目的探讨超声实时组织弹性成像定量分析对肝脏纤维化程度的诊断和鉴别价值。方法回顾性分析60例慢性乙型肝炎患者的临床资料,均经肝穿刺活检确诊,作为研究组。选取同期到本院体检的60例健康对象,作为对照组。两组均行超声实时组织弹性成像定量分析。以肝穿刺活检病理结果为标准,判断超声实时组织弹性成像定量分析对肝脏纤维化的诊断效能。并测算两组实时组织弹性成像参数。结果以肝穿刺活检病理结果为标准,超声实时组织弹性成像定量分析诊断灵敏度为94.74%,特异度为95.45%,准确度为95.00%;研究组平均相对应变值低于对照组,蓝色面积百分比、对比度、偏度、复杂度高于对照组,差异有统计学意义(P 0.05)。结论超声实时组织弹性成像定量分析可为肝脏纤维化程度诊断及鉴别提供较准确依据。  相似文献

目的探究高频超声弹性成像与超声造影技术在乳腺病灶鉴别中的应用价值。方法选取2017年7月~2019年4月我院甲乳外科接收的乳腺疾病患者110例,按照病理检查结果分为恶性组68例和良性组42例。两组患者均接受高频超声弹性成像技术和超声造影技术检查,对比单项检测和联合检测的恶性检出率、准确度、灵敏度、特异度。结果高频超声弹性成像与超声造影技术单独检测恶性检出率均小于病理检测,差异有统计学意义(P<0.05);联合检测恶性检出率与病理检测相比,差异无统计学意义(P>0.05);单独检测准确度、灵敏度、特异度均低于联合诊断,差异有统计学意义(P<0.05)。结论高频超声弹性成像与超声造影技术联合诊断可以有效提高乳腺病灶临床诊断的效果,可以作为乳腺疾病诊断的参考依据。  相似文献

目的分析常规超声与超声弹性成像对乳腺实性结节的诊断价值。方法选取2015年12月~2017年2月我院收治的乳腺实性结节患者93例。所有患者均进行常规超声(常规超声组)与超声弹性成像(超声弹性成像组),将常规超声联合超声弹性成像作为观察组,并以实性结节切除术后病理诊断为参考标准,将其作为对照组。观察并比较敏感度、特异度、准确度、阳性及阴性预测值。结果常规超声组敏感度、特异度、准确度、阳性及阴性预测值与超声弹性成像组比较,差异无统计学意义(P0.05);观察组敏感度为93.41%,与常规超声组的79.57%、超声弹性成像组的78.49%比较,差异有统计学意义(P0.05)。结论在乳腺实性结节的诊断过程中,常规超声与超声弹性成像均有一定的诊断价值,两者联合应用能够更好地提高该疾病诊断的敏感性。  相似文献

目的探析彩色多普勒超声结合弹性成像技术用于乳腺肿块定性诊断的价值。方法选取我院2019年1月至2021年12月收治的60例乳腺肿块患者,病例结果已“金标准”检测确诊,其中乳腺肿块是43例,并对其实施单一诊断、联合诊断,对比诊断准确率、漏诊、误诊率、特异度与灵敏度。结果彩色多普勒超声+弹性成像技术诊断组的准确率、误诊与漏诊率较之彩色多普勒超声诊断组、弹性成像技术诊断组,差异有统计学意义(P<0.05);彩色多普勒超声+弹性成像技术诊断组的特异度与灵敏度较之彩色多普勒超声诊断组、弹性成像技术诊断组,差异有统计学意义(P<0.05)。结论与单一诊断方式对比,彩色多普勒超声结合弹性成像技术联合诊断可以提升整体的恶性乳腺肿块与良性乳腺肿块诊断准确率,促进特异度与诊断灵敏度提升,减少误诊与漏诊。  相似文献

