Two cases of ameloblastic carcinoma of the jaws are reported. Histopathologically, the lesions showed cytologic features of malignancy in addition to classical ameloblastoma patterns and were therefore documented as examples of ameloblastic carcinoma. The negative cytokeratin expression by the malignant cells on histochemical analysis is notably different from that normally observed in classical ameloblastomas.  相似文献

A case of an ameloblastic carcinoma in an unusual situation in the maxilla is presented. The lesion is characterized histologically by areas with features of a typical ameloblastoma and areas with anaplastic transformation. A Caldwell-Luc operation was followed by a recurrence after 3 years. Due to the advanced age and poor general health of the patient an enlarged, but not radical operation was performed. There is no evidence of metastatic tumour.  相似文献

Two cases of ameloblastic fibroma are described. One case was a seven-year-old girl, the other a 13-year-old boy. In both cases the tumour was located in the posterior part of the maxilla. Both neoplasms were associated with failure of tooth eruption, in one case a primary molar, in the other case a permanent molar. Local excision was performed in both cases, with a recurrence observed in one case ten months after surgery.  相似文献

Two cases of unusually aggressive osteomyelitis of the jaws are reported. Both cases manifested severe, rapidly progressive lytic destruction of the maxilla in spite of the fact that patients were young, healthy adults with no local or systemic factors that would predispose them to severe infection in either case. Characteristic historical, clinical, radiographic, and histologic features are elaborated. Also, theoretical aspects of this disease entity are discussed in an effort to explain the unusually aggressive manner in which these cases presented.  相似文献

Summary. The ameloblastic fibroma is an odontogenic tumour with an unusual presentation. It is considered to have both epithelial and mesenchymal elements, but it läcks any calcifled dental structures. Two new cases are presented. The first of these began as a mandibular tumour whereas the second was discovered by chance. The radiographic finding in each case was a unilocular cystic radiolucency. Treatment consisted of excisional biopsy by enucleation in both patients. Histopathological findings were consistent with ameloblastic fibroma.  相似文献

A case of maxillary ameloblastic carcinoma metastatic to the mandible is presented. Of 33 cases of ameloblastic carcinoma reported in the English literature 10 have occurred in the maxilla. Of these, none produced mandibular metastases. The authors review the literature, describing clinical presentation, histological appearance, and treatment of this rare lesion.  相似文献

Bisphosphonates are recently acquiring increasing relevance in the treatment of several diseases. In line with the increased use of these compounds, cases of mandibular osteonecrosis, and to a lesser extent, maxillary osteonecrosis, are being reported. This necrosis is difficult to treat in patients who usually have a previously limited quality of life. A surgical performance carried out by oral and maxillofacial surgeons, stomatologists and odontologists might lead to bone exposure. A treatment based on conservation and as harmless as possible seems to be the most advisable way of acting with these patients in order to minimize the incidence and treat the complications, once the lesions have been ascertained. We report three cases treated in our service of osteonecrosis of the jaws after exodontics. This side effect should be remembered before starting any surgical treatment in these patients.  相似文献

The histologic classification for odontogenic carcinomas is still under revision; thus, the differentiation between the terms "malignant ameloblastoma" and "ameloblastic carcinoma" has not been definitely stated. Nevertheless, it is recommended to reserve the former for those lesions that, in spite of an apparently innocuous histology, have given origin to metastatic growths, and to apply the latter for those ameloblastomas in which there is histologic evidence of malignancy in the primary, recurrent or metastatic lesions. A case of an ameloblastic carcinoma in the mandible is presented. Histologically, it was characterized by areas with features of a typical ameloblastoma and areas with anaplastic appearances.  相似文献

Bisphosphonates are commonly used in medicine to maintain bone density in patients with certain nonneoplastic diseases or cancers. A serious adverse effect of bisphosphonates that has substantial dental significance is osteonecrosis that appears to uniquely affect the mandible and maxilla without occurring in other bones of the skeleton. Patients with bisphosphonate associated osteonecrosis of the jaws may present with pain and exposed necrotic bone. This has substantial clinical implications because surgical procedures (including extractions or endodontic surgical procedures) are contraindicated in the jaws of these patients and the presenting pain may mimic pain of odontogenic origin. This report describes three patients with bisphosphonate associated osteonecrosis and emphasizes the endodontic implications of managing these patients.  相似文献

Five cases of ameloblastic fibrosarcomas (AFS) are reported. The tumour was characterized histologically by a biphasic pattern: the malignant mesenchymal component had the features of an intermediate grade fibrosarcoma in 3 cases, malignant fibrous histiocytoma and osteogenic sarcoma in 2 cases. The epithelial odontogenic component had a benign appearance cytologically. In 1 patient, in the recurrence only the malignant mesenchymal component was present.AFS is a fully malignant tumour, in fact 1 patient died of the tumour after inadequate surgical treatments, and 2 patients had a recurrence after intralesional surgery.The treatment of choice was achieved when surgery with wide surgical margins was performed. As MFH and OGS features are present in the malignant mesenchymal component of this tumour we prefer to use the broad term ameloblastic sarcoma instead of AFS.  相似文献

Ameloblastic fibrosarcoma is very rare and has not previously been reported from Nigeria. The case described here had typical clinical features, but the microscopic findings were unusual and difficult to interpret. The pathogenetic relationship between ameloblastic fibroma and fibrosarcoma is discussed.  相似文献
