53.16±40.23)个月;左心房内径较大,为(49.85±6.82)mm 对(46.77±5.83)mm,P 均<0.01。多因素 COX 回归分析发现,左心房内径增大(OR=1.01,95%CI:1.01~1.28,P <0.05),右心房内径增大(OR=2.85,95%CI:1.15~7.03,P <0.05)、房颤持续时间延长(OR=1.01,95%CI:1.01~1.02,P <0.05)是房颤复发的独立危险因素。结论:除左心房内径和房颤持续时间外,右心房内径增大也是已达到消融终点的长程持续性房颤复发的独立危险因素。  相似文献

BACKGROUND: Conversion to sinus rhythm (SR) is rarely attempted in patients with rheumatic atrial fibrillation (AF) because the length of AF duration and the dilation of left atrium (LA) make maintenance of SR difficult. In this study, predictors of the successful maintenance of SR with amiodarone and electrical cardioversion in rheumatic AF patients receiving percutaneous transluminal mitral valvuloplasty (PTMV) were identified. METHODS AND RESULTS: This study included 23 consecutive patients undergoing PTMV for rheumatic AF (6 men, 53+/-11 years; AF duration 25 +/-24 months; LA diameter 44+/-6 mm; mitral valve area (MV) 1.1+/-0.2 cm(2)). Electrical cardioversion was required for the successful conversion to SR in all patients regardless of whether they had received amiodarone (400 mg/day) 2 months before PTMV (n=8) or 2 months after (n=15). After cardioversion, all patients received amiodarone 200 mg/day. With a follow-up period of 35+/-8 months, 14 patients (61%) remained in SR. A greater reduction in LA size (-4+/-3 mm vs 1+/-1 mm; p=0.004) and an greater increase in MV area (0.8+/-0.4 cm(2) vs 0.5+/-0.2 cm(2); p=0.01) by PTMV, not AF duration, were found to be the independent predictors for patients with successful maintenance of SR as compared with patients with recurrence of AF. CONCLUSION: In rheumatic AF patients receiving PTMV, the successful maintenance of SR with amiodarone and electrical cardioversion can be predicted by the degree to which LA size is reduced and MV area is increased.  相似文献

Radiofrequency catheter ablation (RFCA) is a potential curative treatment for atrial fibrillation (AF) by eliminating the arrhythmia and inducing left atrial (LA) reverse remodeling. However, the effect on LA function, especially after repeated procedures, has scarcely been studied. The aim of this study was to evaluate the impact of RFCA on LA size and function in patients with AF after a first and a repeated procedure. RFCA was performed in 154 patients with symptomatic drug-refractory AF. LA volumes and function were assessed with real-time 3-dimensional echocardiography before and 6 months after the procedure. Recurrence of the arrhythmia was defined as any atrial tachyarrhythmia lasting >30 seconds, clinically documented or by 24-hour Holter recording, after the first 6 months after ablation. Of the 154 patients, 104 (67%) required only a first ablation, and 50 (33%) required redo RFCA. LA volume was reduced after first RFCA (from 60 ± 19 to 52 ± 17 ml for 3-dimensional LA maximum volume, p <0.001, and from 38 ± 18 to 33 ± 15 ml for 3-dimensional LA minimum volume, p <0.000) without impairment of LA contractile function, measured as the active emptying percentage of total volume (39 ± 25% vs 43 ± 26%, p = NS). After repeated RFCA procedures, 3-dimensional LA maximum volume was reduced (from 57 ± 18 to 52 ± 18 ml, p = 0.04), also without further LA contractile function impairment (active emptying percentage of total volume) (36 ± 24% vs 36 ± 25% of total volume, p = NS). This effect was similar in paroxysmal and persistent AF. In conclusion, RFCA induces reductions in LA volumes without a deleterious impact on contractile function, even after repeated ablation.  相似文献

