Hemiarthroplasty has been successfully used for treatment of femoral neck fractures in the elderly. With increased demand being placed on the prostheses, some of them failed and had to be converted to total hip replacement. The aim of this study is to evaluate the result of conversion arthroplasty in Egyptian population. We report 47 patients who had conversion to total hip arthroplasty following failed hemiarthroplasty. There were 32 women and 15 men. The mean age was 64.3 years (range 54 to 83 years); the average follow-up was 44 months (range 24 to 84 months). The main indication for conversion was either acetabular erosion or stem loosening. Harris hip score improved from a mean of 40.9 points preoperatively to a mean of 86.0 points at the last follow-up. Ninety four percent of the patients had either excellent or good results. Eight patients had nine complications, but there were no instances of instability or loosening. Conversion total hip arthroplasty appears as an excellent management strategy for failed hemiarthroplasty.  相似文献

BACKGROUND: Nonunion after a periprosthetic femoral fracture associated with total hip arthroplasty occurs rarely. There is little information, to our knowledge, regarding the prevalence of this complication, its treatment, and the functional outcomes of treatment. The purpose of this study was to identify the patterns and frequency of nonunions of femoral fractures around total hip prostheses and to evaluate the results and problems associated with treatment of this complication in a consecutive series of patients. METHODS: The study included twenty-three nonunions of periprosthetic femoral fractures in twenty-three patients with an average age of fifty-five years (range, twenty-two to eighty-five years) at the time of the initiation of treatment of the nonunion. Thirteen of the fractures occurred during or after a primary total hip arthroplasty, and ten occurred during or after a revision total hip arthroplasty. According to the classification system of Duncan and Masri, there were six B1 fractures (associated with a well fixed prosthesis), seven B2 fractures (associated with a loose stem), and ten B3 fractures (associated with very poor proximal bone). Ten patients were managed with revision to a long-stem prosthesis. Six patients had revision to a proximal femoral replacement prosthesis. A two-stage technique consisting of removal of the prosthesis and open reduction and internal fixation of the nonunion followed by reimplantation of the prosthesis was used in two patients. Two patients were managed initially with bone-grafting alone, and two patients were managed nonoperatively. One patient who had an infection at the site of the nonunion was managed definitively with resection arthroplasty. RESULTS: The duration of clinical follow-up averaged 8.3 years (range, three months to twenty-three years), and that of radiographic surveillance averaged 7.0 years (range, eight months to seventeen years). Of the thirteen patients in whom an attempt to achieve union was made and for whom radiographs were available, nine eventually had bone-healing. Five of the twenty-three femora became infected and were treated with resection arthroplasty. Of the seventeen patients who had not had a resection arthroplasty for infection and for whom radiographs were available at the time of the most recent follow-up, eleven had a stable and well fixed implant and six had a loose implant as seen radiographically or had had a revision because of aseptic loosening. Seventeen patients had no or mild pain at the time of the most recent follow-up, but ten required two-handed support to walk. The overall complication rate was 52 percent (twelve of twenty-three patients). CONCLUSIONS: Nonunion of a femoral fracture associated with a total hip prosthesis is an infrequent problem. Treatment is difficult, with a high rate of complications and relatively poor functional outcomes. The data from this series must be interpreted with caution, as patients were managed over a period of three decades and many did not have the advantage of modern techniques of revision hip arthroplasty. Prevention of nonunion by optimum treatment of the initial fracture is most important. Treatment of a femoral nonunion about a total hip implant should be implemented on the basis of the status of the fixation of the prosthesis and the quality of the surrounding bone.  相似文献

Secondary total hip replacement after fractures of the femoral neck   总被引:5,自引:0,他引:5  
We studied the rate of revision in 84 consecutive total hip replacements performed for failed osteosynthesis of femoral neck fractures and compared it with that for primary arthroplasty for osteoarthritis. The age and sex adjusted risk of prosthetic failure was 2.5 times higher after failure of fixation, but all the excess risk was in patients over 70 years of age. There were radiographic signs of loosening of the femoral component at five to 12 years after secondary arthroplasty in six of 33 survivors. In general, the results of secondary replacement were no worse than those obtained after primary arthroplasty for femoral neck fracture. We consider that internal fixation should be the primary procedure: total hip replacement is a safe secondary procedure when osteosynthesis fails.  相似文献