目的:探讨超声弹性成像与常规超声诊断甲状腺良恶性结节的效果观察及敏感性影响。方法:选取本院2017年12月至2018年12月收治的140例甲状腺结节患者(175个结节)作为研究样本,其中恶性结节74个,良性占位101个,均接受常规超声诊断(常规超声组)与超声弹性成像技术诊断(超声弹性成像组),比较两组患者的良恶性结节诊断结果、诊断符合率、灵敏度、特异度、阳性预测值、阴性预测值等。结果:超声弹性成像组的诊断符合率89.71%高于常规超声组的66.29%、灵敏度87.19%高于常规超声组的58.12%,特异度90.10%高于常规超声组的72.28%,阳性预测值86.84%高于常规超声组的60.56%、阴性预测值91.92%高于常规超声组的70.19%,组间差异有统计学意义(P<0.05)。结论:甲状腺良恶性结节应用超声弹性成像诊断较常规超声诊断准确性相对更高,且具有较高的灵敏度和特异度,值得推广与应用。  相似文献

选取我院2013年3月~2014年3月收治的80例甲状腺结节患者,所有患者分别进行超声弹性成像诊断、常规超声诊断及超声弹性成像与常规超声联合诊断,并分别作为弹性成像组、常规组及联合组,诊断结束后,与病理诊断结果对照,分析三组的良恶性结节诊断符合率。结果弹性成像组良性结节的诊断符合率为85.71%、恶性结节的诊断符合率为78.18%,常规超声诊断组良性结节的诊断符合率为77.14%、恶性结节的诊断符合率为72.73%,联合诊断组良性结节的诊断符合率为97.14%、恶性结节的诊断符合率为96.36%,联合组的诊断符合率高于弹性成像组和常规超声组,差异具有统计学意义(P0.05)。超声弹性成像在一定程度上对甲状腺良恶性结节具有诊断价值,但为了进一步确保临床诊断的准确性,可在诊断甲状腺良恶性结节的过程中应用超声弹性成像联合常规超声诊断。  相似文献

Background

Quantifying passive stretching responses of individual muscles helps the diagnosis of muscle disorders and aids the evaluation of surgical/rehabilitation treatments. Utilizing an animal model, we demonstrated that shear elastic modulus measured by supersonic shear wave elastography increases linearly with passive muscle force. This study aimed to use this state-of-the-art technology to study the relationship between shear elastic modulus and ankle dorsi–plantarflexion angle of resting tibialis anterior muscles and extract physiologically meaningful parameters from the elasticity–angle curve to better quantify passive stretching responses.

Methods

Elasticity measurements were made at resting tibialis anterior of 20 healthy subjects with the ankle positioned from 50° plantarflexion to up to 15° dorsiflexion at every 5° for two cycles. Elasticity–angle data was curve-fitted by optimizing slack angle, slack elasticity, and rate of increase in elasticity within a piecewise exponential model.

Findings

Elasticity–angle data of all subjects were well fitted by the piecewise exponential model with coefficients of determination ranging between 0.973 and 0.995. Mean (SD) of slack angle, slack elasticity, and rate of increase in elasticity were 10.9° (6.3°), 5.8 (1.9) kPa, and 0.0347 (0.0082) respectively. Intraclass correlation coefficients of each parameter were 0.852, 0.942, and 0.936 respectively, indicating excellent test–retest reliability.

Interpretation

This study demonstrated the feasibility of using supersonic shear wave elastography to quantify passive stretching characteristics of individual muscle and provided preliminary normative values of slack angle, slack elasticity, and rate of increase in elasticity for human tibialis anterior muscles. Future studies will investigate diagnostic values of these parameters in clinical applications.  相似文献