OBJECTIVES: We sought to investigate the imaging of the left atrial (LA) muscular bundle and the relationship between the bundle and inducibility of tachyarrhythmia after pulmonary vein isolation (PVI). BACKGROUND: Noninducibility is used as a clinical end point of atrial substrate ablation after PVI. However, little is known about the role of the LA muscular bundles in tachyarrhythmia after PVI. METHODS: Forty-three consecutive patients with paroxysmal atrial fibrillation who underwent catheter ablation were included. Bi-atrial isochronal mapping was performed with the NavX system (St. Jude Medical Inc., St. Paul, Minnesota) during sinus rhythm. After 4 PVI, inducible organized LA flutter with or without transforming to atrial fibrillation (AF) (LA flutter/AF) was ablated with additional lines at the roof and/or mitral isthmus. RESULTS: The existence of bilateral muscular bundles was an independent predictor of LA flutter/AF after PVI (p = 0.02). Patients with LA flutter/AF after PVI had a greater index of the double potentials (5.4 +/- 3.4% vs. 2.8 +/- 1.8%, p = 0.006) and interpotential interval (33 +/- 5 ms vs. 29 +/- 4 ms, p = 0.02) than without LA flutter/AF. The muscular bundles were identified in 28% patients using 16-slice multidetector computed tomography, which were identical to the isochrone map. Patients with noninducible LA flutter/AF after PVI plus the additional linear ablation had a lower recurrence rate as compared with the patients without it (19% vs. 75%, p = 0.02). CONCLUSIONS: Left atrial muscular bundles may provide a conduction block line and barrier, which is important for the formation of LA flutter/AF after PVI. The noninducibility of LA flutter/AF achieved after additional linear ablation may contribute to a better outcome in RF ablation of paroxysmal atrial fibrillation.  相似文献

Introduction: The atrial substrate plays an important role in the maintenance of atrial fibrillation (AF). Further investigation of the biatrial substrate may be helpful for understanding the mechanism of AF. The aim of this study was to investigate the properties of right and left atrial (RA and LA) substrate in AF patients and their impact on the catheter ablation.
Methods: Biatrial electroanatomic mapping using a three-dimensional mapping system (NavX) was performed in 117 consecutive patients with paroxysmal (n = 99) and persistent (n = 18) AF. The biatrial voltage and total activation time (TAT) were obtained during sinus rhythm.
Results: The LA had a lower voltage (1.6 ± 0.5 vs 2.0 ± 0.6 mV, P < 0.001) than the RA. The TAT correlated with the voltage (r =–0.65, P< 0.001). The patients with persistent AF had a lower atrial voltage, higher coefficient of variance for the LA voltage, longer LA TAT, and more extensive scar than those with paroxysmal. The patients with recurrent AF after catheter ablation had a lower LA voltage and higher incidence of LA scarring than those without recurrence. A scar located in the low anteroseptal or low posterior wall of LA was related to recurrence of AF. LA scarring was the independent predictor of AF recurrence after catheter ablation.
Conclusion: The LA voltage was lower than the RA, and the atrial voltage correlated with the TAT. Electroanatomical remodeling of the atria could be crucial to the maintenance of AF. The LA substrate properties may play an important role in the recurrence of AF after catheter ablation of AF.  相似文献

BACKGROUND: Data on combined permanent atrial fibrillation (pAF) surgery and coronary artery bypass grafting (CABG) and/or aortic valve replacement (AVR) are scarce, and the mid- and long-term effects on survival and cardiac rhythm are unknown. MATERIAL AND METHODS: In a prospective analysis 125 patients (Group I: CABG and/or AVR, n = 50; Group II: mitral valve [MV] surgery, n = 75) with pAF (> or = 6 months) underwent either concomitant monopolar (Group I: n = 20; Group II: n = 75) or bipolar (Group I: n = 30) radiofrequency (RF) ablation procedures. Group I patients had a significantly smaller left atrial (LA) size than Group II patients (LA-diameter: 47.7 +/- 4.6 vs. 58.2 +/- 6.1 mm; p < 0.01). Regular follow-up was performed from 3 to 36 months after surgery to assess survival, NYHA-class, and conversion rate to stable sinus rhythm (SR). RESULTS: Early mortality (< 30 days) of Group I patients was 0% (Group II: 2.7%), cumulative survival at long-term follow-up was 0.95 vs. 0.82 (p = 0.31) and NYHA-class improved significantly in both groups, particularly in cases with stable SR. At follow-up 80% of Group I patients had SR (Group II: 70%). In Group I patients the bipolar approach was associated with significantly shorter ablation procedure times compared to the monopolar procedure (12.1 +/- 3.4 vs. 18.9 +/- 1.6 min; p < 0.05). CONCLUSIONS: Concomitant pAF ablation surgery in CABG and/or AVR is safe and at least as effective as in MV surgery, presumably because severe LA enlargement is exceptionally rare in this group.  相似文献