BACKGROUND: Revision of a total hip arthroplasty in a patient who has had congenital hip dysplasia or dislocation is often more difficult than a standard revision operation. The purpose of this study was to assess the efficacy and complications of use of a cementless hemispherical acetabular component for revision of an acetabular component of a failed total hip replacement in patients whose initial problem was arthritis secondary to congenital dislocation or dysplasia. The mean duration of follow-up was approximately eight years. METHODS: We reviewed a consecutive series of sixty-one hips in fifty-three patients who underwent a cementless acetabular revision with use of a hemispherical acetabular component, with or without concurrent femoral revision. Data were collected prospectively. The mean age of the patients at the time of the index operation was fifty-six years. A mean of 1.9 ipsilateral hip operations had been performed previously. Thirty-nine hips (64 percent) had a so-called high hip center prior to the index revision. With one exception, the uncemented acetabular component was fixed with screws. Fifty-one acetabular components were placed with so-called line-to-line fit, and ten were oversized by one to three millimeters. In thirty-eight hips, the femoral component was revised as well. Twenty-nine femora were reconstructed with use of a cemented device, and nine were revised with an uncemented patch-porous-coated femoral stem (a stem on which the porous coating appears in patches). RESULTS: Four patients (five hips) died prior to the five-year minimum follow-up interval. With the exception of one hip treated with resection arthroplasty because of deep infection, none of the hips in these deceased patients had been revised or had a loose component. One living patient (one hip) had a resection arthroplasty, and one additional patient (two hips) had both stable acetabular components rerevised at the time of femoral rerevision at another institution because of loosening and osteolysis. One patient refused to return for follow-up, but the components had not been revised. The remaining fifty-two hips in forty-six patients were followed for a mean of 8.6 years (range, 5.0 to 12.7 years). The mean Harris hip score was 80 points (range, 56 to 100 points) at the time of the latest follow-up. No acetabular component had been revised, although two had migrated. No other acetabular component was loose according to our radiographic criteria. Thus, the mechanical failure rate on the acetabular side was 3 percent (two of sixty-one) for the entire series and 4 percent (two of fifty-two) for the patients who had been followed for a mean of 8.6 years. On the femoral side, the mechanical failure rate was 3 percent (one of twenty-nine) for the cemented stems and six of nine for the uncemented patch-porous-coated stems. CONCLUSIONS: Of the approaches used in this difficult series of patients requiring revision, the hybrid arthroplasty (a cementless acetabular component and a cemented femoral component) yielded overall good results after an intermediate duration of follow-up.  相似文献

Background:

Most proximal femoral fractures are successfully treated with internal fixation but a failed surgery can be very distressing for the patient due to pain and disability. For the treating surgeon it can be a challenge to perform salvage operations. The purpose of this study was to evaluate the short-term functional outcome and complications of total hip arthroplasty (THA) following failed fixation of proximal hip fracture.

Materials and Methods:

In a retrospective study, 21 hips in 20 patients (13 females and seven males) with complications of operated hip fractures as indicated by either established nonunion or fracture collapse with hardware failure were analysed. Mean age of the patients was 62 years (range 38 years to 85 years). Nine patients were treated for femoral neck fracture, 10 for intertrochanteric (I/T) fracture and two for subtrochanteric (S/T) fracture of the hip. Uncemented THA was done in 11 cases, cemented THA in eight hip joints and hybrid THA in two patients.

Results:

The average duration of follow-up was four years (2-13 years). The mean duration of surgery was 125 min and blood loss was 1300 ml. There were three dislocations postoperatively. Two were managed conservatively and one was operated. There was one superficial infection and one deep infection. Only one patient required a walker while four required walking stick for ambulation. The mean Harris Hip score increased from 32 preoperatively to 79 postoperatively at one year interval.