目的 探讨实时剪切波弹性成像评价腰椎间盘突出症患者腓肠肌弹性的价值。方法 选取临床确诊为单侧腰椎间盘突出症且接受侧隐窝入路选择性神经根阻滞联合臭氧消融术及脉冲射频治疗的患者100例,采用实时剪切波弹性成像技术检测治疗前后紧张状态下双侧小腿腓肠肌的平均(EMean)及最大(EMax)弹性模量值(kPa),并进行统计分析。结果 治疗前患侧紧张状态腓肠肌EMean为(11.28±2.60)kPa,EMax为(15.26±2.63)kPa,均较健侧减低(t=78.241、64.634,P均<0.001)。治疗后患侧紧张状态腓肠肌EMean为(13.18±2.38)kPa,EMax为(17.63±2.73)kPa,均较治疗前增高(t=6.407、14.815,P均<0.001)。不同肌力状态下,治疗前和治疗后紧张状态腓肠肌的EMean、EMax值差异均有统计学意义(P均<0.001)。治疗前及治疗后当肌力增加时,紧张状态腓肠肌EMean、EMax值亦增加,不同肌力间两两比较差异均有统计学意义(P均<0.05)。结论 通过实时剪切波弹性成像技术检测腰椎间盘突出患者治疗前后紧张状态下腓肠肌的EMean和EMax,可定量分析肌肉组织恢复情况。  相似文献

This study investigated the acute effects of static stretching (SS) on shear elastic modulus as an index of muscle hardness and muscle stiffness and the relationship between change in shear elastic modulus and change in muscle stiffness after SS. The patients were 17 healthy young males. Muscle stiffness was measured during passive ankle dorsiflexion using a dynamometer and ultrasonography before (pre) and immediately after (post) 2 min of SS. In addition, shear elastic modulus was measured by a new ultrasound technique called ultrasonic shear wave elastography. The post-SS values for muscle stiffness and shear elastic modulus were significantly lower than the pre-SS values. In addition, Spearman's rank correlation coefficient indicated a significant correlation between rate of change in shear elastic modulus and rate of change in muscle stiffness. These results suggest that SS is an effective method for decreasing shear elastic modulus as well as muscle stiffness and that shear elastic modulus measurement using the shear wave elastography technique is useful in determining the effects of SS.  相似文献

Forty-five study participants (28 chronic heart failure [CHF] patients and 17 control participants) were prospectively enrolled in this study to investigate the clinical potential of ultrasound shear wave elastography (SWE) in identifying peripheral muscle weaknesses in chronic heart failure patients. Muscle stiffness in the gastrocnemius muscle during extension (stretch) and the lower arm flexor muscles during flexion was assessed via shear wave elastography, measuring the shear modulus (kPa) for the resting and contractile states in a range of 0–300 kPa. Resting kPa revealed no significant difference between CHF and CP, but exercise kPa for extension and flexion was significantly lower in CHF than CP. The area under the receiver operating characteristic curve of the denominator kPa-Exercise stretch was 0.916, associated with a sensitivity of 89%, a specificity of 71% and a corresponding cutoff value of 81.1 kPa. Shear wave elastography is thus a reproducible and sensitive ultrasound method for evaluating peripheral muscle deficits in patients with CHF.  相似文献