There is evidence that parasympathetic denervation may prevent atrial fibrillation (AF) recurrences. This study aimed at applying an anatomic approach for ablation of atrial ganglionic plexi (GPs) in patients with paroxysmal AF. Nineteen patients with symptomatic, paroxysmal AF underwent anatomically guided radiofrequency ablation at the location of the 4 main left atrial GPs and were prospectively assessed for recurrence of AF or other atrial arrhythmia. This group was compared with 19 age- and gender-matched patients who previously underwent conventional circumferential pulmonary vein ablation. All ablation procedures were uneventful. Circumferential and GP ablations were accomplished with a radiofrequency delivery time of 28 +/- 5 versus 18 +/- 3 min (p <0.001) and a fluoroscopy time of 31 +/- 5 versus 18 +/- 5 min (p <0.001), respectively. Parasympathetic reflexes during radiofrequency ablation were elicited in 4 patients (21%). Arrhythmia recurred in 7 patients (37%) with circumferential ablation and 14 patients (74%) with GP ablation, during 1-year follow-up (p for log-rank test = 0.017). In 2 patients with GP ablation, left atrial flutters were documented in addition to AF during follow-up. Patients who underwent GP ablation had an almost 2.5 times higher risk of AF recurrence compared with those who underwent circumferential ablation (hazard ratio 2.6, 95% confidence interval 1.0 to 6.6, p = 0.038). In conclusion, anatomically guided GP ablation is feasible and safe in the electrophysiology laboratory, but this approach yields inferior clinical results compared with circumferential ablation.  相似文献

OBJECTIVES: This study reports the incidence of, risk factors for, and management of left atrial (LA) thrombus documented by intracardiac echocardiography (ICE) during LA ablation for atrial fibrillation (AF). BACKGROUND: Thrombus formation is a risk associated with LA ablation procedures. METHODS: Intracardiac echocardiography imaging was performed in 232 patients (184 men, average age 55 +/- 11 years) with AF undergoing pulmonary vein ostial ablation. RESULTS: Anticoagulation (activated clotting time >250 s) was maintained after dual transseptal catheterization. Left atrial thrombus (n = 30) was observed in 24 of 232 patients (10.3%). Thrombi measured 12.9 +/- 11.1 mm (length) and 2.2 +/- 1.3 mm (width) and were attached to a sheath or mapping catheter. Most thrombi (27 of 30, 90%) were eliminated from the LA by withdrawal of the sheath and catheter into the right atrium (RA). Two thrombi became wedged in the interatrial septum and incompletely withdrawn into the RA, and one was recognized only on post-procedure review of ICE images. Patients with LA thrombus had an increased LA diameter (4.8 +/- 0.5 vs. 4.5 +/- 0.6 cm, p < 0.02), spontaneous echo contrast (67% vs. 3%, p < 0.0001) and a history of persistent AF (29% vs. 6%, p < 0.0002). Multivariate discriminant analysis showed that spontaneous echo contrast (f = 97.9, p < 0.0001) was the most important determinant of LA thrombus formation. No patient with LA thrombus suffered a clinical thromboembolic complication. CONCLUSIONS: Left atrial thrombus identified on ICE may occur during LA catheter ablation procedures despite aggressive anticoagulation. Spontaneous echo contrast may predict risk for LA thrombus formation. Left atrial thrombus may be successfully withdrawn into the RA under ICE imaging with no overt complications.  相似文献

目的观察维持窦性心律(窦律)对慢性心房颤动(房颤)射频消融术后患者左心房和左心室结构的影响。方法入选38例慢性房颤行射频消融术的患者,分别于术前、术后1年行超声心动图检查,测量的超声心动图指标为左心房前后径、左心房上下径、左心房左右径、左心房最大容积、左心室舒张末内径、左心室收缩末内径,左心室射血分数,评估房颤有无复发对左心房及左心室重构的影响。结果 31例慢性房颤患者完成随访,随访时间为(13.45±1.46)个月,将其按消融效果分为复发组(15例)和非复发组(16例)。随访结果如下:(1)消融术前复发组与非复发组超声指标的基线资料比较,差异无统计学意义(P0.05)。(2)复发组术后12个月左心房前后径、左心房上下径、左心房左右径、左心房最大容积、左心室舒张末内径、左心室收缩末内径、左心室射血分数与术前比较,差异无统计学意义(P0.05);(3)非复发组消融术后12个月左心房前后径较术前减小[(38.73±3.77)mmvs.(41.86±4.73)mm,P0.01],左心房上下径较术前减小[(58.03±4.31)mmvs.(61.70±3.80)mm,P0.01],左心房左右径较术前减小[(43.93±6.06)mmvs.(46.08±6.62)mm,P0.01],左心房最大容积较术前减小[(75.78±22.27)mLvs.(83.18±24.29)mL,P0.01],左心室舒张末内径较术前减小[(45.85±4.98)mmvs.(48.26±5.36)mm,P0.01]、左心室收缩末内径较术前减小[(28.74±4.27)mmvs.(31.44±5.32)mm,P0.01],左心室射血分数较术前增加[68.03%±4.58%vs.62.75%±7.23%,P0.01],差异有统计学意义。结论维持窦律能使射频消融术后12个月的慢性房颤患者左心房及左心室逆向重构,左心室收缩功能改善。  相似文献