Conclusion:

Total hip arthroplasty is an effective salvage procedure after failed osteosynthesis of hip fractures. Most patients have good pain relief and functional improvements inspite of technical difficulties and high complication rates than primary arthroplasty.  相似文献

BACKGROUND: Recent randomized, controlled trials performed at two years postoperatively have shown that a primary total hip replacement is superior to internal fixation for the treatment of a displaced femoral neck fracture in a relatively healthy, mentally competent, elderly patient. The primary aim of the present study was to evaluate the outcomes at four years. METHODS: One hundred and two patients (mean age, eighty years) who had an acute displaced femoral neck fracture were randomly allocated to be treated with total hip replacement or internal fixation. The inclusion criteria were an age of at least seventy years, absence of severe cognitive dysfunction, an independent living status, and the ability to walk independently. The main outcome measurements were hip complications, reoperations, hip function, and health-related quality of life. RESULTS: The mortality rate was 25% in both groups. At the forty-eight-month follow-up evaluation, the rate of hip complications was 4% in the patients treated with total hip replacement and 42% in those treated with internal fixation (p < 0.001) and the reoperation rates were 4% and 47%, respectively (p < 0.001). The arthroplasty group had no additional hip complications or reoperations between the twenty-four and forty-eight-month follow-up visits. In the fixation group, the percentage of hip complications increased from 36% to 42% and the percentage of reoperations increased from 42% to 47% during the same period. The hip function was significantly better and the decline in health-related quality of life was less pronounced in the arthroplasty group than it was in the fixation group at the four, twelve, and twenty-four-month follow-up evaluations. Ninety-seven percent of the patients in the arthroplasty group and 57% of the patients in the fixation group who were available for follow-up at forty-eight months had no hip complications (p < 0.001). CONCLUSIONS: Compared with internal fixation, primary total hip replacement provides a better outcome for mentally competent elderly patients with a displaced femoral neck fracture. The complication and reoperation rates were significantly lower and hip function and health-related quality of life were at least as good at four years after the surgery.  相似文献

Periprosthetic fractures of the femur after total hip arthroplasty are a big orthopaedic problem, particularly in elderly patients and quite a challenge for orthopaedic surgeons. There is no universal method in treating these fractures. Rigid plates fixation can be limited and aggravated especially in the proximal part of the femur where the endoprosthesis stem does not allow for an undisturbed fixation of both femur cortexes by means of screws. Mitkovic's dynamic internal fixator is an original implant allowing for an undisturbed fixation of both femur cortexes regardless of the presence of the endoprosthesis stem. Fixation is made possible by means of movable clamps and a convergent possibility to place screws. A dynamic internal fixator can fix all types of periprosthetic femoral fractures. The paper shows the early experience in fixating periprosthetic femoral fractures after total hip arthroplasty in 14 patients, average age 69.7. According to Vancouver classification, 3 patients had the type A fracture, 9 patients had the type B fracture, and 2 patients had the type C fracture. All fractures were fixed by Mitkovic's dynamic internal fixator. The fracture occurred 2-12 years after primary total hip arthroplasty. The follow-up of the operated patients was 12 months. The method is less invasive than the methods described in books. Mechanical complications are not likely to appear due to the fracture dynamics along the femoral shaft axis, which is made possible by this implant. Our initial experience in femur fracture fixation after hip arthroplasty ahows that it is modern and effective dynamic implant which will contribute significantly to the improving of the treatment of these often very complicated fractures.  相似文献

Seventy-six hips in 67 patients were evaluated an average of 119 months (range, 61-150 months) after total hip arthroplasty with porous coated Omnifit femoral and acetabular components. The patients were young (average age, 45 years), and most were male (67%). Two stems and one cup were revised for aseptic loosening, for aseptic revision rates of 2.6% on the femoral side and 1.3% on the acetabular side. Thigh pain was present in three cases, one of which was activity limiting. Twenty-five (35.7%) hips had evidence of osteolysis confined to proximal Gruen Zone 1 or 7 or to the acetabulum (22 proximal femoral, three both). There were no cases of intramedullary osteolysis in surviving stems. Thirteen (17.1%) hips have undergone reoperation for bone grafting of progressive proximal osteolysis without component revision, at an average 93 months after the total hip arthroplasty. At an average 40 months after reoperation, all stems remain well fixed, and there has been no recurrence of osteolysis of grafted femoral lesions. These results suggest that a circumferentially proximally porous coated femoral component in cementless total hip arthroplasty can provide stable fixation for as long as 12 years after implantation and caseal the canal from distal osteolysis. Serious concerns remain about the incidence of proximal femoral osteolysis.  相似文献