目的应用实时剪切波弹性成像及彩色脉搏波成像技术分析高血压患者颈总动脉纵向及环向血管壁硬度,比较不同级别高血压患者颈总动脉血管壁硬度差异,探讨2种技术在高血压分级诊断中的应用价值。 方法选取2016年2至12月云南省第二人民医院收治的临床确诊的高血压患者120例。根据中国高血压防治指南(2010年版)分级分为1、2、3级高血压组3组各40例。另选取同期云南省第二人民医院体检无高血压病史的健康志愿者40名作为健康对照组。应用实时剪切波弹性成像技术获得所有研究对象颈总动脉前壁及后壁的弹性模量值(Mean、Max、Min),得到颈动脉的纵向血管壁硬度;应用彩色脉搏波成像技术获得所有研究对象颈总动脉壁的脉搏波速度,包括收缩期开始时的脉搏波速度(PWVBS)、收缩期结束时的脉搏波速度(PWVES),得到颈动脉的环向血管壁硬度。采用单因素方差分析比较不同级别高血压组患者与健康对照组受试者颈总动脉壁硬度值(Mean、Max、Min、PWVBS、PWVES),进一步组间两两比较采用LSD-t检验;采用Pearson相关分析法分析高血压患者颈动脉壁硬度值(Mean、Max、Min、PWVBS、PWVES)与高血压级数的相关性。 结果颈总动脉(前壁及后壁)的弹性模量值(Mean、Max、Min)均为高血压3级组>高血压2级组>高血压1级组>健康对照组,且任意2组间比较差异均有统计学意义。Pearson相关分析结果显示,颈总动脉(前壁及后壁)的弹性模量值(Mean、Max、Min)与高血压级数均有相关性(r=0.817、0.767、0.796,P均<0.05)。随着高血压级数的增高,高血压患者颈总动脉(前壁及后壁)的弹性模量值(Mean、Max、Min)呈增高趋势。颈总动脉脉搏波速度(PWVBS、PWVES)均为高血压3组>高血压2组>高血压1组>健康对照组,且任意2组间比较差异均有统计学意义。Pearson相关分析结果显示,脉搏波速度(PWVES、PWVBS)与高血压级数均有相关性(r=0.650、0.585,P<0.05)。随着高血压级数的增高,高血压患者脉搏波速度(PWVBS、PWVES)呈增高趋势。 结论实时剪切波弹性成像及彩色脉搏波成像技术为高血压分级诊断提供了一种有价值的参考。随着高血压级数的增高,高血压患者颈总动脉壁的纵向及环向血管壁硬度均呈增高趋势,且两者具有一致性。  相似文献

BackgroundThe association between decreased muscle strength of rate of force development (RFD) immediately after static stretching (SS) and change in muscle stiffness or muscle slack has remained unclear.ObjectiveThis study aimed to investigate the association between changes in muscle strength and RFD and muscle stiffness or muscle slack immediately after SS.MethodsSixteen healthy male non-athlete volunteers participated in this study. The maximal voluntary contraction torque and shear elastic modulus of medial gastrocnemius muscle were measured before and after 300 s of SS intervention.ResultsThe results showed that maximal voluntary contraction torque and RFD at 100, 150, and 200 ms decreased significantly after the 300-s SS (p < 0.01). Additionally, shear elastic modulus at 0° (decrease in muscle stiffness) and slack angle decreased (increase in slack length) significantly after the 300-second SS (p < 0.01). However, no significant association was observed between changes in muscle strength and changes in mechanical properties of the MG.ConclusionThese results suggested that the decrease in muscle strength and RFD could not be associated with changes in the passive mechanical properties of the medial gastrocnemius muscle.  相似文献

目的评价剪切波弹性成像技术对子痫前期的诊断价值。 方法前瞻性纳入2019年10月至2020年12月在常州第二人民医院超声科检查的晚孕期孕妇177名。根据临床诊断将孕妇分为健康组152名和子痫前期组25例。采用单因素分析比较2组间一般临床资料、二维超声、彩色多普勒超声参数及胎盘(母体面和胎儿面)杨氏模量值(Emean、Emax、Emin)间的差异。采用Spearman相关分析胎盘杨氏模量值与胎儿脐血流参数间的相关性,并建立受试者工作特征曲线,评价胎盘杨氏模量值诊断子痫前期的诊断效能。 结果健康组与子痫前期组孕妇的羊水指数、胎盘厚度以及脐动脉血流收缩/舒张期速度比(S/D)、阻力指数(RI)、搏动指数(PI)比较,差异均无统计学意义(P均>0.05)。除胎盘母体面Emin以外,子痫前期组孕妇胎盘母体面Emean、Emax及胎儿面Emean、Emax、Emin均高于健康组[(5.87±0.41)kPa vs(5.30±0.53)kPa;8.67(8.30,9.66)kPa vs 8.01(7.56,8.50)kPa;(5.15±0.34)kPa vs(4.65±0.52)kPa;(8.07±0.70)kPa vs(7.11±0.85)kPa;(3.28±0.44)kPa vs(3.01±0.54)kPa],差异具有统计学意义(t=-6.215、Z=-4.566、t=-6.298、t=-5.376、t=-2.430,P<0.001、<0.001、<0.001、<0.001、=0.016)。健康组和子痫前期组孕妇胎盘自身的母体面Emean、Emax、Emin均高于胎儿面,差异具有统计学意义(t=11.319、Z=-8.461、t=12.118、t=6.266、Z=-3.807、t=5.416,P均<0.001)。相关性分析显示2组中各杨氏模量值与脐动脉血流的S/D、RI、PI值均无显著相关性(P均>0.05)。当截断值为5.85 kPa时,胎盘母体面的Emean值诊断子痫前期的效能最佳,其敏感度、特异度、准确性分别为68.0%、85.5%、83.1%。 结论子痫前期孕妇胎盘的杨氏模量值高于健康孕妇。剪切波弹性成像技术可通过评价胎盘硬度辅助子痫前期的诊断。  相似文献