目的:探讨变异髋臼初次全髋人工关节置换术中髋臼假体的正确放置位置及手术疗效。方法:对34例(38髋)接受全髋关节置换的髋臼变异的各类髋关节疾病进行术前设计,其中男20例,女14例,平均年龄56.1岁(29~75岁)。股骨头坏死继发骨性关节炎15例(19髋),髋臼发育不良继发骨性关节炎12例,创伤性骨关节炎5例,髋关节融合术后1例,髋关节人工股骨头置换术后1例。结果:术后患者均获得随访,平均随访11个月(5~38个月)。根据Harris髋关节功能评分评定,优(>90分)12髋,良(80~90分)23髋,尚可(70~79分)3髋,失败0髋(<70分)。评定结果:术前Harris评分平均47.9分,术后平均90.3分。结论:对于髋臼解剖结构异常的髋关节疾病患者行全髋人工关节置换时,通过术前对髋臼正确位置的设计,使髋关节中心置于正确的位置上,既可简化术中操作的难度,又可以使臼杯假体得到牢固固定及良好的骨覆盖,有利于人工全髋关节的长期疗效。  相似文献

OBJECTIVES: To assess the rate of anatomic reconstructions as well as approach-related morbidity and complications in the treatment of complex acetabular fractures through a modified extended iliofemoral approach. DESIGN: Prospective clinical study. SETTING: Level I trauma center, University Hospital. PATIENTS: Inclusion criteria were as follows: (a) associated acetabular fracture or transverse fracture with comminuted roof area stated as not sufficiently reconstructable through a single approach, and (b) age between sixteen and sixty-five years. A total of forty-nine patients with fifty complex acetabular fractures could be included out of the series of ninety-six acetabular fractures treated operatively from August 1992 to February 1996. Open reduction and internal fixation of complex acetabular fractures through the modified extended iliofemoral approach were performed. RESULTS: In 80 percent of the fifty fractures the reduction was anatomic with a remaining displacement of less than or equal to one millimeter, in eight cases there was a persistent displacement of two millimeters, and two fractures had a poor result with a three-millimeter displacement. Complications included 8 percent loss of reduction, 13 percent heterotopic ossification grade 3, and 4 percent avascular femoral head necrosis. At the two-year follow-up there were 74 percent good or excellent radiographic and clinical results. Two patients had already been reoperated with total hip replacement, and the two patients with femoral head necrosis are currently scheduled for arthroplasty. CONCLUSIONS: The modified extended iliofemoral approach proved to be appropriate to achieve anatomic reduction in complex acetabular fractures. The high rate of approach-related morbidity has to be considered carefully and may lead to a decreased incidence of extended approaches.  相似文献

目的探讨髋臼骨折经内固定术后出现并发症而导致疗效不佳,行全髋关节置换(TH R)的方法和疗效。方法总结经切开复位内固定治疗的髋臼骨折患者11例,因术后27~63个月(平均46.5个月)内继发创伤性关节炎或股骨头坏死,继而行全髋关节置换术。其中合并髋臼骨缺损6例,进行颗粒性植骨2例,结构性加颗粒性植骨4例;异位骨化4例,均发生于K-L入路。采用后外侧入路9例,髂股延伸切口2例,其中6例为原手术切口。结果11例患者全部得到随访,随访1年6个月~7年3个月,平均3年5个月。出现感染1例,脱位1例,无菌松动1例。术后再次异位骨化2例,其中1例引起坐骨神经症状,再次行神经松解术。所有随访病例髋关节功能均有改善,H arris评分由术前平均47分,提高到术后88分。结论选择正确的手术入路,适当处理异位骨化组织和内固定物,重建髋臼骨缺损,是髋臼骨折经切开复位内固定术后,再行全髋关节置换成功的关键。  相似文献

目的分析股骨近端骨折治疗失败的原因,探讨补救性全髋关节置换术(THR)的疗效。方法2002年2月~2005年4月,对23例股骨近端骨折治疗失败的患者(股骨颈骨折17例,股骨转子部骨折5例,股骨头骨折1例)进行回顾性研究,分析其前期治疗失败的原因,总结此23例患者进行补救性THR后的疗效。结果股骨近端骨折治疗失败的主要原因是骨折后复位不良、内固定方式选择不当或技术错误。21例患者获平均32个月(8~46个月)随访。无感染、脱位及假体周围骨折等并发症发生。21例患者髋关节功能均有改善,Harris评分由术前平均48.3分提高到术后87.6分。结论骨折复位、内固定方式或固定技术对股骨近端骨折的治疗非常重要。老年患者的骨质疏松和年轻患者的高能量损伤也增加股骨近端骨折治疗的难度。对于股骨近端骨折治疗失败的患者,进行补救性的THR重建髋关节功能临床疗效满意。  相似文献

目的分析髋臼骨折术后全髋关节置换术的早期临床效果。方法2002年1月至2007年12月,共对18例髋臼骨折后创伤性髋关节关节炎的患者进行了全髋关节置换术,其中13例髋臼骨折做过切开复位内固定术,另5例行闭合复位骨牵引治疗;10例因为既往手术的疤痕粘连,关节置换时需行较广泛的软组织松解;9例在全髋关节置换术时仍有固定用的内植入物,5例术中需要取出内固定;12例术中用取下的股骨头行结构性或颗粒植骨;16例用非骨水泥髋臼,2例用金属加强环与骨水泥固定聚乙烯髋臼。术后随访行x线摄片评估,髋关节功能作Harris评分。结果随访时间6—70个月不等,平均38个月。3例术后发生髋关节周围BrookerⅡ度异位骨化,其中2例行异位骨化切除;1例术中股骨骨折,1例深部感染,1例髋臼松动并移位。最后随访时,12例(67%)患髋没有或仅偶有轻微疼痛,4例中度疼痛,2例疼痛需要助行工具;Harris评分由术前平均43.5分到术后平均84分。结论髋臼骨折后髋关节骨关节炎仍可通过全髋关节置换手术得到有效治疗,但其手术复杂程度与款臼骨折的处理有关,且并发症的发生率比一般的全髋关节置换手术高。  相似文献

Treatment of fractures of the femoral neck by closed reduction and internal fixation results in a high incidence of complications, and often requires secondary total hip arthroplasty. We retrospectively studied 31 patients who underwent a Stanmore (Howmedica) total hip arthroplasty performed five months (median) after osteosynthesis. The most common indications were: failure of fixation (n = 14), osteonecrosis (n = 9) and secondary osteoarthritis (n = 6). Eight patients had a maximum follow-up of 12 months; the median follow-up was 30 months in the remaining group of 23 patients. Although one patient had radiographical signs of femoral loosening, none of the patients studied needed revision of the total hip arthroplasty. Using the Merle d'Aubigné scoring system, we found excellent results in 94% of the cases. Despite the short-term follow-up and the small number of patients, we conclude that the Stanmore THA was a satisfactory salvage procedure after failure of internal fixation for femoral neck fracture.  相似文献