The aim of this study was to evaluate the performance of passive muscle stiffness in diagnosing and assessing disease progression in Duchenne muscular dystrophy (DMD). Boys with DMD and age-matched controls were recruited. Shear wave elastography (SWE) videos were collected by performing dynamic stretching of the gastrocnemius medius (GM). At ankle angles from plantar flexion (PF) 30° to dorsiflexion (DF) 20°, the shear modulus of the GM was measured for each 10° of ankle movement. Shear modulus at each ankle angle was compared between the DMD and control group. Correlation between passive muscle stiffness and motor function grading was also analyzed. A total of 26 patients with DMD and 20 healthy boys were enrolled. At multiple stretch levels, passive muscle stiffness of the GM was significantly higher in patients with DMD than in those in the control group (all p values <0.05). The shear modulus of GM at an ankle angle of DF 10° had the largest area under the receiver operating characteristic curve in differentiating DMD patients from normal subjects (AUC = 0.902, 95% confidence interval: 0.814–0.990). Motor function grading was a significant determinant of passive muscle stiffness at an ankle angle of DF 10° (B = 21.409, t = 3.372, p = 0.003). Passive muscle stiffness may potentially serve as a useful non-invasive tool to monitor disease progression in DMD patients.  相似文献

目的探讨剪切波弹性成像技术对布加综合征(BCS)患者肝损害的评价作用。 方法收集2013年5月至2015年7月在广州中医药大学附属第一医院诊治的BCS患者16例,所有患者的诊断结果均经介入治疗或下腔静脉造影证实;收集在广州中医药大学附属第一医院行常规体检的健康志愿者50名,所有志愿者均无任何肝病史、手术史及糖尿病等代谢性疾病。所有研究对象均行常规二维超声检查,且二维超声显示肝脏声像图无明显异常表现。对BCS患者及健康志愿者行肝脏超声弹性检查,分析超声弹性图像特征并获得定量弹性值,对比分析BCS患者及健康志愿者肝脏弹性值之间的差异。 结果健康志愿者弹性图像表现为均匀一致的浅蓝色。BCS患者的弹性图像多表现为蓝、绿、黄三色相间的混杂图像。BCS患者肝脏弹性均值、最大值及最小值分别为(18.1±9.4)、(20.6±10.5)及(15.5±8.5)kPa;健康志愿者肝脏弹性均值、最大值及最小值分别为(6.2±1.0)、(7.5±1.2)及(4.8±1.1)kPa;BCS患者与健康志愿者肝脏弹性均值、最大值及最小值比较,差异均有统计学意义(t值=7.69、8.94、7.83,P均<0.01)。 结论剪切波弹性成像技术可用于早期诊断二维超声图像无异常改变的BCS肝损害患者,对患者治疗方式的选择、疗效及预后评价有一定作用。  相似文献