The treatment of acetabular fractures in part relies on the selection of a specific surgical approach that allows for accurate reduction of fracture fragments. Moreover, these acetabular approaches were not developed for the insertion of a total hip replacement. Therefore, if a total hip arthroplasty is to be the treatment of an acute acetabular fracture, a single incision that permits reduction of the acetabular fracture fragments and ease of insertion of the arthroplasty components would be desirable. The Levine anterior approach provides both accesses to the anterior wall/column for reduction and fixation and to the femoral shaft for insertion of a total hip replacement. This paper describes that surgical technique and our initial clinical experience with this approach for acute acetabular fractures. A consecutive group of 10 patients with acetabular fractures, all involving the anterior wall/column with articular impaction (>50% of the acetabular roof) including 2 cases with an associated posterior hemitrans-verse component, were reviewed. After fracture reduction and fixation, a hybrid total hip replacement was implanted in all cases with an average acetabular component size of 56 mm (range 52-64). At a mean follow-up of 36 months (range 24-53), all fractures united, and all acetabular components remained fixed with no evidence of migration or loosening. There were 2 complications, a Brooker grade II heterotopic ossification and 1 postoperative anterior dislocation treated successfully with closed reduction and spica cast immobilization. The average Merle d'Aubigné hip score at latest follow-up was 16 (range 13-18). The Levine anterior approach is a reliable, safe, and efficient technique that permits early mobilization of patients with anterior wall/column acetabular fractures requiring a total hip replacement.  相似文献

BACKGROUND: A periprosthetic fracture around the femoral component is a rare but potentially problematic complication after total hip arthroplasty. Reconstruction can be challenging, especially when severe bone stock deficiency is encountered. Proximal femoral replacement is one method of treating the severely deficient proximal part of the femur. The present report describes the outcomes of revision total hip arthroplasty with use of a proximal femoral replacement in a cohort of patients who had a Vancouver type-B3 periprosthetic fracture. METHODS: With use of a computerized institutional database, all patients in whom a Vancouver type-B3 fracture (characterized by severe proximal bone deficiency and a loose femoral stem) had been treated with a proximal femoral replacement were identified. A modular femoral replacement with proximal porous coating had been used in all cases. The twenty-one patients who were identified had had a mean age of 78.3 years (range, fifty-two to ninety years) at the time of the index operation. The clinical and radiographic records of these patients were reviewed. RESULTS: At the time of the latest follow-up (mean, 3.2 years), all but one of the patients were able to walk and had minimal to no pain. Complications included persistent wound drainage that was treated with incision and drainage (two hips), dislocation (two hips), refracture of the femur distal to the stem (one hip), and acetabular cage failure (one hip). CONCLUSIONS: Despite a relatively high complication rate, we believe that proximal femoral replacement is a viable option for the treatment of periprosthetic fractures in older patients with severe bone deficiency. If a proximal femoral replacement is used, the stability of the hip must be tested diligently intraoperatively and a constrained acetabular liner should be utilized if instability is encountered. In order to enhance the bone stock, the proximal part of the femur, however poor in quality, should be retained for reapproximation onto the implant.  相似文献

Fifteen patients who had undergone hip reconstruction using a proximal femoral allograft-prosthesis composite because of failed total hip arthroplasty were reviewed retrospectively. The average length of allograft was 11 cm. The average follow-up period was 7.6 years. Ten patients retained the allograft-prosthesis construct, yielding a success rate of 67% for the procedure. The average preoperative Harris Hip Score of all patients was 21.7 points. At the latest follow-up, the average hip score for 10 patients with retained allograft-prosthesis composite was 81 points. Complications included junction nonunion (13%), allograft fracture (6.7%), loosening of acetabular component (6.7%), trochanteric escape (26.7%), allograft infection (20%), and allograft resorption (20%). Although proximal femoral allograft-prosthesis composite achieved good clinical results in most of the patients at medium-term follow-up, the late development of infection and resorption of the allograft warranted continued observation of patients who had undergone such procedures.  相似文献

Cementless total hip replacement techniques are increasingly used in revision arthroplasty. A major challenge is to achieve implant stability in a femur distorted by a failed arthroplasty. Five patients with aseptic loosening of cemented primary or revision total hip replacements complicated by significant proximal femoral disease, four with marked angular deformity of the femur and one with a subtrochanteric nonunion, were treated successfully with cementless revision arthroplasty combined with proximal femoral osteotomy. At follow-up examination, all femoral and trochanteric osteotomies had healed and D'Aubigne and Postel scores for pain, function, and range of motion had improved. All porous prostheses demonstrated radiographic features consistent with bone ingrowth fixation. No progressive stress shielding has been observed. Concomitant femoral osteotomy to correct anatomic deformity, in association with cementless total hip arthroplasty, results in union of the osteotomy and restoration of hip function.  相似文献
